Abstract
This book presents the basic theoretical and historical concepts and it describes current perspectives and data, focusing on good practices in community psychiatry in Greece and in other parts of Europe. Concepts such as the biopsychosocial model, psychiatric reform, psychosocial rehabilitation and the recovery model, as well as new case management models are approached from a critical, anthropocentric perspective. The current socioeconomic crisis in Europe brings with it new realities in mental health systems. New forms of social suffering are forcing the psychiatric community to re-examine what is considered normal. In order to respond to the complexity of the newly emerging needs, social and community psychiatry has been compelled to broaden the objectives of intervention and research alike, developing new and dynamic relations with complementary scientific fields such as social anthropology, psychoanalysis and microeconomics. The present work is the result of collaboration between professionals from across these different fields.

Abstract
This book introduces novel and groundbreaking theories on social medicine, social medicine therapy and pharmacogelotology. Aimed at improving the global health care system in terms of cost-effectiveness and efficiency, the research included in this book represents a paradigm shift from traditional drugs to social medicine. Tracing the history of social medicine, from Natural Healing Power (NHP), Oriental Medicine's vitalism, to Homeostasis (Natural Healing Strength) and Reciprocity (Social Healing Strength), the book first focuses on laying the theoretical foundations. It then highlights how social medicine can be specialized into various social medicine therapies (i.e., aromatherapy, stone therapy, diet therapy, exercise therapy, light therapy, etc.), just like stem cells. This is followed by arguments that 21st century pharmacy should be a harmonious system where the replacement of traditional drug products (i.e., herbal, chemical, and biological products) with new social medicine takes precedence. To that end, the author focuses on the '4+2 system' with 4 representing diet, body, stress, and facial-image control, and 2 representing the complementary and alternative medical methods of evacuation(-) and filling(+). In the context of pharmacogelotology, the book then goes on to present findings on theories of laughter and laughter therapy practices, which are systematically examined and described in detail. Finally, it calls for the development of social medicine structures by governments that aim to help local authorities use their resources effectively, and for local governments to establish the long-term planning on social medicine therapy for healthy ageing.

Abstract
As America's most populous state goes, so goes the nation. And within that state, the 38 counties comprising South Texas--a highly populated, largely Hispanic area characterized by high uninsured rates and low numbers of health care providers--are emblematic of nationwide crises in public health. The South Texas Health Status Review: A Health Disparities Roadmap brings health care disparities to light in instructive detail. Over thirty key health conditions and risk factors, such as breast cancer, diabetes, obesity, tuberculosis, smoking and alcohol use, lead exposure, birth defects, and homicide rates, are analyzed by age, sex, race/ethnicity, and location (urban or rural). The results are compared with the rest of Texas and the nation as a whole. A section on conclusions and recommendations outlines factors contributing to these conditions and offers suggestions for improvement. This important data should stimulate further population studies and provide impetus for targeted public health interventions and community outreach programs to reduce health inequities and advance care access in the state and the U.S. at large. Major health issues covered in the report include: communicable diseases, cancer incidence and mortality, maternal and child health, chronic diseases, behavioral risk factors in adults, environmental health issues, suicide, homicide, and vehicular mortality. The South Texas Health Status Review contains valuable lessons for researchers in health promotion and health disparities, as well as community health professionals. Policymakers involved in reducing health disparities will find it especially useful.

Abstract
More people in communities of color are contracting, living with, and being treated for HIV/AIDS than ever before. In 2005, 71% of new AIDS cases were diagnosed in people of color. Though the number of racial and ethnic minority HIV/AIDS cases continues to grow, the health care community has been unable to adequately meet the unique medical needs of these populations. African-American, Latino/Latina, and other patients of color are less likely to seek medical care, have sufficient access to the health care system, or receive the drugs they need for as long as they need them. HIV/AIDS in U.S. Communities of Color is an outstanding resource for any professional working with these underserved populations. Comprehensive chapters cover issues as far-reaching as: anti-retroviral therapy; dermatologic manifestations and co-morbidities of the disease in patients of color; unique risks to women and MSMs of color; participation of minority cases in HIV research; and substance abuse and mental health issues. Complied by world experts in HIV/AIDS, this book analyzes the main causes of treatment failure and promotes respect for individual and cultural values, teaching readers to provide responsive, patient-centered care and devise preventive strategies for communities of color.

