This volume, written by well-known experts in the field, covers all aspects of Anti-Neutrophil Cytoplasmic Antibody (ANCA) Associated Vasculitis (AAV). The expression refers to a group of diseases, characterized by destruction and inflammation of small vessels. The clinical signs vary and affect several organs, such as the kidney, lung, skin, nervous system and others. The opening chapters give some historical hints, explain the genetic basis of the disease and provide insights into the pathogenesis derived from recent experimental studies and guides the reader through classification and nomenclature. A large part of the book is then devoted to a detailed description of the specific related diseases and their clinical presentations, the disease course, and potential complications. The advice regarding treatment is based on the best currently available evidence in this constantly evolving area. The book is part of Springer’s series Rare Diseases of the Immune System, which presents recently acquired knowledge on pathogenesis, diagnosis, and therapy with the aim of promoting a more holistic approach to these conditions. AAVs are systemic autoimmune diseases of unknown cause that affect small (to medium) sized blood vessels. They include granulomatosis with polyangiitis (formerly Wegener's granulomatosis), microscopic polyangiitis, and eosinophilic granulomatosis with polyangiitis (formerly Churg–Strauss syndrome). Abstract
This book, now in a second revised edition, casts light on the often complex diagnosis of musculoskeletal tumors and tumor-like conditions by drawing on the experience of the Rizzoli Orthopedic Institute during more than 100 years of treatment and research in the field. The different entities are described from a multidisciplinary perspective, highlighting clinical, radiological, and histological correlations. Particular emphasis is placed on differential diagnosis, which often needs to take into account nontumoral conditions. The recent identification of further significant biological and genetic features, and the impact of these discoveries on the classification of round cell tumors, vascular lesions, and spindle/pleomorphic entities in bone, is also extensively discussed. The Rizzoli case archive dates back to September 1900 and contains the original material relating to more than 29,000 bone lesions and 11,000 soft tissue lesions. It is a fabulously rich resource, and this book will be invaluable for pathologists, radiologists, and clinicians at all levels of experience.

Abstract
This book offers an overview of the latest advances in the epidemiological, pathophysiological, diagnostic and therapeutic aspects of secondary hypertension, provided by a group of the most highly respected European experts in this field. It is common belief that secondary forms of hypertension are rare, although in 95% of patients the cause of high blood pressure is unknown – euphemistically referred to as “essential”. There is increasing evidence from epidemiological studies that the prevalence of secondary hypertension is much higher than previously thought, accounting for 20%-25% of all hypertensive patients. There are additional, very good reasons for identifying those patients with secondary hypertension: firstly, they are exposed to a greater risk of suffering major cardiovascular events than patients with essential hypertension who have the same blood pressure level, and for this reason require particular care. Secondly, these patients can be relieved of a lifetime of pharmacological therapy or, at least, have a chance to be treated with more specific and effective medications. A high degree of clinical skill is required to recognize the often vague and subtle symptoms and signs that characterize patients with secondary hypertension, and clinicians need to fully comprehend the mechanisms responsible for the development and maintenance of high blood pressure. Shedding new light on a complex area of cardiovascular medicine, the book enables readers to better treat affected patients.

Abstract
This open access book discusses individual, collective, and institutional responsibilities with regard to vaccination from the perspective of philosophy and public health ethics. It addresses the issue of what it means for a collective to be morally responsible for the realisation of herd immunity and what the implications of collective responsibility are for individual and institutional responsibilities. The first chapter introduces some key concepts in the vaccination debate, such as ‘herd immunity’, ‘public goods’, and ‘vaccine refusal’; and explains why failure to vaccinate raises certain ethical issues. The second chapter analyses, from a philosophical perspective, the relationship between individual, collective, and institutional responsibilities with regard to the realisation of herd immunity. The third chapter is about the principle of least restrictive alternative in public health ethics and its implications for vaccination policies. Finally, the fourth chapter presents an ethical argument for unqualified compulsory vaccination, i.e. for compulsory vaccination that does not allow for any conscientious objection. The book would appeal both philosophers interested in public health ethics and the general public interested in the philosophical underpinning of different arguments about our moral obligations with regard to vaccination.

This book presents, in condensed form, the clinical results achieved by means of various locoregional tumor therapies employed for different indications. In so doing, it will allow the reader to rapidly retrieve the information on indications and effectiveness that is required for optimal integration of these highly effective therapies into modern, complex treatment strategies. The renaissance in locoregional tumor therapies is an ongoing process. New chemotherapeutic schedules, especially for liver and lung tumors, have been developed during the past few years. Other targeted therapies, such as radioembolization, microwave ablation, and HIFU techniques, and new pharmaceuticals for intra-arterial therapy have been developed and implemented in clinical practice. Abstract
This book systematically focuses on central sleep apneas, analyzing their relationship especially with heart failure and discussing recent research results and emerging treatment strategies based on feedback modulation. The opening chapters present historical background information on Cheyne-Stokes respiration (CSR), clarify terminology, and explain the mechanics and chemistry of respiration. Following a description of the physiology of respiration, the pathophysiology underlying central apneas in different disorders and particularly in heart failure is discussed. The similarities and differences of obstructive and central apneas are then considered. The book looks beyond the concept of sleep apnea to daytime CSR and periodic breathing during effort and contrasts the opposing views of CSR as a compensatory phenomenon or as detrimental to the failing heart. The diagnostic tools currently in use for the detection of CSR are thoroughly reviewed, with guidance on interpretation of findings. The book concludes by describing the various forms of treatment that are available for CSR and by explaining how to select patients for treatment.
