Abstract
While there is a wealth of published information on addiction medicine, the psychological aspects of alcohol abuse, and behavioral medicine with regard to addiction, virtually none of the existing resources were written with the primary care provider in mind. Addressing Unhealthy Alcohol Use in Primary Care is a handy reference for primary care clinicians who are confronted by patients with these problems daily, and who wish to successfully address these issues in their practice. Focusing on the literature and science relevant to practicing providers, this book covers the range of interventions appropriate for this setting. Topics include assessment, brief counseling interventions, pharmacotherapy, referrals to both specialty care and Alcoholics Anonymous (and other self-help programs), psychiatric comorbidity and other drug use, and other information specific to the needs of the primary care clinician. The vast majority of people with health risks and consequences related to alcohol use receive little to no attention from healthcare, despite the fact that alcohol is a leading cause of early preventable death. This book is a guide to identifying and addressing unhealthy alcohol use in hopes of improving the lives of patients.

Abstract
Alcohol Use Disorders and the Lung: A Clinical and Pathophysiological Approach is an excellent resource for clinicians who care for individuals affected by alcohol use disorders in diverse settings. Although alcohol abuse alone does not cause acute lung injury, it renders the lung susceptible to dysfunction in response to the inflammatory stresses of sepsis, trauma, and other clinical conditions recognized to cause acute lung injury. In parallel, these same pathophysiological effects of alcohol abuse significantly increase the risk of a wide range of serious lung infections. Many clinicians involved in the primary treatment of alcohol use disorders, such as addiction psychiatrists, will find this text of interest as it will expand their understanding of the health consequences of alcohol use disorders. In parallel, clinicians who specialize in pulmonary and/or critical care medicine will have a unique resource that provides a comprehensive review of the pathophysiology of alcohol-related lung disorders and insights into evolving therapeutic options in these vulnerable individuals. Alcohol Use Disorders and the Lung: A Clinical and Pathophysiological Approach fills a gap in the literature and presents the evolving clinical research that may soon lead to novel therapies that can improve lung health in individuals with alcohol use disorders and co-existing conditions such as HIV infection.

In this book, recognized anesthesiology experts present the latest findings on anesthesia for electro-convulsive therapy (ECT). With the development of modified electroconvulsive therapy (m-ECT) and devices to provide brief-pulse stimulation, ECT is currently being re-evaluated as an important procedure in psychiatric disorder therapy and is increasingly being used worldwide to treat several psychiatric conditions. This trend is due to the social phenomenon of the growing number of patients for whom ECT is applicable. Since the new approach is more safe and effective than the original method without anesthesia, m-ECT is currently applied under general anesthesia in most advanced countries. This book provides an overview and practice of total anesthesia management in the perioperative period of ECT. It offers an indispensable resource not only for professional anesthesiologists but also healthcare professionals who use this therapy.

Abstract
This is the first comprehensive two-volume collection on anhedonia, a disorder that played an important role in psychopathology theories at the beginning of the twentieth century. Anhedonia is a condition in which the capacity of pleasure is partially or completely lost, and it refers to both a personality trait, and a "state symptom" in various neuropsychiatric and physical disorders. It has a putative neural substrate, originating in the dopaminergic mesolimbic and mesocortical reward circuit. Over the past three decades cognitive psychology and behavioral neuroscience have expanded our understanding of anhedonia and other reward-related processes. The aim of this new two-volume collection on anhedonia is to highlight the contributions of eminent scientists in this field as well as to provide readers with comprehensive accounts of recent developments as perceived by the authors. This monograph is divided into five parts. Volume I contains parts one and two (Conceptual Issues and Neurobiological Advances) including 14 chapters that serve as an introduction and overview of conceptual issues. Volume II contains three parts (Anhedonia in Psychotic Disorders, Anhedonia in Mood and Personality Disorders, and Anhedonia in Neurological and Physical Disorders) including 15 chapters that provide an overview of the construct, measurement of anhedonia in schizophrenia spectrum disorders, hedonic capacity and related factors in schizophrenia and schizoaffective disorder, anhedonia as an indicator of genetic liability for schizophrenia, and as a trait marker for depression, the role of an anhedonia in trauma-related disorders, anorexia nervosa, stress-induced eating disorders, schizotypal traits and risk of suicide. This book will be of interest to a broad spectrum of readers including psychiatrists, psychologists, neurologists, neuroscientists, endocrinologists, pharmacologists, general practitioners, geriatricians, graduate students, and health care providers in the fields of mental health.
Contents
Part I. Anhedonia in Psychotic Disorders -- 1. Anhedonia in Schizophrenia: A Brief History and Overview of the Construct -- 2. Measuring Anhedonia in Schizophrenia-Spectrum Disorders: A Selective Update -- 3. Hedonic Capacity and Related Factors in Schizophrenia and Schizoaffective Disorder -- 4. Anhedonia as an Indicator of Genetic Vulnerability to Schizophrenia -- 5. Anhedonia in Schizophrenia: A Deficit in Translating Reward Information into Motivated Behavior -- Part II. Anhedonia in Mood and Personality Disorders -- 6. Neural Correlates of Anhedonia as a Trait Marker for Depression -- 7. Anhedonia in Trauma Related Disorders: The Good, the Bad, and the Shut-Down -- 8. Anhedonia and Anorexia Nervosa: A Neurocognitive Perspective -- 9. Anhedonia and Negative Symptom Schizotypy -- 10. Abstract
This book provides a basic clinical guide to the principles and practice of artificial ventilation, both manual and mechanical. It covers the development of artificial ventilation through the ages and the essential anatomy and physiology behind it. While there are many detailed texts available on mechanical ventilation, they are usually aimed at the hospital specialist and cover the many complex modes of ventilation used in the hospital setting. This book covers the basics of airway and ventilation management for non-specialists working in pre-hospital and emergency medicine. It fulfils the need for a resource that explains simply and clearly basic respiratory physiology, the pathophysiology behind respiratory failure and the practical aspects of artificial ventilation. This book links the two areas of hospital and pre-hospital practice together to promote better understanding of artificial ventilation by medical, paramedical and nursing personnel working in different fields of medicine.
Contents
A Brief History of Artificial Ventilation -- The Structure of the Airways and Lungs -- How the Lungs Work: Mechanics and Gas Exchange with the Blood -- Respiratory Failure -- The Management of Respiratory Failure: Airway Management and Manual Methods of Artificial Ventilation -- Basic Principles of Mechanical Ventilation -- Portable Mechanical Ventilators -- Managing Ventilation During Transport -- Paediatric Artificial Ventilation for the Non-Specialist -- Artificial Ventilation in Difficult and Extreme Environments -- Mass Ventilation -- Conclusions.

