By using this unique method, practitioners ranging from surgeons-in-training, gastroenterologists-in-training and those early in their career to senior colorectal specialists and gastroenterologist who want to incorporate or improve their advanced endoscopic skills will be able utilize techniques and learn from this gathering of experts. The guiding principle of this work is to create a resource for surgeons and gastroenterologists that extends beyond the currently available texts, and that surgeons and gastroenterologists can turn to when wanting to "brush up" on techniques, find a useful "tip or trick" for a complex patient, or simply learn a reproducible methods for advanced endoscopic procedures. This unique book highlights current knowledge, demonstrates standards of medical care, and provides clear step-by-step reproducible techniques even for the most advanced procedures. Beyond the simple application of technical knowledge the book addresses the deeper questions about the optimal "next step" in dealing with more complex patients. International experts also address future challenges and innovations in lower gastrointestinal endoscopy. Finally, it focuses on specific "tips and tricks" that experts in the field have learned. The format follows that of both a "how to" manual as well as an algorithm-based guide to allow the reader to understand the thought process behind the proposed treatment strategy. Throughout the text, each author provides an ongoing narrative of his/her individual techniques along with color illustrations and diagrams to "personally" take the reader through the crucial steps of the procedure, and key points of patient care inherent to that topic. Additionally, where appropriate, links to online videos will give the reader an up-front look into technical aspects of EMR, ESD, endoscopic stent placements, CELS, as well as NOTES. This book will be useful to colorectal surgeons, general surgeons, and gastroenterologists who want to learn or improve their skills in lower gastrointestinal endoscopy and advanced endoscopic interventions. It describes the clinical presentation of the syndrome, including its huge variety of symptoms and the variability of its natural course.The book starts out with several vivid vignettes, and then explains how and why the concept was introduced. In addition, it explains what is currently known about the underlying medical conditions and brain mechanisms, proposes a diagnostic algorithm, and makes recommendations for treatment. Throughout the book, a recurring question is whether or not Charles Dodgson (aka Lewis Carroll) suffered from the symptoms he described so aptly in Alice’s Adventures in Wonderland. Accordingly, it should appeal to anyone interested in the brain and its disorders, as well as readers interested in the life of Lewis Carroll.

Abstract
This textbook on burn care is focused specifically on the needs of a surgeon. It provides a "how to do" text that presents the practical strategies for initial resuscitation, skin grafting, burn-specific critical care and ultimately guide the surgeon for the best outcomes for the burn surgeon. It provides the reader with basic pathophysiology but avoids a detailed review of the molecular science of burns. Guidelines for basic care with the chapters covering the timeline used for each patient spanning from initial resuscitation (ABCs) to managing their re-socialization are provided. The text covers the key components to the initial management of the burn patient including airway, breathing and circulation. The reader is introduced to the metabolic changes of a major burn and how nutrition and pharmacologic manipulation affects these systemic effects of a burn injury. The management of burn sepsis as well as the basic concepts of wound healing of the different depths and severity of burns are covered. Another chapter covers the basic techniques of skin grafting, rehabilitation and reconstruction with a focus to optimize the cosmetic and functional outcome of the burn. Special chapters cover management of outpatient burns and pediatric injuries. Special burns, electrical, chemical, cold injuries and finally the treatment of other diseases that lead to massive skin loss are covered. Special considerations for pain, scar management, psychosocial recovery, intentional burns, burn prevention and disaster preparation complete the text. Abstract
Written by leaders in the field, the third edition of Common Surgical Diseases: An Algorithmic Approach to Problem Solving, provides surgical residents and medical students with a current, concise and algorithmic approach to frequently encountered clinical challenges. Each chapter details every common surgical disease in the form of a succinct text coupled with step-by-step algorithm. It also walks the reader through the evaluation, diagnosis, treatment and follow-up of the most common surgical problems. Thoroughly updated and revised, the third edition focuses on problems most frequently encountered by general surgeons and their residents and students. More factual information is included in the form of charts and tables for quick and easy reference. The section on critical care is updated and expanded. The section