Abstract
This truly comprehensive title addresses all aspects of the evaluation and management of carotid artery disease. The extracranial carotid artery is an area of confluence for both medical and surgical specialists. Given its unique position and function in the body, disease states that involve the carotid artery require a unique approach to ensure the best outcomes for patients. Developed by a multidisciplinary team of thought leaders from across medical, surgical, and radiological disciplines, Carotid Artery Disease: Evaluation and Management provides a diverse resource where readers can find presentation, evaluation, and management recommendations for any process involving the extracranial carotid artery, be it related to atherosclerotic, traumatic, inflammatory, or even oncologic disease. Indeed this title is a unique offering in that one can find traditional, and even exclusively medical, conditions within the same binding as surgical or interventional ones. Typically, access to this type of information requires the purchase of multiple different texts, but this complete title distills the evaluation and management of carotid artery disease into a one-of-a-kind, practical, accessible reference. A much-needed and invaluable contribution to the clinical literature, Carotid Artery Disease: Evaluation and Management will be of great interest to anyone early in his or her career and needing an introduction to the field, as well as seasoned clinicians in need of state-of-the-art, refresher information.

Abstract
Diagnosing a patient with unexplained ocular pain can be time-consuming and difficult, but taking an anatomic approach and excluding causes along the way can aid in the diagnosis. This book provides the reader with a systematic evaluation plan for these cases, written and edited by leaders in the field. A Case-Based Guide to Eye Pain is written for both ophthalmologists and neuro-ophthalmologists since there are not enough neuro-ophthalmologists to treat the number of patients with unexplained ocular pain and general ophthalmologists are having to take on the diagnosis and treatment of these patients. Organized in an easy-to-use manner, each case covers the following key elements: the chief complaint, history of the present illness, the examination, assessment and plan, follow-up, alternate perspective, summary points, and key references. Tables are also available to help the reader rapidly sort through cases that may apply to a sign, symptom, historical feature, diagnostic test or treatment option. This allows the practitioner who has a patient with a particular concern to use the tables to identify a case discussion. Additionally, A Case-based Guide to Eye Pain includes an appendices with the general approach to eye pain and anatomy of the trigeminal pathway and its relation to eye pain.

Abstract
Older adults can be especially susceptible to the debilitating effects of chronic pain, yet there are often barriers to successfully alleviating pain on the part of elderly patients and the health care professionals who treat them. This comprehensive guide to geriatric pain management provides the most current information available on assessment and treatment of pain in older adults. In a concise, reader-friendly format, the book provides techniques, tips, and tools for assessing pain and examines barriers to appropriate treatment. It addresses the physiological and psychosocial factors underlying the process and occurrence of pain and helps nurses to develop a comprehensive multimodal approach to pain management that includes pharmacological and nonpharmacological interventions. The guide provides detailed coverage of medications commonly used for pain management, including all contraindications and side effects, so that nurses will be able to evaluate the best use of a medication in the context of comorbidities and sensitivities of each individual. Also addressed are chronic illnesses common to the elderly population, palliative and hospice care, treatment of concurrent depression and anxiety, treatment of cognitively impaired elderly, and techniques for assessment and intervention in cases of substance abuse.

Abstract
The main aim of this volume is to provide an updated overview of the genetic basis of a group of complex disorders that are considered rare individually, but show a relatively high prevalence on the whole in developed countries: the rare autoimmune diseases. These are chronic disorders that involve long and expensive treatments with considerable side effects that may dramatically reduce the quality of life of affected people. Therefore, shedding light into their pathologic mechanisms is a major concern given their growing social and economic awareness and impact. General sections include different chapters on single entities as systemic lupus and sclerosis, Behçet’s disease, Sjogren’s syndrome and polymiositis, and sections devoted to autoimmune vasculitis, hepatic autoimmune conditions, as well as those affecting the nervous system, and a concluding chapter in which the shared predisposition amongst all diseases is discussed. All the chapters are given a homogeneous scheme in which the authors explain the clinical singularities of every disease and report the recent breakthrough discoveries related to them, giving a critical interpretation and suggesting future perspectives. All contributors are renowned researchers with high expertise in each disease. Due to the exponential increase in our understanding of the genetic causes of these complex diseases, a major advance in biomedical discovery is taking place. However, most of the achievements in this field are very recent and there is a lack of bibliography gathering them together. This book is thus filling a gap and will prove a useful companion to clinicians in the first place (internists, rheumatologists, clinical immunologists, and hematologists) but also to basic and applied researchers.