Abstract
This text illustrates bone marrow aspirate, imprint and biopsy specimens showing characteristic features of a wide variety of neoplastic and non-neoplastic conditions. While the focus is on Wright-stained smears and hematoxylin-eosin stained biopsies, other key histochemical and immunohistochemical stains are illustrated that are vital for proper diagnosis. After a brief review of the normal bone marrow, reactive changes in the marrow are illustrated, including the bone marrow response in constitutional disorders and to metabolic changes throughout the body. This is followed by specific infectious disorders in the marrow and other non-neoplastic disorders. The remainder of the Atlas illustrates the various neoplasms that involve the bone marrow, including leukemias, lymphomas and non-hematopoietic neoplasms. The hematologic neoplasms are classified using the 2016 World Health Organization (WHO) classification. This overview of bone marrow disorders illustrates a wide variety of diseases that practicing pathologists and hematologists will encounter in their routine practice.

Abstract
This atlas provides a clear, concise overview of the most challenging circumstances faced by clinicians and pathologists when dealing with melanocytic neoplasms. The book is structured as a case series; for each case, the clinical and dermoscopic appearances are presented, accompanied by a brief but comprehensive description and compelling histopathologic images. When available, in vivo confocal microscopy images are also included to highlight additional diagnostic clues. Identification of key messages and selected references will further guide the reader in the diagnosis and management of the neoplasm under consideration. It is well known that melanocytic lesions can be difficult to interpret. Some lesions show an ambiguous combination of morphologic criteria, and in these cases interpretation entails a high degree of subjectivity that results in low interobserver agreement even among expert pathologists. This atlas demonstrates how the addition of clinical information, including that provided by dermoscopy, can assist in reaching a more confident diagnosis.

Abstract
Sleep Medicine is a field that attracts physicians from a variety of clinical backgrounds. As a result, the majority of sleep specialists who interpret sleep studies (PSG) do not have specialized training in neurophysiology and electroencephalography (EEG) interpretation. Given this and the fact that PSGs usually are run at a third of the speed of EEGs and that they usually have a limited array of electrodes, waveforms frequently appear different on the PSGs compared to the EEGs. This can lead to challenges interpreting certain unusual looking activity that may or may not be pathological. This Atlas of Electroencephalograpy in Sleep Medicine is extensively illustrated and provides an array of examples of normal waveforms commonly seen on PSG, in addition to normal variants, epileptiform and non-epileptiform abnormalities and common artifacts. This resource is divided into five main sections with a range of topics and chapters per section. The sections cover Normal Sleep Stages; Normal Variants; Epileptiform Abnormalities; Non-epileptiform Abnormalities; and Artifacts. Each example includes a brief description of each EEG together with its clinical significance, if any. Setting the booká apart from others in the field is the following feature: Each EEG discussed consists of at least three views of the same page -- one at a full EEG montage with 30mm/sec paper speed, the same montage at 10mm/sec (PSG speed) and a third showing the same thing at 10 mm/sec, but with the abbreviated PSG montage. Unique and the first resource of its kind in sleep medicine, the Atlas of Electroencephalograpy in Sleep Medicine will greatly assist those physicians and sleep specialists who read PSGs to identify common and unusual waveforms on EEG as they may appear during a sleep study and serve as a reference for them in that capacity.

Abstract
This book presents a vast collection of radiologic images of cases seen in a very busy emergency room. It encompasses common and very unusual pathology and every imaging modality. The book is divided into four parts on pathology of the vascular system, the chest, the abdomen and pelvis, and the reproductive organs. At the beginning of the presentation of each condition, the diagnosis and expected imaging features are set out. Images obtained with the modalities that best depict that particular abnormality are then presented, with marking of the salient pathology for easy understanding. Brief captions cite the clinical indication, explain the marked abnormal imaging features, and identify the modality and its characteristics. Whenever possible, differential diagnosis is covered using further images so the reader can see the spectrum of imaging features that distinguish pathologies despite similar clinical presentation. In addition, guidance is provided on selection of further modalities to confirm the diagnosis. The book will be of value to a wide audience: It will help residents to analyze different diseases and relate pathophysiology to imaging and assist students in appreciating what is normal and what is abnormal. It will serve as a useful guide to the busy practicing radiologist, especially when encountering an unusual case. Finally, it will aid clinicians in understanding the complexity of these acute clinical problems and in delivering better focused treatment.