on pre-operative considerations has new chapters on how to best manage patient's medications before surgery (anticoagulants, anti-platelet drugs) and prophylaxis of deep venous thrombosis. Other new chapters include access for hemodialysis, adrenal incidentaloma, esophageal cancer, pancreatic cancer, management of abdominal wall defects, hyperglycemia, necrotizing soft tissue infections and SIRS/sepsis. Over the past decades extraordinary advances have been made in the understanding of cellular and molecular processes of wound healing and the pathobiology of chronic wounds. This knowledge has led to wound care innovations that facilitate more rapid closure of wounds with better functional and aesthetic outcome. A sensible and resource-saving utilization of these innovative technologies requires a broad knowledge of these processes and innovations. This book intends to give an overview about today's wound care developments in tissue engineering and skin replacement. Abstract
Primary care physicians are often the first caretakers to whom patients present for chronic pain. The current literature targeted at these providers is sparse, and the existing literature is very broad and large in scope and size. There are no quick pocket guides on this topic that the general practitioner may use as a point of care reference. This first of its kind text provides a practical, hands-on approach to the evaluation and treatment of chronic pain in the family medicine setting. Divided into four main sections, the first part focuses on the general approach to any patient suffering from chronic pain. Chapters in this section include the physical exam, formulating a diagnosis, formulating a variety of treatment plans including medication, physical therapy and psychiatric therapy, and specialist referral, among others. The second section focuses on evaluation and management of the chronic pain patient who requires opioid therapy. This section includes useful guidance on how to convert into morphine equivalents, interpretation of urine drug tests, and helpful office procedures for managing refills. It gives practical guidance on how to prescribe opioid reversal agents and opioid tapering regimens. A systems based approach to managing the patient is also discussed. The third section informs the reader of viable alternative and complementary treatment options. Five main treatment types are discussed each with their own separate chapter. These include musculoskeletal-based therapy, medicinal therapies, sense-based therapy, relaxation-based therapy and physical/exercise-based therapy. Given the controversy and limited training primary care physicians have on marijuana and cannabis, one chapter is dedicated specifically to inform the primary care physician on marijuana as a medicinal therapy for chronic pain. The fourth section focuses entirely on myofascial pain and trigger point therapy. The chapters in this section teach the reader how to examine and diagnose myofascial pain and distinguish between fibromyalgia. They also provide general principles of myofascial trigger point therapy and how to practically perform these in sthe clinical setting. Topics discussed include: trigger point massage, cold and stretch, isometric contraction, trigger point injection, and ischemic compression. It also includes general instruction so that the practitioner can teach patients how to perform trigger point therapy easily in their own home. The last chapter details chronic myofascial back pain and how to examine and practically treat with trigger point therapies. Evaluation and Management of Chronic Pain is a first-of-its-kind pocket-guide text specifically designed for primary care providers. It also appeals to residents, medical students and any other professional interested in treating chronic pain.

Abstract
Unmet healthcare needs of young populations and individuals in suboptimal health conditions are the key issue of currently observed epidemics of non-communicable disorders. Moreover, an unprecedented decrease in the average age of onset of these disorders is recorded. The majority of non-communicable disorders carry a chronic character by progressing over a couple of years from a reversible suboptimal health condition to irreversible pathology with collateral complications. The time-frame between both conditions is the operational area for predictive diagnosis and identification of persons at risk by innovative screening programmes followed by the most cost-effective personalised treatment possible, namely primary prevention tailored to the person. The book propagates the paradigm change from delayed, costly but frequently ineffective medical services to the holistic approach by predictive, preventive and personalised medicine clearly demonstrating multifaceted benefits to the individual, healthcare sector and society as a whole. The book is focused on the needs of young people: teenagers, adolescents and young adults; regardless of the age, individuals in suboptimal health conditions, who are interested in remaining healthy by optimising their modifiable risk factors – both endogenous and exogenous ones; several patient cohorts demonstrating similar phenotype of Flammer syndrome.
