Biomechanics is often overlooked when dealing with orthopedic injuries, whether regarding prevention or treatment, and practicing surgeons and surgeons-in-training may feel overwhelmed when referring to a book with a more complicated basic science approach. In order to make the subject clinically relevant to orthopedic trauma surgery, this unique text presents numerous clinical case examples to demonstrate clearly and effectively the principles biomechanics of injury, fixation and fracture healing. Divided into five sections, the opening chapters cover the essentials of stress and strain relevant to bone and joints and how this relates to fractures and their healing, complete with illustrative case material. This case-based approach is carried throughout the book, with part two discussing biomechanical principles of external fixation for diaphyseal and periarticular fractures, limb lengthening and deformity correction. Tension band wiring for both olecranon and patella fractures are covered in part three, and both locking and nonlocking plates are illustrated in part four. The final section describes biomechanical principles of intramedullary nails for a variety of fractures and nonunions, as well as arthrodesis and lengthening. Generous radiological images and intraoperative photos provide a helpful visual enhancement for the clinical material. This book is intended as a state-of-the-art reference guide that will help surgeons arrive at the correct diagnosis, make appropriate treatment decisions, and perform various surgical procedures in patients with intra-articular hip fractures. The volume describes the complex anatomy of the acetabulum and proximal femur and correlates it with appearances in radiological imaging. In addition, it explains the process of assessing and classifying typical fracture patterns, presenting and discussing specific, up-to-date treatment strategies. The book clearly highlights the advantages and disadvantages of several surgical approaches for the hip and pelvis, such as surgical hip dislocation, hip arthroscopy, the pararectus approach, and combined approaches. Individual chapters are also devoted to the management of specific fracture types: acetabular fractures, femoral head (Pipkin) fractures, femoral neck fractures, traumatic hip dislocations, and pathological fractures due to osteoporosis or tumors. Distal radial fractures are still puzzling us 200 years after Colles' classic description. Despite the advent of various new imaging techniques, including 3D CT, the availability of diagnostic adjuncts such as arthroscopy, and treatment options that include more than 30 different volar locking plates, the rate of unsatisfactorytreatment resultsremains very high. Furthermore, while we are aware of more associated injuries, such as carpal ligament tears and injuries to the radioulnar ligament (TFCC), there is still intense debate on the relevance of these injuries. Abstract
When it was developed, the Ilizarov device represented a revolutionary advance that made it possible to correct previously untreatable conditions through the stimulation of bone growth based on the principle of distraction osteogenesis. The device subsequently gained popularity among surgeons throughout the world since it can be used for the treatment of fractures that have failed to heal satisfactorily and for deformity correction. The technique is, however, complex and requires specialist knowledge if it is to be applied optimally.This is the second edition of a well-received book that focuses primarily on external fixation using the Ilizarov device but also considers other devices employed for the purpose. The opening chapters include discussion of biomechanical principles, use of a system of coordinates to allow safer insertion of K-wires and half pins, preoperative preparation, and principles of frame construction. External fixation of a variety of fractures in different pathologic settings is then clearly explained in a series of detailed chapters with the aid of high-quality illustrations. Numerous case reports are included to illustrate the results of different treatment methods. In addition, principles of postoperative management are described and advice is provided on correction of errors and treatment of complications. Since the first edition the text has been thoroughly updated, with inclusion of contributions from leading world experts. This volume will serve as an indispensable manual both for trainee orthopedic surgeons embarking on a steep learning curve and for more experienced surgeons requiring advice and guidance in demanding cases.
