Abstract
Echocardiography has become an invaluable tool in intensive care medicine because of its ease of use and instantaneous provision of precise information on cardiovascular functional anatomy at the bedside. This book contains all the information that readers will require in order to perform echocardiography and lung ultrasound, and to interpret the findings correctly, permitting optimal guidance of the hemodynamic management of unstable patients. Furthermore, lung ultrasound is dealt with for both diagnosis and treatment of respiratory failure requiring mechanical ventilation. After an introductory chapter on the essential physics of ultrasonography, standard echocardiographic examination via both the transthoracic and the transesophageal approach is described. In the third section, the functional anatomy of all normal and pathologic thoracic structures as observed on echocardiography is presented in detail. A wide range of basic and advanced applications of echocardiography in the emergency and intensive care setting are then described. A final supplementary section considers further applications of ultrasound in the intensive care unit, such as ultrasound-guided vein cannulation, Doppler renal evaluation, transcranial Doppler, and lung and pleural ultrasound. This superbly illustrated book has been specifically designed to meet the needs of all intensivists, emergency physicians, and anesthesiologists who intend to use ultrasound extensively to guide optimal management of the critically ill.

The field of structural heart disease interventions is experiencing a stage of rapid growth with the development of percutaneous mitral valve repair therapies and percutaneous aortic valve replacement therapies. The MitraClip represents a new therapeutic frontier for mitral regurgitation and is expected to revolutionize repair of the mitral valve. For the first time in the world, physicians now have the capability to repair the mitral valve percutaneously without the need for a sternotomy or cardiopulmonary bypass. However, as with all new endeavors, there is a steep learning curve, not just for the individual operator but also for the entire field. The MitraClip technology and technique is technically complex and is guided by echocardiography and fluoroscopy. It is an imaging intensive procedure that requires deep 3-dimensional understanding of normal and abnormal cardiac anatomy. Not all mitral regurgitation cases are suitable and not all cases are straightforward. It is therefore crucially important for physicians in this specialty to communicate their experience, successes, failures and complications with each other. Whereas recent approaches have recommended to remove particular drugs from the medication regimen to avoid adverse effects, Drug Therapy for the Elderly underlines both indispensable and dispensable elements of drug treatment, providing the practitioner with: Background information on the elderly population regarding their needs for particular drugs (with an eye to frailty, co-morbidity patterns, and special sensitivities regarding drug metabolism and excretion); Practical advice about drug treatment surveillance parameters in the elderly; In-depth discussion of drugs in relation to the elderly with specific diagnoses; Integration of multimorbidity/polypharmacy situations into prioritization schemes; A plan for how to deal with the complexity of polypharmacy situations in a five-minute consultation. Abstract
Improvements in surgical and medical management of congenital heart disease (CHD) result in an increased life expectancy and a greater number of children living into adulthood. Consequently, adult cardiologists are caring for increasing numbers of patients with congenital heart defects. In many instances, these patients have undergone palliative but not corrective procedures and may be at higher risk of cardiovascular morbidity and mortality. In the past several years, the use of CT angiography (CTA) has increased, driven mainly by technical improvements that allow shorter imaging times and high spatial and temporal resolution. Thus, CTA combines widespread availability with the advantages of volumetric data acquisition, short acquisition times, and the possibility of simultaneous evaluation of myocardial and extracardiac structures, ventricular function and coronary anatomy making it a very appealing imaging modality in CHD patients. CT Atlas of Adult Congenital Heart Disease is an all inclusive, invaluable tool to describe the features of classic CHD, hereditary cardiomyopathies and aortopathies. It includes comprehensive pathology descriptions and discussions of the utility of CTA in the diagnosis of all forms of adult CHD. There have been many changes in the field of coagulation during the past decade. New concepts of epidemiology of risk factors for thrombosis now help clinicians predict who is more likely to form clots after surgery, or after being placed on oral contraceptives. New anticoagulants have the potential to redefine how patients with atrial fibrillation and venous thrombosis are managed. There are new forms of recombinant clotting factors which have changed our approach to hypofibrinogenemia and von Willebrand's disease. Newer antiplatelet agents are available and their use in patients receiving cardiac stents has mushroomed. The management of thrombosis in the setting of pregnancy has changed over the past decade, as well as the way clinicians approach women with multiple miscarriages. An entire new class of compounds, the thrombopoietins, are available to treat individuals with immune thrombocytopenic purpura (ITP). The Coagulation Consult covers major topics of interest to hematologists who are asked to consult on individuals with coagulation related diseases, and encompasses the field's most recent developments. This "case-directed" book describes state-of-the-art approaches to patients with bleeding and clotting disorders, as well as laboratory tests for coagulation. Chapters include different vignettes, focus on typical clinical consult questions, and lay out specific types of treatment.
