The purpose of this book is to bring together the latest findings on metabolic disorders that are strongly implicated in various critically ill patients. Since the beginning of the 20th century, maintaining the "milieu interieur" has been a major challenge for intensivists. In addition to considerable technological developments in intensive care units, important advances in our understanding of metabolic disorders observed in critically ill patients have been made during the 10 last years. Today, the intensivist can't ignore these disorders when selecting the most appropriate treatment for an illness. Cellular metabolic abnormalities are responsible for systems and organ failures, so the modern approach of organ dysfunctions now includes prevention or treatment of such disorders. This book is a comprehensive tool, allowing the physician to understand, diagnose and treat these metabolic disorders. Water, electrolyte, acid-base, glycemic and endocrinologic problems, as well as metabolic abnormalities observed in renal, cerebral and hepatic failure are presented in different chapters. The last part of the book is devoted to modern nutritional concepts, the consequences of energy modifications, mitochondrial dysfunction, hypothermia, oxidative stress and ischemia reperfusion, which open the way for new therapies.

This book provides a holistic and multi professional approach to maximising the chances of a successful recovery for the ICU patient surviving a period of critical illness. An admission to ICU with a critical illness has a well-recognised physical, psychological and social impact all on the individual and their loved ones of which need to remain in sharp focus from admission. Successful discharge from ICU requires significant attention to detail on all aspects of care concurrently to ensure a return to the best quality of life achievable. Early focus on structured rehabilitation and avoidance /prevention of ICU related complications are identified as paramount to improve both short and long term outcomes. This attention to detail needs to continue through the transition to the ward and planning discharge home. ‘Passport to a successful ICU discharge’ explores key areas to look forward to a successful recovery beginning with admission through to discharge in a holistic way. A Passport to discharge covers some key aspects to be achieved to the highest standards to create the best environment for success. A focus on promoting the best quality of life for the individual by careful attention to detail to benchmark every stage of their ICU journey which begins with admission. In particular, however, he presents a new approach videography that allows the ophthalmologist to document all (!) eye diseases with the slit lamp and a video camera in a practicable, rapid, and affordable manner. The necessary methods, techniques, and equipment, including converging, diverging, and contact lenses, are clearly explained in the text and four didactic videos. The 20 most important videographic settings are described, and recipes for their use are presented, along with diagnostic tips. The accompanying collection of clinical images represents the world's first ever general atlas of ophthalmology from the perspective of the slit lamp. The author has already demonstrated proof of the value of ophthalmological videography in his own practice. It is his hope that this introductory book, with its many images never previously produced with a slit lamp, will stimulate others to exploit the approach's potential.
