It presents a detailed cross-section of patients across all age groups, with different etiologies of the disease and possible complications, to present sensible management scenarios to physicians treating patients with hypoparathyroidism. The cases presented include the management of acute and chronic hypocalcemia following neck surgery, as well as autoimmune, syndromic, idiopathic, and autosomal dominant etiologies. Conventional management with calcium and active vitamin D as well as parathyroid hormone therapy are discussed. In addition, cases are presented highlighting the chronic manifestations of the disease, including complications involving the renal, neurologic, and skeletal systems, as well as cataracts and quality of life. In the first section of the book examining epidemiology, the contributors review controversies over the definition of insulin resistance in children and what is known about how insulin resistance in youth differs from adults, the measurement of insulin resistance in youth in the research and clinical settings, and current knowledge regarding the epidemiology of insulin resistance in the pediatric population. The second section of the book explores pathophysiology, including current knowledge of the molecular, metabolic, and physiologic mechanisms of insulin resistance, the unique pathophysiology of pregnancy and puberty, the contributions of the prenatal and early childhood environment to the development of insulin resistance, and adipose and biochemical mediators. This section concludes with discussion of the relationship between insulin resistance and cardiovascular disease and liver disorders. A third section, new to this second edition, explores insulin resistance in unique models: intrauterine growth restriction and girls with polycystic ovary syndrome and metabolic syndrome. The final section of the book explores the concepts of treatment through medications directed at insulin sensitivity, as well as exercise, weight loss medications and consequences of bariatric surgery. In recent years, several scientific papers have highlighted the role of the thyroid system in cardioprotection, and several clinical studies – including multicenter studies – have identified the role of thyroid hormones (TH) in the development and progression of cardiovascular disease, making an update of TH and heart relationship both necessary and timely. This second edition of Thyroid and Heart failure provides a careful and updated review of the experimental, clinical and epidemiological results in the field of TH and heart failure, with a particular focus on the translational - bench to bedside - significance of the obtained results. It discusses topics such as the molecular, structural, functional, cellular and histological cardiac changes; the reversibility of these changes with TH replacement therapy; the systemic effects – kidney, hormonal pathways systems, brain, muscle, etc – of altered TH metabolism in HF, as well as the clinical and prognostic implications. The opening chapters discuss the biology, genetics and current standard of care for females with Turner syndrome in order to provide proper background and context for the remaining chapters. The main section of the book, taking a body system approach, is comprised of chapters written by an expert in his or her subspecialty and will discuss pathophysiology and diagnosis, as well as therapeutic options. Reproductive, cardiac, renal, endocrine, neurologic, musculoskeletal, and sensory issues and symptoms associated with Turner syndrome are all covered in detail. Additional chapters describe current resources available to both caregiver and patient as well as future directions for research and management. Currently, there are no similar books on the market that take an all-inclusive, multidisciplinary approach in the care of individuals with Turner syndrome. As such, this book should be the standard of care for management of this challenging and multifaceted condition.

Specific physical examination findings in each primary endocrine disease are reviewed and grouped under the categories of clinical features and pathophysiology, followed by related clinical and pathophysiology pearls. A brief description of the clinical finding is followed by a summary of the pathophysiologic mechanisms underlying the stated clinical finding. Relevant normal physiology and pathophysiology are reviewed where applicable, to facilitate learning. There are numerous texts on the pathophysiologic basis of diagnostic and laboratory findings in endocrinology. This Atlas is designed to illustrate different techniques on how to perform successful parathyroidectomy by using traditional four gland exploration approach and minimally invasive approaches, such as the open minimally invasive approach, video-assisted approach, back-door approach, transoral endoscopic parathyroidectomy approach (TOEPVA), and endoscopic lateral parathyroidectomy approach. It illustrates removal of a right and left, and superior and inferior parathyroid glands. Written by renowned endocrine surgeons and experts in the field, each chapter begins with a case description that defines the main aspect of surgery. Each picture, which is taken intraoperatively, is accompanied by corresponding drawings for easier understanding of the anatomical structures and steps of the procedure. In addition, most of the authors provided a video of the same case as it is depicted in the chapter. The Atlas also gives some common pitfalls of the procedure in an effort to avoid complications and improve patient outcomes. Abstract
This book provides a visual demonstration of normal and ectopic locations of parathyroid adenomas using different modalities in patients with PHPT and to describe parathyroid gland–related pathology. It includes several modern imaging modalities for localization of parathyroid glands and parathyroid adenomas, such as Sestamibi scan, SPECT/CT Sestamibi scan, neck ultrasound, MRI, thin-cut CT, and 4D CT scans. Written by experts in the field, chapters include pathology images corresponding to radiology imaging for some presented cases (gross and high-power view). Authors have also collected radiological images of difficult-to-localize parathyroid adenomas in ectopic (abnormal) locations. The atlas is organized by location of the adenomas in upper and lower eutopic locations followed by ectopic locations. Each case demonstrates dual or triple modalities such as US, Sestamibi scan, or SPECT/CT Sestamibi scan, thin-cut CT scan, or 4D CT performed on the same patient. A chapter on parathyroid pathology is also included to help the reader understand challenges in pathological interpretation.
