This book offers a comprehensive guide to the technical basis of laser ablation, describing and reporting in detail on the latest findings. The world of medicine is currently working to reduce the invasiveness of treatment, in order to improve patients’ quality of life. Image-guided ablations are rapidly becoming an effective alternative to several surgical treatments. Among the many techniques available for ablation, laser is still not widely used, though its efficacy has been amply demonstrated. The scientific community is now showing a growing interest in laser techniques for image-guided ablations, and many physicians are willing to start using lasers in their clinical practice. The book is divided into 16 chapters, including historical notes, technical aspects, outcomes of ex-vivo experiments, and results of the application of this technique in various clinical scenarios. It will be of great interest to a broad range of physicians (interventional radiologists, surgeons, gastroenterologists, endocrinologists, urologists), from less experienced trainees to expert physicians who want to introduce a novel clinical practice.

Abstract
This book provides a comprehensive guide to the treatment of small hepatocellular carcinoma (sHCC) using a minimally invasive technique: radiofrequency ablation (RFA). RFA has emerged as a new treatment modality and become the main modality of locoregional therapy. Extensive clinical research indicates that RFA is as effective as surgical resection for sHCC, and it has the advantage of being less invasive. However, the outcomes after RFA are largely dependent on the operators' experience--known as the "learning curve". This book presents the characteristics of sHCC and discusses why sHCC is the best candidate for RFA. Then it introduces all the commercially available RFA systems, and their working principles, advantages, disadvantages and so on. It goes on to demonstrate how to perform RFA under the guidance of ultrasound, CT, laparoscopy, or during open operation. Finally, it discusses the radiologic assessment and follow-up after RFA, as well as adjuvant therapies and clinical trials on RFA. The authors are experts from the fields of pathology, radiology, surgery, and gastroenterology, as well as manufacturers. With this book, readers gain have a clear idea of when and how to do RFA. It aims to standardize and generalize the procedure of RFA, which will be very helpful in improving the outcome of RFA for sHCC. Editor Minshan Chen, MD, PhD, is a Professor of surgery at the Department of Hepatobiliary Surgery, Sun Yat-sen University Cancer Center, Guangzhou, China. Thermal ablation has become an integral part of oncology, especially in the field of interventional oncology. This very comprehensive book encompasses the different technologies employed in thermal ablation, its indications and the results achieved in various clinical conditions. The first part of the book clearly explains the basics of thermal ablative techniques such as laser-induced thermotherapy, radiofrequency ablation, microwave ablation, cryotherapy, and localized tumor therapy. The latest developments in the application of minimally invasive therapies in localized neoplastic disease are demonstrated. In the main part of the book, techniques of guiding the applicators to the target structures by use of different imaging tools such as ultrasound, computed tomography and magnetic resonance imaging are discussed. The results are presented for a variety of clinical indications, including liver and lung tumors and metastases and some rather rare conditions involving the kidney, the head and neck, the prostate, and soft tissue structures. Abstract
Focal therapy is a promising option for selected patients who have localized low or intermediate-risk prostate cancer, providing a compelling alternative between active surveillance and radical therapies by targeting the index lesion and preserving as much tissue as possible. Numerous cohort studies have already investigated multiple focal techniques, such as cryotherapy, high-intensity focused ultrasound, brachytherapy, photodynamic therapy, laser therapy, irreversible electroporation and cyberknife methods, all of which have demonstrated positive oncological outcomes with 70 to 90 % negative follow-up biopsy. These various ablative techniques have produced only minor side-effects concerning urinary function, a low rate of erectile dysfunction, and have demonstrated a limited rectal toxicity. As a result, the primary end-point has now shifted and a new strategy needs to be established for patient follow-up and for defining treatment failure. Written by international experts in the field, this book is mainly focused on new techniques, all of which are amply illustrated. Technical Aspects of Focal Therapy in Localized Prostate Cancer will be of great practical value to all urologists and oncologists.

This comprehensive text provides a complete description of the newest technologies to diagnose and manage the most common prostate cancer diagnosed today: low risk disease. The book reviews new data about genetic markers, transperineal mapping biopsy and mpMRI, how to apply each of these technologies in patients with elevated PSA, when a prior prostate biopsy performed by the standard TRUS method is negative, and, in cases where low risk disease is already diagnosed, how to differentiate those men who might harbor more aggressive disease from those who do not. Over 75% of newly diagnosed prostate cancer meets the criteria for low risk disease which has created a dilemma for both patients and clinicians because of the inaccuracy of the standard TRUS biopsy method. Active surveillance programs have been initiated and are reviewed. How the new technologies impact surveillance programs are addressed. Clinical stage designation is updated and a new intra-prostatic staging system is discussed. Prostate biopsy techniques utilizing transrectal ultrasound, transperineal mapping, elastography and mpMRI are compared. Abstract
This comprehensive book serves as a review for the Fellow of Interventional Pain Practice (FIPP) exam and functions as a concise guide for all interventional pain doctors. Through educational initiatives, it helps to promote consensus-building among experts on the effectiveness of existing techniques and avenues for advancement of therapeutic performances. The book is divided into four sections (head and neck, thoracic, lumbar and sacral/pelvic), and each chapter is devoted to the safe, standardized approach to interventional procedures. To prepare both the examiner and the examinee for the FIPP examination, each chapter contains the relevant C-arm images and outlines the most common reasons for “unacceptable procedures performance” and “potentially unsafe procedures performance.” Distinguishing it from many of the previous guides, it also includes labeled fluoroscopic high quality images and focuses on the current FIPP-examined procedures with all accepted approaches. Abstract
This text encompass an up-to-date, comprehensive review of the state-of-the-art for gland preserving therapies. Fully updated and revised, this text evaluates the scientific evidence for the evolving trend to treat intermediate risk, clinically localized prostate cancer in a focally ablative manner with novel gland-preserving, focal therapy methods. Various ablative devices such as high intensity focused ultrasound, irreversible electroporation, photodynamic therapy, cryotherapy and laser ablation, among others, is discussed in regard to their strengths and limitations as a therapeutic modality. Emphasis is placed on patient selection and outcomes utilizing both advanced imaging techniques and pathologic evaluation. Current and new approaches to image cancer foci within the prostate (multiparametric ultrasonography, multiparametric magnetic resonance image, etc.) are presented along with various biopsy techniques, including robotics to map prostate cancer. Patient selection based on imaging and genomic classification, adjuvants to enhance therapy, treatment strategy, outcomes and patient centered concerns is discussed, providing an acceptable balance between cancer control and improved quality of life for patients.