Abstract
Representing a state-of-the-art appraisal of this viral infection and its complications, this book comprises contributions from international authorities in infectious diseases and neuropathic pain. Important new information is presented on the role of the virus in terms of vascular risk, notably in heart attack, stroke and granulomatous angiitis (temporal arteritis). Similarly, new information on gastrointestinal involvement, often in the absence of rash and as seen with vasculopathies, is covered. The reader will benefit from new research into the pathology, pathophysiology and treatment of postherpetic neuralgia and its complications, and special attention is paid to prevention through zoster vaccination using the current vaccine, and a novel, broader option that can be used in immunocompromised patients. This books follows the two editions of the book, Herpes Zoster and Postherpetic Neuralgia, and is divided into sections for the convenience of the reader. A section on herpes zoster includes epidemiology and natural history of the varicella zoster virus, herpes zoster ophthalmicus, neurological complications, the role of varicella zoster virus in giant cell arteritis, concern about increased vascular risk of heart attack and stroke, antiviral therapy, and treatment of skin manifestations. A section on postherpetic neuralgia includes important information on the effect of herpes zoster and postherpetic neuralgia on quality of life, the neuropathology and pathophysiological mechanisms in postherpetic neuralgia, and the new concept of persistent ganglionitis as the cause of postherpetic neuralgia. A comparison is made between facial postherpetic neuralgia and trigeminal neuralgia. There is an extensive section on treatment, including the role of opioids, the general treatment of postherpetic neuralgia, intervention and neurosurgical approaches, and covering guidelines for clinical trial designs in postherpetic neuralgia. A final section addresses the questions of whether aggressive treatment of acute herpes zoster can prevent postherpetic neuralgia and includes a critically important chapter on herpes zoster vaccines.

Abstract
Much attention has been drawn to the impact of the inflammatory response in cardiovascular surgery. Minimizing deleterious effects of cardiovascular surgery consists of understanding the pathogenesis of the inflammatory response and how to modulate it using medical strategies and especially specific operative approaches. Despite the attention on this response, the concept has been underestimated and overlooked, and scientific information on it is still lacking in the medical literature. Inflammatory Response in Cardiovascular Surgery includes the most recent and exciting topics regarding the relationship between inflammation and cardiovascular surgery. It provides comprehensive instruction to assist understanding of the inflammatory mechanisms involved and clarifies the basic immunologic and physiologic concepts. The book then includes discussion of the neuroendocrine response, pathogenesis of atherosclerosis and mechanisms of inflammation, while emphasizing the impact on surgery and pathologic aspects. As such this textbook will be vital reading for all involved in the management of cardiovascular surgical patients, such as cardiovascular and transplant surgeons, anesthesiologists, intensive care physicians, cardiovascular and vascular fellows, and researchers.

Abstract
This book is a state of the art guide to the diagnosis of extracranial vascular pathologies with modern noninvasive neuroimaging and vascular imaging techniques. The opening sections provide a thorough introduction to arterial and venous anatomy, basic hemodynamics, and the principles of noninvasive vascular diagnostics, including by means of color Doppler ultrasound, CT and CT angiography (356- and 640-slice systems), and MRI and MR angiography (1.5 and 3 T). The main body of the book is devoted to the use of these methods to image cerebral ischemia and a wide variety of extracranial arterial and venous anomalies and pathologies. Neuroimaging and vascular imaging diagnostic criteria are clearly identified with the aid of many high-quality images, and the advantages and disadvantages of each modality for each pathology are explained. Information is also presented on etiology, pathophysiology, and other relevant aspects. A concluding section discusses the role of complementary noninvasive functional tests. The book will be a valuable resource for neurologists, angiologists, neuroradiologists, neurosurgeons, trainees, and all physicians who care for patients with cerebrovascular diseases.
