Ultrasound guidance of liver surgery is a very sophisticated approach that permits the performance of otherwise unfeasible operations, discloses the true extent of tumors, increases the indications for hepatectomy, and renders surgery safer. Despite this, it has remained relatively neglected in the literature over the past two decades, during which time much progress has been achieved. This is the first atlas on the subject, and it is comprehensive in scope. The state of the art in the use of ultrasound for resection guidance is carefully documented, and new techniques for exploration of the biliary tract and facilitation of transplant surgery are presented. Further important topics include the role of ultrasound in laparoscopic approaches, the use of contrast agents for diagnosis and staging, and developments in the planning of surgical strategy. The editor is a leading authority whose group has been responsible for a variety of advances in the field. He has brought together other experts whose aim throughout is to provide clear information and guidance on the optimal use of ultrasound when performing liver surgery. Single-access laparoscopic surgery (SALS), performed using a single transumbilical access, has recently proved to bring advantages over other forms of laparoscopic surgery due to the reduction in the number of incisions and the consequent decrease in morbidity, less blood loss and postoperative pain. Furthermore, cosmesis may be improved because incision within the umbilicus renders surgery virtually scarless. Nevertheless, as with all new surgical concepts, questions may be raised regarding safety, usefulness, and appropriateness. This book analyzes the use of SALS for different indications and abdominal surgical procedures based on a careful literature review of the experience of the most important surgical teams employing this approach and also the authors' personal observations. The coverage is broad, encompassing, for example, the role of SALS in appendectomy, repair of abdominal wall defects, cholecystectomy, gastric surgery, nephrectomy, splenectomy, hemicolectomy, and pancreatic, liver, and gynecological surgery.

Hysteroscopic instrumentation is described and the application of hysteroscopic techniques in patients with polyps, fibroids, malformations, and infertility is explained and evaluated. Individual chapters are devoted to the current minimally invasive treatment of endometriosis, endometrial polyps, and adenomyosis. The role of laparoscopy in gynecologic oncology and the laparoscopic approach to hysterectomy are fully explored, and the relative merits of laparoscopic supracervical hysterectomy and total laparoscopic hysterectomy are evaluated. Various other applications of laparoscopy are considered, with coverage of techniques including traditional straight stick laparoscopy, barbed suture, and robotic and single port surgery. Neurophysiology and the use of laparoscopic therapy to restore vital function are discussed, and complications of laparoscopic surgery are also addressed. Endovascular neurosurgery is a recently introduced but rapidly evolving medical field, which uses minimally invasive interventions to treat major life-threatening vascular lesions of the Central Nervous System. Although its history counts less than 15 years of worldwide acceptance, it has rapidly displaced the traditional open neurosurgical techniques, being nowadays the first treatment choice for brain aneurysms and vascular malformations. Thus, the experience of each neuroendovascular center and performer is invaluable, offering the base for learning and teaching the new generation of interventionalists as well as for the evolvement of the method itself. This book presents the basic principles of endovascular neurosurgery starting from clinical cases. Through this close-to-clinical-reality-process, the reader will be able to more thoroughly understand the pathophysiology of the brain and spine vascular lesions as well as the decision-making strategy, related to the indications, endovascular methods and results, finding suggestions and solutions to his/her clinical questions and problems. Besides chapters devoted to CNS vascular embryology and anatomy, clinical cases organized in groups based on the treated lesions are introduced: ruptured and unruptured cerebral aneurysms of the anterior and posterior circulation, side-wall and bifurcation aneurysms, arteriovenous malformations (AVM), dural arteriovenous fistulae (dAVF), arterial stenosis and angioplasty as well as spinal vascular lesions. A separate chapter is devoted to the organization and necessary equipment of the angio room and the department offering neuroendovascular service. Abstract
This handy book offers a complete overview of current minimally invasive techniques for the management of endocrine diseases of the neck, reflecting how, in the past few years, the development of various ultrasound-guided procedures and endoscopic/video-assisted surgical techniques has changed the approach to thyroid and parathyroid diseases. The book opens by describing the main diagnostic tools for thyroid, parathyroid, and cervical lymph nodes, focusing especially on the new technologies for tissue characterization. The different minimally invasive treatment procedures involving either a percutaneous approach (ethanol injection, laser thermal ablation, radiofrequency thermal ablation, high-intensity focused ultrasound) or minimally invasive surgery are then described in detail. The volume is intended for all clinicians and surgeons with an interest in the field of endocrinology and neck surgery. It provides a synthetic but clear description of the various available techniques from a technical point of view and discusses the main issues concerning each of them, permitting comparative evaluation in order to identify the best option in a particular case.
