Aspirin has been in clinical use for over 100 years, making it one of the oldest commonly used drugs in medicine, and is now predominantly used as an antiplatelet agent. Major new antiplatelet drug classes have recently become available for the clinician that offer the promise of major improvements in the treatment and prevention of arterial thrombotic disease. In the field of anticoagulation, the vitamin K antagonists such as warfarin have not been available for as long as aspirin in the antiplatelet field, and have also had very little in the way of competition until recently; although a number of parenteral anticoagulants have been available, the vitamin K antagonists have been the only orally active anticoagulant drugs. The last few years have seen the arrival of new drug classes, the orally active direct thrombin inhibitors and factor Xa antagonists, which offer a number of advantages, but not without important drawbacks. In the face of these important advances in both antiplatelet and anticoagulant therapy, clinicians often feel confused, even overwhelmed, as to what is now available and which drugs are useful (and indicated) in which circumstances. Antiplatelet and Anticoagulant Therapy is designed with such people in mind. It is written in an easy-to-follow format and with many illustrations to aid clarity and the assimilation of information. Each chapter is written by established authorities in their fields who are also experienced in explaining often complex concepts. Abstract
This book considers paradigmatic clinical cases in order to cast light on key issues relating to elective or emergency stent implantation and the use of oral anticoagulation (OAC) in patients with atrial fibrillation. The topics addressed include the optimal periprocedural antithrombotic treatment (uninterrupted vs interrupted OAC, intraprocedural use of heparin and glycoprotein IIb/IIIa inhibitors, etc.), the most appropriate type of stent (bare metal vs drug eluting vs "bioactive"), the optimal regimen (e.g., triple therapy of OAC, aspirin, and clopidogrel vs the combination of OAC and a single antiplatelet agent), and the most suitable duration of the antithrombotic treatment prescribed at discharge (1 vs 6-12 months). The case-based management recommendations will be of wide practical value in the current health care context, where percutaneous coronary intervention is available even to patients with relevant co-morbidities, such as those warranting long-term OAC, and the indications for OAC are much broader than in the past. The book will appeal especially to clinical and interventional cardiologists, internal medicine specialists, hematologists, and family physicians and will also be of interest to cardiology and internal medicine residents and fellows.

Abstract
This book focuses on the salient features of the biology of Bone Morphogenetic Proteins and the advances in our understanding of their structure and function and of downstream signaling, as well as their governance in systems biology from bone and dentin to kidney, cancer, diabetes, iron homeostasis and angiogenesis, including rare musculoskeletal disorders. BMPs, also referred to as growth and differentiation factors, are members of the TGF-beta superfamily and are highly conserved from fruit flies to mammals and are responsible for the formation of practically every organ during embryo development and involved in adult tissue injury and repair. BMPs establish their extracellular gradient by interacting with their respective antagonists and regulate their function through SMAD-dependent down-stream signaling effector genes. This volume is aimed at scientists and professionals dealing with metabolic disorders, nutrition, systems biology diseases, rare musculoskeletal conditions, and disorders related to iron metabolism, including anemia of chronic disease, hereditary hemochromatosis and beta thalassemia.

Abstract
This volume contains a collection of writings from the leaders in the fields of Molecular Biology and Melanoma Research which will begin to tell the ever-expanding story of the most recent findings, discoveries, and potential of BRAF-directed targets in melanoma. Recent research has shown that BRAF inhibitors are effective for a short period of time, but there is little hope that these drugs as single agents will lead to durable benefit in a majority of patients. Among scientists and researchers who work in drug discovery, there is a lot of interest in the development of molecularly targeted cancer agents. Namely, the identification of a molecular target, the selection of molecules which effectively inhibit this target. What is starkly different about the development of this class of compounds, however, is that the mechanism of action of these agents are not as straightforward as was once previously assumed and the mechanisms of resistance that tumor cells employ to evade complete destruction are unlike any that have been described before. These discoveries in addition to utilization of modern molecular biology techniques have led to a series of hypotheses regarding which other types of molecules could be used in combination with BRAF-inhibitors in hopes of revolutionizing the potential of therapeutics in melanoma.

Controversial topics are discussed in detail with an emphasis on helping the reader make informed decisions, particularly when treating patients for which several treatment approaches may be appropriate because trial data does not clearly define the best option. Abstract
The growing knowledge about disturbances of epigenetic gene regulation in hematopoietic stem cell disorders such as myeloid leukemias and preleukemias (myelodysplasias) is now being translated into treatment approaches that target the epigenetic defects pharmacologically. Such approaches include reactivation of tumor suppressor genes and other antiproliferative genes by reversal of DNA hypermethylation through azanucleosides, and use of HDAC inhibitors to reverse gene silencing mediated by chimeric, leukemia-specific transcription factors. This book first presents the latest evidence derived from preclinical models regarding the epigenetic regulation of hematopoietic stem cell differentiation and hemoglobin production. The significance of DNA methylation abnormalities in hematopoietic disorders and of epigenetic disturbances in lung cancer and other solid tumors is then discussed. A major part of the book, however, relates specifically to the translation of basic research and drug development to clinical applications, and in this context both present and future clinical strategies are considered. Individual chapters are devoted to the use of DNA hypomethylating agents and chromatin-modifying agents, and the treatment of hematologic malignancies and solid tumors by means of epigenetic agents is discussed in detail.
