Abstract
An ever growing population of patients are affected by cardiac disease. Most of these diseases are chronic in nature and require long term outpatient follow up. Subsequently, outpatient providers are finding themselves responsible for an increasing number of patients with cardiovascular issues. Appropriate diagnosis and management of cardiac disease is essential to providing high quality care in an efficient manner. There is a wide spectrum of providers involved in the care of these patients, many of which are not cardiologists. The Manual of Outpatient Cardiology is written in a practical, didactic format designed to deliver point-of-care information to practitioners of cardiology and assist non-cardiologists with the efficient management of cardiac disease. Highly illustrated with schematics and useful clinical figures, this book is an essential reference to all outpatient cardiology procedures.

Abstract
This book about ECG tracings and Holter recordings presented by Drs. Jan and Richard Adamec and Dr. Hein Wellens comprises quite an amazing collection. Each tracing is different from the others and sometimes presents unexplained arrhythmias at first glance. To fully understand these examples, one must understand their peculiarities. Nevertheless, each enigma has a solution, and each case is solved. Comprehension of the different arrhythmias includes their therapeutic consequences; sometimes the best therapeutic option is to do nothing. In studying these cases, it is essential to look very carefully and to understand the pattern of events' first to identify the mechanism, then to determine the clinical importance, and finally to decide whether the case is severe or benign. The classic ECG books, although very comprehensive, do not address this subject the same way. The examples presented in this book are from real-life cases from the practice of Drs. Jan and Richard Adamec and could not be explained adequately in a textbook. These real-life ECG tracings are very helpful because as cardiologists, our goal is to find an explanation for any pathologic tracing. The importance of this book, therefore, lies in its presentation and content, which is composed entirely of tracings from a practice in the Lake Geneva area. These tracings will certainly be of interest to doctors worldwide.

Abstract
The scientific field of intermittent hypoxia and its effects on living organisms is surrounded by divergent and controversial findings. Does the condition play pathogenic roles in disease states, such as sleep apnea, chronic pulmonary disease, cardiovascular disease and cancer? Or does exposure to intermittent hypoxia actually induce protective responses? Dealing with these long-standing controversies and new concepts, Intermittent Hypoxia and Human Diseases provides an open forum for the most up-to-date scientific understanding of both adaptive (beneficial) and maladaptive (detrimental) responses to intermittent hypoxia and their potential pathogenic and prophylactic roles in the development and progression of major human diseases. The Editors and their extremely experienced contributors have assembled this highly disease-oriented book and all the fundamental research findings are selectively presented as mechanistic explanations of a particular human disease. It is therefore especially prepared for the interests of clinicians, allied healthcare professionals, athletic trainers, and translational medical researchers.

Abstract
Tetralogy of Fallot is the most common form of cyanotic congenital heart disease, and one of the first to be successfully repaired by congenital heart surgeons. Although "fixed", patients born with tetralogy of Fallot cannot be considered "cured". Improving survival and quality of life for this ever-increasing adult population will continue to challenge the current and future generations of cardiologists. Adult patients with tetralogy of Fallot should be seen by a cardiologist specializing in the care of adults with congenital heart disease, to be monitored for late complications. They need to be checked regularly for any subsequent complications or disturbances of heart rhythm. This monograph is intended as both an introduction to the subject and a timely, comprehensive review, and will be welcomed by adult cardiologists, pediatric cardiologists, internists, surgeons, obstetricians, and intensivists who wish to learn about the most recent discoveries and advances concerning tetralogy of fallot in adults. It will also be of interest to advanced undergraduates wanting to learn more about the subject.

