It points to continuing gaps in knowledge, and explores the potential for further advances in the design of objective monitoring devices. Epidemiologists have studied relationships between questionnaire assessments of habitual physical activity and various medical conditions for some seventy years. In general, they have observed positive associations between regular exercise and good health, but because of inherent limitations in the reliability and accuracy of physical activity questionnaires, optimal exercise recommendations for the prevention and treatment of disease have remained unclear. Inexpensive pedometers and accelerometers now offer the epidemiologist the potential to collect relatively precisely graded and objective information on the volume, intensity and patterns of effort that people are undertaking, to relate this data to past and future health experience, and to establish dose/response relationships between physical activity and the various components of health. Such information is important both in assessing the causal nature of the observed associations and in establishing evidence-based recommendations concerning the minimal levels of daily physical activity needed to maintain good health.

Another reason is that too many approaches focus on the math without explaining the clinical relevance of these principles and the laboratory testing that enables us to measure the very lung function that these principles are describing. This book answers that need by providing a series of chapters that guide the reader in a natural order of learning about the respiratory system. In particular, after a general overview of the structure-function design of the lung and the history of pulmonary function testing, authors begin with the drive to breathe, and then follow the pathway of air as it is drawn into the lung, undergoes gas exchange, and is then exhaled back out again. However, even if global macrocirculatory variables are restored, abnormalities in tissue oxygenation may persist. Tissue hypoperfusion is connected to the development of organ failure and, if it goes unrecognized, may worsen the prognosis. As a result, there is a growing interest in methods for monitoring regional perfusion in peripheral tissues to predict or diagnose ongoing hypoperfusion. In this work, eminent experts from a range of disciplines convey a working knowledge of how regional monitoring in shock can complement the conventional global parameters of oxygen transport, and demonstrate that real-time bedside assessment of tissue oxygenation is readily achievable using noninvasive monitoring techniques.
