Abstract
This book deals with a special form of facial rejuvenative surgery, which considers the three-dimensional anatomy of the face and benchmarks it against biomechanical models of the face. Traditional approaches to the ageing human face have been based on a two-dimensional approach. The authors, however, consider the human face using a three-dimensional platform that reflects height, width and depth and thus defines spatial divisions or pilasters. The anatomically accurate and bio-physically based three-dimensional computational models, originally used to simulate facial expressions and subsequently in the dynamic evaluation of the various soft tissues, offer a valuable new tool for surgical techniques. The book is divided into four chapters, the first of which describes the historical developments, evolution of facelift procedures and review of traditional two-dimensional approach. The second chapter presents three-dimensional surgical anatomy of the face and neck with the introduction of "facial pilasters". Treating the human face as a collection of compliant and rigid structural components which obey engineering principles and its biomechanical behaviour is discussed in chapter three. The last chapter provides the reader comprehensive information on the surgical procedure for contour vector congruent facelift, including case studies. An alternative approach (Rejuvenative Facial Lipomorphoplasty) to the de-volumed ageing face with little sagging and skin excess is introduced together with case studies. This book was motivated by the desire to define the key steps in facelift techniques and provides an essential guide for young plastic surgeons, as well as experienced surgeons who are dissatisfied with the current two-dimensional platform.

This is addressed in Diet and Nutrition in Critical Care. This major reference work encapsulates the latest treatments and procedures to meet the dietary and nutritional needs of the critically ill. Where evidence is available this is presented. However, where evidence is absent, the authors highlight this and provide guidance based on their analysis of other available data and their clinical experience. Diet and Nutrition in Critical Care is a two volume set which addresses the needs of all those concerned with diet and nutrition in the critically ill and covers General Aspects, Enteral Aspects, and Parenteral Aspects. Each volume is stand alone and is further divided into separate sections. The Sections in the General Aspects are: General conditions in the severely ill; General metabolic effects and treatments; Assessment protocols; General nutritional aspects; Specific nutrients; Adverse aspects. The next two volumes Enteral Aspects and Parenteral Aspects contain the following sections: General aspects and methods; Specific nutrients; Specific conditions; Adverse aspects and outcomes; Comparisons or dual parenteral and enteral support or transitions; Preclinical studies. The authors recognise the limitations in simplistic divisions and there is always difficulty in categorising treatment regimens. For example, some regimens involve transitions from one feeding protocol to another or the development of co-morbid conditions and in some cases enteral support may be supplemented with parenteral support. This complexity however, is addressed by the excellent indexing system. Contributors are authors of international and national standing, leaders in the field and trendsetters. Emerging fields of science and important discoveries relating to artificial support will also be incorporated into Diet and Nutrition in Critical Care. This textbook addresses the themes that are at the forefront of neonatal clinical care and research, based on natural divisions in care during pregnancy, and postnatally by gestational age at birth. The book offers a unique approach, in that it proposes discussion of important general principles underpinning neonatal care that are not addressed in most general neonatology textbooks, such as ethical issues, counselling, effective training methods, quality and safety, among other subjects. These are fundamental aspects and challenges that need to be appreciated by senior clinicians. A chapter authored by parents describing their perspectives of neonatal intensive care is unique and will be highly educational, with the potential to influence the way in which individuals view and deliver neonatal care. The authors discuss common and important conditions, to promote adoption of sound evidenced based practice where this is available. However, where evidence is limited, as is the case in many areas of neonatal practice, the authors aim to encourage critical thinking and evidence appraisal, which are necessary skills for busy clinicians wishing to filter evidence to guide delivery of care. This text is suitable for senior trainees wishing to pursue a career in neonatal medicine, early career neonatologists and paediatricians with an interest in neonatology. It is also of interest to established neonatologists wishing to update their neonatal knowledge. This book explains the Food and Drug Administration (FDA) as an institution and provides understanding of its nomenclature, societal role, policies, goals and challenges. While many readers will have some knowledge of the FDA, few have an appreciation of the many specific areas of FDA authority. For example, how many realize that the US is one of only two countries which allow direct-to-consumer drug advertising, which is regulated by FDA? Or that FDA itself advertises to try to prevent young people from smoking cigarettes, and that all proprietary drug names must be approved by FDA? How many doctors or other readers are familiar with the formal definition distinguishing drugs from devices and the importance this has for development costs and for our knowledge about the ultimate products? How many know how much nutritional supplements are regulated by FDA? The FDA for Doctors is not for those looking for detailed instruction on dealing with the FDA or its operations. Rather, it is written by a doctor with doctors in mind, with the hope that the information in this book will make physicians, and other readers, more thoughtful and insightful, especially with regards to therapeutics and the many broad societal issues underlying FDA's activities. With over 40 years of experience as a clinical investigator in many trials done for FDA registration, the author has been a member of and chaired an FDA advisory committee, and has been a consultant to several divisions of the FDA. He has also had the opportunity to serve on the U.S. Senate Labor Committee, helping with its FDA oversight activities. In addition to a long academic medical career the author has been a consultant to and a full time employee of drug and device companies.

Abstract
The oocyte is the largest and most mysterious cell in the body of mammalian organisms. Through its growth and maturation, it reaches extreme levels of specialization, while maintaining at the same time a condition of totipotency. Its unique ability, in co-operation with the spermatozoon, to give rise to a fully developed organism formed from hundreds of different tissues and myriads of individual cells has inspired intellectuals of all ages. Oogenesis finds impetus and purpose in casting scientific perspective towards this unique cell for the benefit of scientists and assisted reproductive technology (ART) specialists. The authors of the chapters are distinguished authorities in their respective areas of competence. From the opening of the book the reader is lead on a fantastic voyage from the formation of the primordial oocyte to the development of the early embryo, passing through crucial processes of oogenesis, such as co-ordination of oocyte and follicle growth, gene expression and organelle reorganization during growth and maturation, epigenetic mechanisms, regulation of meiosis, totipotency, cell polarity, oogenesis in vitro and maternal regulation of early development.