Abstract
Echocardiography has become an invaluable tool in intensive care medicine because of its ease of use and instantaneous provision of precise information on cardiovascular functional anatomy at the bedside. This book contains all the information that readers will require in order to perform echocardiography and lung ultrasound, and to interpret the findings correctly, permitting optimal guidance of the hemodynamic management of unstable patients. Furthermore, lung ultrasound is dealt with for both diagnosis and treatment of respiratory failure requiring mechanical ventilation. After an introductory chapter on the essential physics of ultrasonography, standard echocardiographic examination via both the transthoracic and the transesophageal approach is described. In the third section, the functional anatomy of all normal and pathologic thoracic structures as observed on echocardiography is presented in detail. A wide range of basic and advanced applications of echocardiography in the emergency and intensive care setting are then described. A final supplementary section considers further applications of ultrasound in the intensive care unit, such as ultrasound-guided vein cannulation, Doppler renal evaluation, transcranial Doppler, and lung and pleural ultrasound. This superbly illustrated book has been specifically designed to meet the needs of all intensivists, emergency physicians, and anesthesiologists who intend to use ultrasound extensively to guide optimal management of the critically ill.

Abstract
This comprehensive reference textbook examines in detail physiological and pathophysiological aspects of the cardiovascular system and cardiovascular disease in the sporting population. In the first part of the book, the non-invasive and invasive diagnostic techniques employed in cardiology are systematically reviewed, with explanation of their indications and clinical utility. The second, more extensive part is devoted to clinical cardiology and covers the great majority of cardiological problems encountered among athletes and other sportsmen and women. Syncope, cardiac arrhythmias, and sudden cardiac death are extensively addressed, and guidance is provided on sports practice for individuals with different forms of heart disease. Cardiologic rehabilitation is discussed, and attention is also paid to the legal implications of diagnosed or unsuspected cardiovascular diseases for sportspeople. Sports Cardiology will be an invaluable resource for trainee and experienced health care professionals, including in particular cardiologists and sports medicine physicians. It will provide the knowledge required in order to make critical decisions regarding eligibility, thereby preventing tragic events and especially sudden death: the most devastating occurrence in sport.

Abstract
The prevalence of hypertension is almost three times as high as that of diabetes mellitus type 2, with both conditions being major risk factors for stroke, ischemic heart disease, cardiac arrhythmias, and heart failure. The exact prevalence of hypertension related to hormonal derangements (endocrine hypertension) is not known but estimated to affect less than 15% of hypertensive patients. Recent scientific discoveries have increased the understanding of the pathophysiologic mechanisms of hypertension. In Endocrine Hypertension, a renowned panel of experts provides a comprehensive, state-of-the-art overview of this disorder, discussing when to assign an endocrine cause in one of many conditions that may present with hypertension. The first part of Endocrine Hypertension is dedicated to adrenal causes. The second part of the volume concerns potential nonadrenal causes of hypertension, such as growth hormone excess or deficiency, primary hyperparathyroidism, vitamin D deficiency, testosterone deficiency, insulin resistance, obesity-associated hypertension, and the role of central mineralocorticoid receptors and cardiovascular disease. An important contribution to the literature, Endocrine Hypertension is an indispensable reference not only for endocrinologists, diabetologists, and adrenal investigators, but also for translational scientists and clinicians from cardiology, internal medicine, pediatrics, family medicine, geriatrics, urology, and reproductive medicine / gynecology.

Abstract
Magnesium is an essential mineral which is required for growth and survival of humans. Since magnesium is a mineral and not synthesizable it must be obtained through dietary foods and/or supplements. Magnesium in Human Health and Disease reviews the benefits of magnesium supplementation to reach recommended intakes as well as provides new research that suggests how reaching levels above the recommended intakes can promote health and treat various diseases. Magnesium deficiency can cause low serum potassium and calcium levels, retention of sodium, and low circulating levels of regulatory hormones. These changes in nutrients cause neurological and muscular symptoms such as tremor and muscle spasms. Further magnesium deficiency causes loss of appetite, nausea, vomiting, personality changes and death from heart failure. Causes of magnesium deficiency include alcohol abuse, poorly controlled diabetes, excessive or chronic vomiting and/or diarrhea. Thus the effects of inadequate and deficient intakes or levels of magnesium is critical to health and are reviewed by the expert clinicians in this book. Magnesium in Human Health and Disease provides the most current research to support the potential benefits or lack thereof for normal and high supplementation with magnesium. Animal model research and early human trials are reviewed to document other disease states such as hypertension, cholesterol level, type 2 diabetes and cardiovascular disease that would benefit from increased magnesium.