Abstract
This stimulating volume uses multiple lenses to analyze the complex causes of health disparities affecting minorities, in particular African Americans, and explains how this knowledge can be used to reduce their destructive effects. Pinpointing genetic, non-genetic, and epigenetic factors underlying health conditions common to the population--including heart disease, hypertension, diabetes, and cancer--the author traces intricate links among these factors in the current environmental and social context. The section on non-genetic factors in health disparities, such as social determinants and health behaviors, adds depth to the ongoing discourse on public health and health policy objectives. And the chapters on gene/environment interactions outline the vast potential for developing new multidisciplinary frontiers in shrinking health inequities and personalizing care. Included in the coverage: The African diaspora and disease-specific disparities; The genetic basis to health disparities; The role of epigenetics; Economic factors and health; Psychological issues and how they affect disparities; Gene-environment interactions in health disparities; Race, a biological or social concept. Compelling and accessible, Health Outcomes in a Foreign Land will challenge and inspire medical students, epidemiologists, public health professionals, biomedical research scientists, and social scientists to go farther in their work. A wider audience would include policymakers, government officials, nurses, physicians, lawyers, economists, community outreach investigators, and interested general readers.

Abstract
The vast majority of the global population has pigmented non-Caucasian skin; accordingly, this book focuses on the diagnosis and management of skin diseases in dark-skinned populations, paying particular attention to different reactive profiles, the frequency and the clinical pictures of diseases in pigmented ethnic skin that arise in hot climate zones. Supplemented by a wealth of high-quality, full-color images, this comprehensive work covers the full range of dermatological entities and issues characteristic of the tropical and subtropical regions of the globe, including those that are now emerging in developed countries as a result of intensified travel, globalized business, and migration. Each disorder is described by respected experts on subtropical and tropical countries, and each chapter is clearly structured, examining the incidence, pathogenesis, clinical presentation, management and course. The book illustrates the clinical features and includes major schedules for the diagnosis and management of skin diseases presented in darkly pigmented to black skin. As such, it offers an invaluable tool for all dermatologists in developing and developed countries, especially those who are used to dealing with skin lesions mainly in white Caucasian skin. While enabling physicians to provide proper advice, medical services and support, Pigmented Ethnic Skin and Imported Dermatoses also represents a guide for a broader range of professionals working for governmental institutions, health and refugee agencies.

Abstract
This comprehensive book thoroughly addresses all aspects of health care transition of adolescents and young adults with chronic illness or disability; and includes the framework, tools and case-based examples needed to develop and evaluate a Health Care Transition (HCT) planning program that can be implemented regardless of a patient's disease or disability.áHealth Care Transition: Building a Program for Adolescents and Young Adults with Chronic Illness and Disability is a uniquely inclusive resource, incorporating youth/young adult, caregiver, and pediatric and adult provider voices and perspectives. Part I of the book opens by defining Health Care Transition, describing the urgent need for comprehensive transition planning, barriers to HCT and then offering a framework for developing and evaluating health care transition programs. Part II focuses on the anatomic and neuro-chemical changes that occur in the brain during adolescence and young adulthood, and how they affect function and behavior. Part III covers the perspectives of important participants in the HCT transition process—youth and young adults, caregivers, and both pediatric and adult providers. Each chapter in Part IV addresses a unique aspect of developing HCT programs. Part V explores various examples of successful transition from the perspective of five key participants in the transition process - patients, caregivers, pediatric providers, adult providers and third party payers. Related financial matters are covered in part VI, while Part VII explores special issues such as HCT and the medical home, international perspectives, and potential legal issues. Models of HCT programs are presented in Part VIII, utilizing an example case study. Representing perspectives from over 75 authors and more than 100 medical centers in North America and Europe,áHealth Care Transition: Building a Program for Adolescents and Young Adults with Chronic Illness and Disability is an ideal resource for any clinician, policy maker, caregiver, or hospitalist working with youth in transition.

