Abstract
This book presents up-to-date information on the clinical-pathophysiological features of acute renal injury and discusses the KDIGO diagnostic criteria, as well as novel experimental findings, including in the area of regenerative medicine. It also highlights the clinical-pathophysiological importance of AKI in clinical settings, including differential diagnoses and management of AKI. In the past, the pathology associated with sudden renal impairment was characterized as acute renal failure (ARF). However, in the 2000s, the joint efforts of specialists in fields including nephrology, intensive care medicine, and cardiovascular medicine led to the introduction of a novel concept known as acute kidney injury (AKI). As medical care progressed, patients such as high-risk elderly subjects who were not deemed to be candidates for invasive therapy came to be treated in intensive care units (ICUs). As a result, kidney injury as a subset of multiple organ failure was re-considered as AKI, especially in intensive care medicine. AKI was then proposed as a novel disease concept to emphasize the importance of early diagnosis and early intervention to improve prognosis. Presenting novel features, such as the definition of AKI, risk factors and management; biomarkers, such as neutrophil gelatinase-associated lipocalin (NGAL) and L-type fatty acid-binding protein (L-FABP); long-term outcomes of AKI; as well as renal regeneration using iPS cell, manipulation of embryonic genes, and Xenotransplanted embryonic kidney, this book is of interest to all physicians and researchers in this field around the globe.

Abstract

CKD is a global health problem with a rising morbidity and mortality. The last decade has seen significant improvements in determining the incidence, prevalence, and complications of CKD, mainly thanks to the definitions of CKD developed by the National Kidney Foundation’s Kidney Disease Outcomes Quality Initiative (K/DOQI). However, increased recognition of CKD has led to awareness of the limitations of its clinical diagnosis and treatment, which are essential to patients’ wellbeing. This book is of particular value not only to nephrologists, but also to general practitioners and residents with an interest in CKD. It offers a well-organized exposition of the current knowledge base. Compared with previously published books on kidney disease and CKD, it has a smaller number of more concise chapters. As a result, readers can easily obtain an overview of the most important topics in CKD. We hope that practitioners will gain as much from reading this practical guide to clinical management of CKD as we have from editing it. This book presents the latest information on the clinical-pathological features of diabetic kidney disease. The data included is based on a cohort study of biopsy-proven diabetic nephropathy patients and nephrosclerosis patients, who were observed over a long term, and on the long-term registry for diabetic nephropathy (diabetic kidney disease) in Japan. It provides a clinical-pathological axis in clinical settings, including differential pathological/clinical diagnoses of CKD in diabetic patients (e.g. the presence of “classic” diabetic nephropathy and/or nephrosclerosis and/or other primary kidney diseases). The abundant biopsy specimens with long-term medical records provide a detailed pathological and clinical description. The book also includes urine-sample data for developing and validating possible candidates for novel biomarkers for diabetic kidney disease. Many countries, including Japan, have ageing populations, in which nephrosclerosis contributes to the progression of kidney lesions in patients with diabetic kidney disease. As such, a comparison of a diabetic nephropathy cohort with nephrosclerosis is indispensable to offer better treatments. The cross talk between the heart and kidneys is highly relevant in the field of interventional cardiology, given the increasing number of trans-catheter procedures being performed in patients with underlying kidney disease. These procedures also have a significant impact on kidney function and require thoughtful interdisciplinary planning by a cardiorenal team, to achieve optimal outcomes This book assembles the collective expertise of several international leaders in the field of interventional cardiology and nephrology to summarize this complex interface. The book is divided into seven sections to comprehensively cover the topic, including sections on best practices with reduction of contrast associated acute kidney injury, cutting edge techniques to minimize kidney risk with complex interventions, impact of transcatheter valvular procedures on kidney function and the utility of cardio-nephrology teams. Full guidance is provided on preoperative evaluation and management and post-transplantation care. In addition, the pathology of antibody-mediated rejection and acute cellular rejection in this context is discussed.
