Abstract
This book offers a pragmatic approach to day-to-day metabolic and nutritional care based on physiological considerations. Due to the numerous controversial trials published in the last 15 years, there is no clear guidance for intensive care physicians regarding the metabolic and nutritional management of patients. This has resulted in a return to underfeeding and related complications in most ICUs worldwide as shown by the latest Nutrition Day data. Using a structured, logical approach, the book examines practical solutions for artificial feeding in complex areas of critical care (brain injuries, burns, cardiac failure, ECMO, intestinal failure, long term patient, renal failure, metabolic diseases, obesity, old patients) and discusses measurement of the results of metabolic interventions. It also includes dedicated chapters focusing on specific problems, in order to avoid complications. It particularly focuses on recently recognized initiation and progression factors and the varying treatment strategies in different regions, such as Asia, Europe, and the United States. More than 40 years have passed since Dr. Jean Berger first described primary IgA nephropathy ("Nephropathy with mesangial IgA-IgG deposits") as a new disease entity. Immunohistopathologically, IgA nephropathy is characterized by the granular deposition of IgA (IgA1) and C3 in the glomerular mesangial areas with mesangial cell proliferation and the expansion of mesangial matrices. It is clear that IgA nephropathy is one of the most common types of chronic glomerulonephritis in the world. This disease may lead to end-stage kidney disease, with its enormous economic impact on healthcare everywhere. Efforts by many investigators around the world have gradually clarified various aspects of the pathogenesis and treatment of IgA nephropathy. However, there are many controversial strategies for the treatment of patients with IgA nephropathy throughout the world, as there are several limitations for treatment in each country. This volume provides a transdisciplinary and translational review of many of the leading murine models used to study the mechanisms, mediators and biomarkers linking energy balance to cancer. It provides a review of murine models that should be of interest to basic, clinical and applied research investigators as well as nutrition scientists and students that work in cancer prevention, cancer control and treatment. The worldwide obesity pandemic has been extensively studied by epidemiologic and observational studies and even, in some cases, by randomized controlled trials. However, the development and control of obesity, its comorbidities and its impact on cancer usually occurs over such long periods that it is difficult, if not impossible to conduct randomized controlled trials in humans to investigate environmental contributions to obesity, energy balance and their impact on cancer. In contrast, model organisms, especially mice and rats, provide valuable assets for performing these studies under rigorously controlled conditions and in sufficient numbers to provide statistically significant results. In this volume, many of the leading and new murine models used to study the mechanisms and mediators linking cancer with obesity, sleep, exercise, their modification by environment and how they may continue to be used to further elucidate these relations as well as to explore preclinical aspects of prevention and/or therapeutic intervention are considered. This volume provides an important compilation and analysis of major experimental systems and principles for further preclinical research with translational impact on energy balance and cancer.

The obesity pandemic continues to increase on a world-wide basis with over 70% of the United States population being either overweight or obese. Hematologic malignancies have recently been identified among the obesity associated malignancies spanning the lifespan from childhood to the elderly and include leukemia, myeloma, lymphoma and others. In addition to the potential etiologic association between obesity and hematologic malignancies, the presence of obesity has profound effects on therapy by impacting pharmacokinetics of chemotherapeutic agents, dose, adipocyte metabolism and drug distribution. These may be particularly important in hematopoietic stem cell transplantation. Another important aspect of the association of obesity with hematologic malignancies is the increased incidence of obesity in children who successfully complete therapy for acute lymphoblastic leukemia. This and other observations indicate important relations between the hematopoietic systems and adipose tissue metabolism. Energy Balance in Hematologic Malignancies will provide an important volume in the series on Energy Balance and Cancer and serve as a basis for better understanding etiology, mechanisms, therapeutic implications and experimental approaches. While this book should be of specific interest and utility to all pediatric and adult hematologists and oncologists dealing with hematologic malignancies, it should also provide an important resource for all investigators concerned, from a clinical or research basis, with the impact of obesity and exercise on hematologic malignancies.