Abstract
This salient volume surveys the state of access to primary care and preventive health services by migrants, refugees, and asylum seekers across Europe. Experts in public health and allied fields identify obstacles to healthcare interventions for migrants, including costs, legal status, health-related behaviors and beliefs, and cultural and language barriers. The book includes the latest data concerning access to specific preventive services (e.g., vaccinations, colorectal screenings), specific issues of women and sexual minorities, and the potential for health promotion in prevention. Best practices for improving access are outlined as a basis for public health and policy directives toward reducing health disparities between migrant and native populations. Among the topics covered: Access to medical examination for prevention among migrants Access and barriers to infant vaccinations, female cancer screening and colorectal screening among migrant populations; Provision and policy gap between the primary and preventive care required by and the care provided to LGBTQ+ migrants, refugees, and asylum seekers; Health related lifestyles and intermediate health conditions of migrants; Quality of primary healthcare and preventive health services provided to migrants; Adaptations of primary health care for migrants; Access to primary health care and policies on migration and health at a time of economic crisis. Dedicated to bridging research and policy gaps in this vital area, Primary Care Access and Preventive Health Services of Migrants is intended for an international audience of academics, researchers, policymakers, and practitioners in public health and related disciplines.

Abstract
This volume tells the history of homosexuality in the United States military beginning in 1986, when the issue first came to the forefront of social consciousness. Each chapter is written through the eyes of gay mental healthcare providers, covering how to steadily adapt and learn to treat veterans struggling with the traumas associated with the stigma of homosexuality in service. Topics include the "Don't-Ask-Don't-Tell" (DADT) policy, its repeal in 2011, and addresses the current trends and challenges. Unlike any other professional book, this text includes the personal stories of gay military mental healthcare providers, as well as gay civilian clinicians who have worked with the military population in various segments in history. These accounts offer invaluable support for medical professionals working with this demographic. Chapters cover the various psychological damage service personnel encounter as it uniquely pertains to those struggling with the stigma of LGBTQ rights. Chapters include clinical pearls for particular psychiatric concerns, lessons learned for the future, and hard-earned successes as stigmas and perceptions evolved over time. Gay Mental Healthcare Providers and Patients in the Military is an excellent resource for psychiatrists, psychologists, social workers, policymakers, and all professionals who are interested in LGBTQ rights in the context of veteran psychiatry.

Abstract
This book provides a comprehensive review of the complex, growing mental health challenges faced by culturally diverse populations of children and adolescents. Suicide Among Diverse Youth: A Case-Based Guidebookáis the first book of its kind, and is designed specifically to bridge the knowledge and skills gap encountered by most clinicians dealing with youth from diverse cultural backgrounds, particularly those different than that of the clinician. The title begins with two introductory chapters, which cover cultural aspects of suicidality among youth, culturally informed treatment of suicidality with diverse youth, and exampl es of preventative approaches. These are followed by population specific chapters which cover a broad spectrum of diverse populations, including underserved ethnic and racial populations in the United States, LGBTQ youth, as well as various immigrant populations from Eastern European and Middle Eastern countries. These case-based chapters are structured in a cohesive, easy-to-read format that promotes ease of reference, beginning with a clinical case report, review of literature, unique characteristics and risk factors associated with suicidality, and evidence-based practice provided by the authors from their considerable experience. The authors are often from the same ethnic, racial, or cultural group that they discuss in their writings; providing experiential knowledge where scientific knowledge is lacking. Suicide Among Diverse Youth: A Case-Based Guidebook is a unique resource that offers the clinical material needed to treat diverse adolescent patients with sensitive, intersectional, and culturally-informed care, and will provide an indispensable resource for medical professionals working with, and caring for these patients.

