Abstract
This book addresses health disparities commonly encountered in pediatric and adult pulmonary, critical care, and sleep medicine, providing a definitive reference on this prominent issue. Expert authors explore questions such as: * What is the evidence that respiratory health disparities exist? * What do we know about the causes of the disparities? * What are the clinical implications? * What can be done to address disparities and thus achieve respiratory health equality? Recognizing the magnitude of this problem, the American Thoracic Society (ATS) Executive Committee created a Health Equality and Diversity Committee, with an initial mandate of defining respiratory health disparities and respiratory health equality in the United States. This book will follow the format of a workshop on respiratory health equality held before the 2015 ATS International Meeting and led by editor Juan Carlos Celedón. Written by the workshop presenters and other distinguished guests, this book will focus closely on major risk factors for disparities in respiratory health, and potential approaches to eliminate such disparities. Achieving Respiratory Health Equality in the United States is a timely resource for researchers, clinicians, and public health practitioners in respiratory medicine.

Abstract
This book reviews current knowledge on the diagnosis and treatment (surgical and non-surgical) of cartilage defects at the knee joint. In the discussion of diagnosis, the focus is primarily on imaging findings, particularly those obtained with MRI. The remainder of the book is devoted to the full range of current conservative and surgical treatments, with attention to both treatment indications and results as reported in the recent literature. In reviewing non-surgical treatment, oral and intra-articular medical management is evaluated and rehabilitation and physical therapy are also considered. The three main types of surgical technique--microfracture surgery and similar procedures, mosaicplasty and related techniques, and autologous chondrocyte transplantation--are then extensively discussed in a series of highly informative chapters. This book will be of great practical value for clinicians, assisting in daily decision making.
Contents
Regeneration of Articular Cartilage of the Knee: Basic Concepts -- Articular Cartilage Defects of the Knee: Diagnosis and Treatment -- Imaging Techniques of Articular Cartilage -- Refixation of Detached Fragments -- Debridement, Joint Lavage and Cartilage Shaving -- Drilling, Abrasion and Microfractures -- Osteochondral Transplantation and Mosaicplasty -- Autologous Chondrocyte Implantation -- Alignment Osteotomies -- Rehabilitation of Chondral Lesions of the Knee -- Medical Treatment: Intra-articular Injections of Hyaluronic Acid.

Abstract
The present book intends to provide a comprehensive guide on the field of robotic bariatric surgery. It covers all the stages to fulfill credentialing for performing robotic surgery, from an introduction to the robot device until comprehensive descriptions of surgical procedures. Also, robotic surgery is presented as an institutional program and we describe how to establish a robotic program in a hospital environment. The currently accepted and most common procedures – sleeve gastrectomy, gastric bypass and duodenal switch – are described in detail, with a step-by-step description of the techniques, followed by a wealth of photos and videos for each case. Special attention is given to the employment of robotic bariatric surgery in exceptional conditions, such as in super-obese patients, reoperations and revisional procedures. Critical issues, for the success of the robotic surgical interventions, such as anesthesia, are also addressed. Finally, the outcomes of robotic bariatric surgery are described, including long-term weight loss, improvement and resolution of comorbidities and improvement in quality of life. Bariatric Robotic Surgery is the first book specially devoted to this modality of surgical intervention. It is a fundamental tool for surgeons, residents and fellows who want to start a robotic bariatric surgery program. The book also helps experienced robotic surgeons to keep contact with the various available robotic surgical techniques.

Abstract
This book provides a guide for the management of patients with cardiovascular emergencies in the ER. It covers a broad range of the most important and frequent acute cardiovascular events including coronary syndromes, aortic syndromes, pulmonary embolism, and left heart failure. Pragmatic in nature, chapters discuss frequent clinical presentations in a thorough fashion while emphasizing a practical and concise approach to diagnosis and treatment. In addition, the book explores how new knowledge and technological advances are improving the quality of patient care in the emergency room, highlighting technological advances in the use of pharmacotherapy, biomarkers, and imaging techniques such as X-ray, echocardiography, CT, MRI, and nuclear. Cardiology in the ER: A Practical Guide is an essential resource for physicians and related professionals, residents, and fellows in cardiology, emergency medicine, intensive and critical care, and internal medicine.

The condition known as 'chest pain with normal coronary arteries' or 'cardiac syndrome X' has puzzled physicians since the advent of coronary arteriography. Although epicardial coronary artery spasm, as seen in Prinzmetal's variant angina, explains a proportion of cases of typical chest pain despite normal coronary arteriograms, many patients who seek medical attention for exertional and rest angina in the absence of obstructive coronary artery disease are not variant angina cases. This syndrom…

Abstract
The condition known as 'chest pain with normal coronary arteries' or 'cardiac syndrome X' has puzzled physicians since the advent of coronary arteriography. Although epicardial coronary artery spasm, as seen in Prinzmetal's variant angina, explains a proportion of cases of typical chest pain despite normal coronary arteriograms, many patients who seek medical attention for exertional and rest angina in the absence of obstructive coronary artery disease are not variant angina cases. This syndrome therefore constitutes both a diagnostic and therapeutic challenge Chest Pain with Normal Coronary Arteries has been written by many of the most active international research groups and comprehensively tackles the clinical presentation and the pathogenesis of the condition, as well as its management. Abnormalities of the coronary microcirculation have remained elusive to conventional imaging and researchers appear only recently to be making progress in obtaining much needed information in this field. The functional aspects of the coronary microcirculation, its clinical presentation and prognosis, as well as the diagnostic tests used for the assessment of microvascular dysfunction are important topics highlighted in this book, which also includes useful clinical diagnostic algorithms, thus bringing this subject closer to the practicing cardiologist. This book thus represents a practical tool for the clinician and a bank of information and new ideas for research scientists and clinical researchers interested in understanding the causes and mechanisms of the condition.

Abstract
This atlas comprehensively describes the application of computed tomography (CT) and magnetic resonance (MR) imaging in real-world scenarios using 192 illustrative clinical cases. These imaging techniques are revolutionizing the diagnostic and therapeutic approach for cardiovascular patients and are progressively becoming viable sub-specialties among radiologists and cardiologists. Clinical Atlas of Cardiac and Aortic CT and MRI features clinically relevant case-based examples of how CT and MR imaging techniques can be applied to identify the pathological features of a range of acquired and congenital heart diseases. Using more than 1000 high-quality figures of distinctive CT and MR imaging features of most cardiovascular diseases, both acquired and congenital, it therefore provides a valuable resource for both specialist and non-specialist radiology/cardiology practitioners seeking to develop a deep understanding of how to recognize the features of a variety of heart diseases using CT and MR imaging techniques.
