Abstract
This superbly illustrated book describes how techniques previously developed by Dr. Rollin Daniel for use in open rhinoplasty can be adapted for use in the closed approach. It is the author's opinion that this combination offers greater feedback during surgery, a shorter recovery period, and absence of scar. Full explanation is provided of preoperative preparation, including evaluation of the nasal surface aesthetics using the concept of geometric polygons as aesthetic subunits to define both the existing deformity and the aesthetic goals. The surgical techniques developed and modified to achieve the desired surface appearance are then described step by step with the aid of a wealth of color photos. It is illustrated how the novel dissection and redrape control methods reduce the healing time and enhance outcome, and an extensive series of case analyses is presented to document the benefits of the approach. The book is written in a "cookbook format" that will enable plastic surgeons quickly to learn how to utilize the closed approach to rhinoplasty to optimal aesthetic benefit.

Abstract
This book introduces the reader to a clear and consistent method for in-depth exploration of subjective psychopathological experiences with the aim of helping to restore the ability within psychiatry and clinical psychology to draw qualitative distinctions between mental symptoms that are only apparently similar, thereby promoting a more precise characterization of experiential phenotypes. A wide range of mental disorders are considered in the book, each portrayed by a distinguished clinician. Each chapter begins with the description of a paradigmatic case study in order to introduce the reader directly to the patient's lived world. The first-person perspective of the patient is the principal focus of attention. The essential, defining features of each psychopathological phenomenon and the meaning that the patient attaches to it are carefully analyzed in order to "make sense" of the patient's apparently nonsensical experiences. In the second part of each chapter, the case study is discussed within the context of relevant literature and a detailed picture of the state of the art concerning the psychopathological understanding of the phenomenon at issue is provided. An Experiential Approach to Psychopathology, and the method it proposes, may be considered the result of convergence of classic phenomenological psychopathological concepts and updated clinical insights into patients' lived experiences. It endorses three key principles: subjective phenomena are the quintessential feature of mental disorders; their qualitative study is mandatory; phenomenology has developed a rigorous method to grasp "what it is like" to be a person experiencing psychopathological phenomena. While the book is highly relevant for expert clinical phenomenologists, it is written in a way that will be readily understandable for trainees and young clinicians.

Abstract
Patient empowerment is examined as a multi-dimensional factor influencing the use of diabetes self-management apps. The research design includes three studies conducted in Singapore. Study 1 examines how features of diabetes self-management apps correspond with theoretical indicators of empowerment, as well as app quality. Study 2 uses semi-structured face-to-face interviews with diabetes patients to draw first conclusions about the relevance of empowerment for diabetes app use. Study 3 includes an online patient survey, and uses cluster analytical methods to test the preliminary Study 2 results (typology of app use), as well as binary logistic regression to compare the strength of influence of various anteceding factors on the likelihood of diabetes app use. The studies show that especially the support by private social patient networks and the medical specialties of supervising physicians play a crucial role for technology-supported self-management.

This comprehensive, evidence-based textbook presents the latest knowledge on bad breath, describing insights from basic research and offering expert guidance on current approaches to diagnosis and treatment. The new edition has been thoroughly updated to take account of recent advances in understanding. The opening chapters examine in detail the origins of bad breath, including from the nose and pharynx. Microbial and biochemical aspects are fully explained, and information is also provided on odor perception. The clinically focused chapters cover all forms of laboratory measurement of breath odors, chairside diagnostic techniques, and the full range of available treatment options. The value of self-administered questionnaires in assessing bad breath is discussed, and attention is paid to the problem in specific population groups and to psychological aspects. Finally, future prospects are reviewed. The authors draw on more than forty years of combined experience in the field, both in the laboratory and as consultants to thousands of patients. The book will be of interest to all dentists as well as ENT specialists and family physicians.

This book explores, through case studies, the interplay between religion, culture, government, and politics in diverse societies on questions arising in the domain of bioethics. The case studies draw from multiple disciplinary perspectives, including history, theology, law, bioethics, public policy, science, and medicine. The text's global perspective permits a comparison of the differing approaches adopted by countries facing similar bioethical quandaries and the extent to which religion has or has not been instrumental in addressing such dilemmas. Secular and religious societies across the globe are being confronted with complex questions involving religious belief and the extent to which specific religious perspectives have in the past or should in the future be adopted as official policy. Bioethical issues involving the interplay of religion and government have become particularly notable in recent years. How these issues are resolved has major implications for individuals, healthcare providers, and the future of medical research and medical care. Topics explored among the chapters include: Homosexuality: Sin, Crime, Pathology, Identity, Behavior Medical Error: Truthtelling, Apology, and Forgiveness Refusal of Medical Treatment Medical Deportation Case Study: Nazism, Religion, and Human Experimentation The New Frontier: Cloning Case Studies in Society, Religion, and Bioethics will find an engaged audience among researchers and scholars in history, religion/theology, medicine, and bioethics interested in the influence of religion on bioethical decision-making. Students—particularly upper-level undergraduate and graduate students interested in bioethics, humanities, and theology—will find the text helpful in understanding the processes through which religion may serve as a basis for both societal policy and law and individual decision-making in health-related matters.