Abstract
This unique resource offers an in-depth, comprehensive look at different types of mental health needs of transgender and gender diverse youth, how these intersect with gender identity, gender expression, and sexual orientation, and provides practical information on how to ethically, responsibly, and sensitively care for these patients. Affirmative Mental Health Care for Transgender and Gender Diverse Youth: A Clinical Guide begins with three introductory chapters which contain practical information regarding assessment, psychological interventions, and the potential medical and surgical interventions that are indicated for youth with gender identity concerns. The remaining chapters are illustrated by multiple cases build around overarching chapter themes. Each case chapter opens with broad questions applicable to clinical practices, while the cases themselves focus on a particular co-occuring mental health condition. The case chapters are structured with intersectionality in mind, including elements of ethnic, racial, and cultural diversity, and the patients range over the full developmental spectrum, from pre-pubertal children to older adolescents. cases range in complexity as well, to provide readers with the tools they need to evaluate patients, and to assist in the decision of which presenting factors to prioritize in treatment at which time. Ending each chapter are clinical take-home messages, closing with additional practical knowledge that can be applied to other cases providers may see in their own practices. Written by expert clinicians in the field, Affirmative Mental Health Care for Transgender and Gender Diverse Youth: A Clinical Guide is an ideal resource not only for child and adolescent psychiatrists, but for clinicians across all mental health disciplines working with gender non-conforming youth, and who are interested in providing informed, affirmative, and intersectional care.

Abstract
This ambitious sourcebook surveys both the traditional basis for and the present state of indigenous women's reproductive health in Mexico and Central America. Noted practitioners, specialists, and researchers take an interdisciplinary approach to analyze the multiple barriers for access and care to indigenous women that had been complicated by longstanding gender inequities, poverty, stigmatization, lack of education, war, obstetrical violence, and differences in language and customs, all of which contribute to unnecessary maternal morbidity and mortality. Emphasis is placed on indigenous cultures and folkways--from traditional midwives and birth attendants to indigenous botanical medication and traditional healing and spiritual practices--and how they may effectively coexist with modern biomedical care. Throughout these chapters, the main theme is clear: the rights of indigenous women to culturally respective reproductive health care and a successful pregnancy leading to the birth of healthy children. A sampling of the topics: Motherhood and modernization in a Yucatec village Maternal morbidity and mortality in Honduran Miskito communities Solitary birth and maternal mortality among the Rara´muri of Northern Mexico Maternal morbidity and mortality in the rural Trifino region of Guatemala The traditional Nga¨be-Bugle´ midwives of Panama Characterizations of maternal death among Mayan women in Yucatan, Mexico Unintended pregnancy, unsafe abortion, and unmet need in Guatemala Maternal Death and Pregnancy-Related Morbidity Among Indigenous Women of Mexico and Central America is designed for anthropologists and other social scientists, physicians, nurses and midwives, public health specialists, epidemiologists, global health workers, international aid organizations and NGOs, governmental agencies, administrators, policy-makers, and others involved in the planning and implementation of maternal and reproductive health care of indigenous women in Mexico and Central America, and possibly other geographical areas.

Abstract
This book is unique in its global approach to applying the Guidelines for Culturally Competent Nursing Practice that were recently endorsed by the International Council of Nurses (ICN) and distributed to all of its 130 national nursing associations. The purpose of this book is to illustrate how these guidelines can be put into clinical practice and to show how practitioners from different countries with diverse populations can implement them. The first chapter provides the conceptual basis for Culturally Competent Health Care and describes how the guidelines were developed. Each of the next 10 sections presents a chapter describing a specific guideline followed by three or four chapters with detailed case studies to illustrate how the guideline was implemented in a particular cultural setting. All case studies follow a similar format and are written by international authors with clinical expertise and work experience in the culture being presented. This book will be useful for advanced practice nurses, healthcare students, clinicians, administrators, educators, researchers, and those who provide community health or population-based care.

