Abstract
Indications for central venous cannulation in critically ill patients have increased dramatically in recent years owing to the use of more complex therapies unsuitable for delivery via peripheral veins. The main drawbacks of central venous access are morbidity and the growing scarcity of experienced operators. Ultrasound-guided peripheral venous access offers a solution, in that it reduces morbidity and can be performed by a dedicated nursing team. The aim of this book is to teach the fundamentals of this emerging technique. Advice is provided on the choice of materials and on vein selection. The advantages and disadvantages of peripherally inserted central venous catheters (PICC) in relation to other types of central venous catheter are discussed, and the principles of use and practical applications of ultrasound for venipuncture are explained. Maneuvers for PICC positioning, techniques for the evaluation of PICC tip placement, and the prevention, diagnosis, and management of complications are all described in detail. Advice is also provided on the organization of a dedicated PICC team within a hospital or a supportive care program, and psychological, legal, and economic issues are considered.

Abstract
This is the first textbook entirely dedicated to the endovascular treatment of complications related to arteriovenous accesses for hemodialysis (native fistulas and prosthetic grafts). The book addresses the anatomy of upper limb arteries and veins as well as the clinical presentation of patients and the way to perform and read a fistulogram. Many details and illustrations are provided, clarifying the subtleties in catheterization and the dilation of stenoses. From a strategic point of view, it is explained for the first time that many stenoses must either be ignored or deliberately underdilated. Many images help understand the different stages of recovery from thrombosed accesses. The number of incident dialysis patients increases by 5 % every year, which means that this is a field of growing interest. There are several annual or bi-annual meetings held by numerous national or international societies (vascularaccesssociety.com, sfav.org, vasmad.org, dialysiscontroversies.org, asdin.org).

Abstract
Much of the gains in survival over the last 50 years in pediatric oncology have occurred through successively more intensive treatment regimens and concomitant improvement in the supportive care required to manage the complications of such regimens. Supportive Care in Pediatric Oncology provides a thorough, up-to-date review of the medical literature to provide an evidence-basis and grading of recommendations for supportive care management. Where evidence is lacking, consensus guidelines and "expert" opinion are also graded based on the available literature to support such recommendations. Commonly encountered acute issues are addressed including the management of febrile neutropenia, prevention of infection and immunization practice; transfusion support and utilization of hematopoietic growth factors; tumor lysis syndrome and hyperleukocytosis; neurologic, cardiopulmonary and abdominal emergencies; pain, nausea and vomiting, and mucositis; nutritional issues; thrombosis and management of central venous catheters; and radiation side effects. Finally, knowledge gaps and opportunities for research within the supportive care field are highlighted.
