Abstract
This book reviews locoregional radionuclide cancer therapies (LRCT). Proving an increasingly viable alternative to radiotherapy, radionuclide therapy includes a diversity of choices of well characterized biochemical and physiologic target molecules. The delivery and retention of radionuclides may be monitored by advanced imaging for exact tissue localization and for real-time dosimetry to enable personalized precision medicine. Radiopharmaceuticals in human cancer therapies are typically delivered in systemic routes but can also be designed for locoregional routes to harness pharmacokinetic advantages of higher payload and lower systemic toxicities. This book explores the latest advancements and clinical considerations of the locoregional approach. Throughout the chapters, the clinical and scientific bases of cancer treatment and the locoregional use of radionculides are explored. Mathematical models of radiation dosimetry of locoregional radionculdies on tissues are studied using common models for multiple commercially available radionuclides. Rodent and canine tumor models of LRCT are compared for selected radionuclides and radiopharmaceuticals. The practical aspects of radiopharmaceuticals production, marketing, transport, as well as radiation protection are reviewed. Finally, the combination of LRCT with immunotherapy and other cancer therapies and prospective future use of LRCT are discussed. This is a guide for practicing nuclear physicians, interventional radiologists, radiation oncologists, radiation scientists, veterinarians and oncologists to expand their knowledge base and to prepare for designing locoregional radionuclide cancer therapies in animals and in humans.

Completely revised and updated, and utilizing the most current evidence and practice guidelines for the treatment of osteoporosis, this comprehensive third edition discusses the basic aspects of bone metabolism, the pathophysiology of osteoporosis, current diagnostic techniques and medical treatment strategies. Osteoporosis is a common disorder that is prevalent in over 20 million Americans over the age of 60. One and a half million osteoporotic fractures occur in the United States every year, including 300,000 hip fractures. Mortality rates after hip fracture approach 25%, with another 50% of patients experiencing a major decrease in their prior level of independence and quality of life. Despite the wide prevalence and severe consequences of osteoporosis, it remains a disorder that is severely under-diagnosed and treated. In this context, specialists and primary care physicians alike are having increasing difficulty keeping up with the rapid changes to the field and incorporating these advances to clinical care. In the years since the last edition of this book was published, the osteoporosis field has changed drastically. In addition to revising and updating existing chapters and removing a few that are no longer as relevant, new chapters discuss an advanced understanding of the cellular and molecular mechanisms underlying the disorder, the introduction of new diagnostic imaging techniques, a more nuanced appreciation of the risks and benefits of osteoporosis therapies, and the introduction of two new classes of osteoporosis medications. Now in a revised and expanded second edition, this unique text presents topics related to insulin resistance in youth and its consequences across the lifespan. In the first section of the book examining epidemiology, the contributors review controversies over the definition of insulin resistance in children and what is known about how insulin resistance in youth differs from adults, the measurement of insulin resistance in youth in the research and clinical settings, and current knowledge regarding the epidemiology of insulin resistance in the pediatric population. The second section of the book explores pathophysiology, including current knowledge of the molecular, metabolic, and physiologic mechanisms of insulin resistance, the unique pathophysiology of pregnancy and puberty, the contributions of the prenatal and early childhood environment to the development of insulin resistance, and adipose and biochemical mediators. This section concludes with discussion of the relationship between insulin resistance and cardiovascular disease and liver disorders. A third section, new to this second edition, explores insulin resistance in unique models: intrauterine growth restriction and girls with polycystic ovary syndrome and metabolic syndrome. The final section of the book explores the concepts of treatment through medications directed at insulin sensitivity, as well as exercise, weight loss medications and consequences of bariatric surgery. Abstract
This third edition of this text is organized into seven sections that address the educational needs of dietitians and other health care professionals around the world who seek current information about nutritional management of chronic kidney disease (CKD). Part I addresses the differences in the epidemiology of CKD and renal replacement therapy worldwide, such as environmental, ethnic, cultural, political and macroeconomic factors. Part II includes a thorough review of the components of the nutrition assessment, which includes information about psychosocial issues affecting nutritional status in kidney disease and drug-nutrient interactions, and parts III and IV review preventative strategies for common disorders associated with CKD such as hypertension, type 2 diabetes, obesity and cardiovascular disease, and current evidence-based treatment recommendations for the nutrition management of non-dialyzed, dialyzed and transplanted adults are addressed. Part V presents the nutritional concerns of CKD populations with special needs (i.e., pregnancy, infancy, childhood, adolescence and the elderly). The nutrition management of other disorders associated with kidney disease are covered in Part VI; these include protein-energy wasting and the inflammatory response, bone and mineral disorders, nephrotic syndrome, nephrolithiasis, and acute kidney injury. Lastly, Part VII is devoted to cutting-edge research on topics of concern in nutrition in kidney disease such as the gut microbiome including pre- and probiotics, appetite regulation, advanced glycation end products, physical activity and structured exercise, and dietary patterns including plant-based diets. When appropriate, the new clinical practice guidelines in nutrition for individuals with CKD are integrated into the chapters. Abstract
This book discusses major changes in our understanding of the most prevalent non-orthopedic, sports-related condition – overtraining syndrome (OTS), arguing that it should be considered as the manifestation of burnout in athletes, rather than simply the result of excessive training. While the chronic adaptations of the cardiovascular and musculoskeletal systems to exercise are well documented, those of the endocrine system are less well known, and adaptations of the hormonal ranges for athletes are yet to be determined. There is also a lack of standardized diagnostic criteria, consistent assessment methods and biomarkers. This book offers a systematic review of the hormonal aspects of overtraining syndrome, and a comparison with sports-related syndromes triggered by chronic deprivation of different sorts, including the female athlete triad (and its derivative, RED-S) and burnout syndrome of the athlete (BSA). It demonstrates that these conditions, although studied separately from each other, may all be different manifestations of the same condition, leading to ‘maladaptive’ (dysfunctional forced adaptations to a hostile environment) changes in response to chronic depletion of energy and mechanisms of repair, causing multiple dysfunctions. The author proposes that OTS/Paradoxical Deconditioning Syndrome (PDS), RED-S/TRIAD and BSA are parts of a same condition, or at least a group of similar conditions. Further, the book offers a chronological overview of OTS, based on preliminary research. Given its broad scope, this concise reference book will appeal to a range of health professionals. It allows readers, including those without a strong academic background, to gain a systematic understanding of OTS.