Abstract
Those familiar with the 1st edition of this book from 2008 will recognize the original 62 cases that have been modernized with updated references and enjoy the addition of 41 brand new cases. Clinical Anesthesia: Near Misses and Lessons Learned, 2nd edition is a collection of actual cases, complied from the author's forty years of practice in major metropolitan hospitals in the United States, Norway and South Africa. It offers the reader succinct case presentations describing a problem on one page and a solution on the next, with a discussion, other potential solutions with satisfactory outcomes, and tips to help avoid problems altogether when possible. Clinical Anesthesia: Near Misses and Lessons Learned, 2nd edition serves as an easy and practical way for the reader to gain familiarity with potentially unexpected problems in clinical anesthesia In addition, this can be an excellent study aid for the American Board of Anesthesiology oral exam. Since near misses are relatively rare, there really is no way to be prepared to successfully manage such crises other than reading about them, so residents, fellows, and practicing, certified registered nurse anesthetists will benefit from the learning of these actual near misses. Additionally, faculty will find the plethora of discussion topics for large or small group settings an ideal way to communicate anesthesia and clinical care problems to the audience.

This book is designed to serve as an up-to-date reference on the use of cone-beam computed tomography for the purpose of 3D imaging of the craniofacial complex. The focus is in particular on the ways in which craniofacial 3D imaging changes how we think about conventional diagnosis and treatment planning and on its clinical applications within orthodontics and oral and maxillofacial surgery. Emphasis is placed on the value of 3D imaging in visualizing the limits of the alveolar bone, the airways, and the temporomandibular joints and the consequences for treatment planning and execution. The book will equip readers with the knowledge required in order to apply and interpret 3D imaging to the benefit of patients. All of the authors have been carefully selected on the basis of their expertise in the field. In describing current thinking on the merits of 3D craniofacial imaging, they draw both on the available scientific literature and on their own translational research findings.

Abstract
This edited volume presents a detailed portrait of couples living with mixed HIV status, where one partner is HIV-positive and the other negative. Readers will come to understand the various and complex ways in which these mixed-status, or serodiscordant couples build a life together within the shadow of HIV-related stigma. Spanning the globe, coverage explores serodiscordance as a negotiated practice and process, inseparable from the social context in which it is situated. The book shows how couples draw on diverse and sometimes contradictory cultural discourses of medicine, romance, and "normality" to make sense of and manage their mixed HIV status and any perceived risks, not uncommonly in ways that depart from prevailing HIV prevention messages. Throughout, compelling personal stories accompany the empirical research, sharing the firsthand experiences of men and women in serodiscordant relationships. Bringing together research from diverse disciplines and geographical regions, this book contributes important insights for future HIV health promotion as well as offers new knowledge to scholarship on the cultural intersections of illness and intimacy. It will appeal to a broad audience working across the fields of HIV, health, gender, sexuality, development, and human rights.

Abstract
The ethnic composition of Britain's population is changing rapidly as the 21st century progresses. Some commentators predict a catastrophic future of increasing ethnic segregation and community breakdown, whereas others say that the benefits of ethnic pluralism and cultural diversity will lead to a more integrated society. This book addresses many of the key issues and debates associated with ethnicity and integration in Britain. It provides the reader with an enhanced understanding of ethnic population change and residential concentration, ethnic household dynamics, internal and international migration, the relationship between ethnicity and health, crime, identity and language, as well as ethnic population projections. Collectively, the findings presented here constitute an evidence base for policymakers and practitioners to draw upon when formulating solutions to the range of problems at local, regional and national level that are associated with an increasingly multi-ethnic society. This book is the third volume of the interdisciplinary series entitled "Understanding Population Trends and Processes". Volume 1 reports the findings of research on fertility, living arrangements, care and mobility, whilst Volume 2 contains selected studies of spatial and social disparities in Britain using various secondary data sets.

Abstract
Childhood acute leukemias are one of the main causes of death in children aged 1 to 14 years in some countries; and unfortunately, we have been unable to prevent it. Certainly, a good parcel of it is due to the poor understanding about its etiology. This book aims to describe the most important theories and hypothesis regarding childhood acute leukemia. Written by the most outstanding researchers in the field, this book intends to contribute to a greater understanding of the etiology of this disease. It goes beyond the simple and common analysis of risk factors, which hardly allows us to draw definite conclusions. By addressing the etiology of the disease, discussing from molecular biology until epidemiology and clinical manifestations, this book will guide present and future approaches, contributing for a better clinical management of leukemia in children. The knowledge regarding etiology is a crucial step for a better evaluation, prevention and treatment of a disease. Thus, this book finally intends to provide such knowledge, allowing physicians and practitioners to a better manage of childhood acute leukemias.

This book presents the current clinical evidence on the efficacy of herbal and nutritional treatments for anxiety that is experienced in association with psychiatric disorders, and explains how health professionals can apply this knowledge to the benefit of patients presenting with a wide range of symptoms, including comorbid mood disorders. All chapters are written by world-leading researchers who draw on the findings of human clinical trials to provide uncompromising assessments of individual treatments, including herbal anxiolytics with sedative actions, adaptogens, cognitive anxiolytics, and nutraceuticals. Traditional treatments requiring further study--including the plant-based psychotropic Ayahuasca and other phytotherapies of potential value in the treatment of anxiety--are also reviewed. In the closing chapters, a series of helpful case studies are provided by mental health clinicians in order to illustrate how herbal and nutritional treatments can best be integrated into an overall treatment plan for individuals with a range of comorbid diagnoses. Mental health professionals, researchers, and general readers will find that the book provides an excellent review of current scientific knowledge gained from the study of herbal and nutritional treatments, together with important clinical recommendations for their use in patients experiencing clinically significant levels of anxiety.
