A fundamental issue facing the global community is meeting the challenges of population aging and achieving healthy aging to maintain an active older population and reduce the number of disabled people. The focus of this book is on theoretical issues and empirical findings related to trends and determinants of healthy aging, including factors related to "healthy longevity" of the oldest-old, aged 80 and over. The group is the most rapidly increasing elderly sub-population and is most likely to need assistance in daily living in all countries. Chapters include both longitudinal and cross-sectional data from North America, Europe, and Asia in country-specific studies and cross-national comparisons. Part I focuses on the definition, components, concepts, measurements, and determinants of healthy aging, and discusses the trends and patterns of disability and healthy life expectancy at the macro level. Part II addresses individual healthy aging, including its biological and socio-demographic aspects. Part III focuses on issues concerning the family and healthy aging, and Part IV explores formal and informal care for healthy aging through governmental policy interventions and community service programs.

This will be the fifth edition of successful clinical manual aimed at all practitioners treating children with cancer and blood diseases, such as leukemia, anemia, and bone marrow failure. As with previous editions, this edition will be a comprehensive book on patient management, replete with algorithms and flow diagrams on diagnosis and management. Its concise and easy-to-read format will enable readers to make accurate diagnoses and permit them to treat patients even if they have not had extensive previous hematologic or oncologic experience. It will contain a list of normal values at various ages in children, which is extremely useful as a reference in the management of patients. Since the previous edition was written five years ago, there have been considerable advances particularly in the treatment and management of oncologic diseases in children. These oncology sections (approximately 40% of the book) will be entirely rewritten to incorporate all current protocols. In addition, advances in many other areas will require that all hematology sections of the book be updated. The successful basic format and content of clinical manifestations, diagnosis and differential diagnosis will be maintained - as in previous editions, the editor will closely edit chapter content to maintain consistency in format, depth, and style. In addition to the core group of hematologists and oncologists from Schneider Children's Hospital, who contributed to the 4th edition, the editor is inviting 15 new authors who are all experts from around the country - in order to keep up with the advances in management of hem/onc diseases. Abstract
Vertigo is an enigma. It is a malady for all ages. It is also a greatly misunderstood and underrated clinical condition. Most physicians see vertiginous patients in their daily practice but no consensus has yet been reached on the true extent of this highly debilitating condition. Vertigo has diverse causes and forms. Diagnosis usually relies heavily on a detailed history and thorough clinical examination, but imaging techniques and a range of vestibular tests are also helpful. Most patients can be treated conservatively with anti-vertiginous drugs and/or rehabilitation therapy, but some require surgical intervention. This monograph on vertigo is based on the personal, hands-on, practical and clinical experience of managing vertigo in a large university-affiliated hospital. It covers all aspects of the subject, offering the latest information on epidemiology, aetiopathogenesis, diagnostics, the battery of vestibular and allied tests, treatment, rehabilitative management and ethical considerations. The neglected but substantial problem of paediatric vertigo is addressed, as are the many pathologies in the elderly population that can lead to disequilibrium and repeated falls. This book will prove of value to a range of practitioners, including family physicians, otologists, audiologists, vestibular scientists, neurologists, paediatricians, geriatricians, physiotherapists, rehabilitation therapists and general doctors, as well as medical and nursing students.

Abstract
This transdisciplinary volume outlines the development of public health paradigms across the ages in a global context and argues that public health has seemingly lost its raison d'être, that is, a population perspective. The older, philosophical approach in public health involved a holistic, population-based understanding that emphasized historicity and interrelatedness to study health and disease in their larger socio-economic and political moorings. A newer tradition, which developed in the late 19th century following the acceptance of the germ theory in medicine, created positivist transitions in epidemiology. In the form of risk factors, a reductionist model of health and disease became pervasive in clinical and molecular epidemiology. The author shows how positivism and the concept of individualism removed from public health thinking the consideration of historical, social and economic influences that shape disease occurrence and the interventions chosen for a population. He states that the neglect of the multifactorial approach in contemporary public health thought has led to growing health inequalities in both the developed and the developing world. He further suggests that the concept of "social capital" in public health, which is being hailed as a resurgence of holism, is in reality a sophisticated and extended version of individualism. The author presents the negative public policy consequences and implications of adopting methodological individualism through a discussion on AIDS policies. The book strongly argues for a holistic understanding and the incorporation of a rights perspective in public health to bring elements of social justice and fairness in policy formulations.