Abstract
‘This is an incredibly audacious book. Derek Bolton and Grant Gillett brilliantly succeed in providing the big picture that was lacking in the defense of the biopsychosocial model promoted by Engel 40 years ago.’ - Steeves Demazeux, Assistant Professor in philosophy, Bordeaux-Montaigne University, France This open access book is a systematic update of the philosophical and scientific foundations of the biopsychosocial model of health, disease and healthcare. First proposed by George Engel 40 years ago, the Biopsychosocial Model is much cited in healthcare settings worldwide, but has been increasingly criticised for being vague, lacking in content, and in need of reworking in the light of recent developments. The book confronts the rapid changes to psychological science, neuroscience, healthcare, and philosophy that have occurred since the model was first proposed and addresses key issues such as the model’s scientific basis, clinical utility, and philosophical coherence. The authors conceptualise biology and the psychosocial as in the same ontological space, interlinked by systems of communication-based regulatory control which constitute a new kind of causation. These are distinguished from physical and chemical laws, most clearly because they can break down, thus providing the basis for difference between health and disease. This work offers an urgent update to the model’s scientific and philosophical foundations, providing a new and coherent account of causal interactions between the biological, the psychological and social. Derek Bolton is Professor of Philosophy and Psychopathology at the Institute of Psychiatry, Psychology and Neuroscience at King’s College London, UK. He was awarded a double first in Moral Sciences and a PhD in Philosophy at the University of Cambridge. His subsequent professional career was in clinical psychology and he has published extensively in philosophy of psychiatry as well as basic and clinical health science. Grant Gillett is Professor of Bioethics at the University of Otago, New Zealand. His professional career was in Neurosurgery, punctuated by a D. Phil. at Oxford University in philosophy of mind and meta-ethics. He has published extensively in neuroethics, philosophy of mind and language, philosophy of medicine (particularly psychiatry), and the philosophy of medical and social sciences.

Abstract
This open access book introduces the human development model to define disability and map its links with health and wellbeing, based on Sen’s capability approach. The author uses panel survey data with internationally comparable questions on disability for Ethiopia, Malawi, Tanzania and Uganda. It presents evidence on the prevalence of disability and its strong and consistent association with multidimensional poverty, mortality, economic insecurity and deprivations in education, morbidity and employment. It shows that disability needs to be considered from multiple angles including aging, gender, health and poverty. Ultimately, this study makes a call for inclusion and prevention interventions as solutions to the deprivations associated with impairments and health conditions.

Abstract
This second volume in the Food Policy series focuses on critical nutrition and dietetics studies, offering an innovative and interdisciplinary exploration of the complexities of the food supply and the actors in it through a new critical lens. The volume provides an overview of the growth of critical nutrition and dietetics since its inception in 2009, as well as commentary on its continuing relevance and its applicability in the fields of dietetic education, research, and practice. Chapters address key topics such as how to bring critical dietetics into conventional practice, applying critical diets in clinical practice, policy applications, and new perspectives on training and educating a critical nutrition and dietetic workforce. Contributing authors from around the globe also discuss the role of critical nutrition dietetics in industry, private practice, and consultancy, as well the role of critical dietetics in addressing the food, hunger, and health issues associated with the world economic crisis. The authors designed the volume to be a reference work for students enrolled in undergraduate and postgraduate courses in Critical Nutrition, Critical Food Studies, and Critical Dietetics. Each chapter offers concise aims and learning outcomes, as well as assignments for students and a concise chapter summary. These features enhance the value of the volume as a learning tool.
