Abstract
This book focuses on the fundamentals of the use of biologics in asthma, describing the rationale, principles, mechanisms of action, and indications. It offers an excellent balance between basic science and the analysis of clinical trials, updating readers with new developments that are changing the global scenario for targeted biological anti-asthma therapies, especially with regard to more severe disease. A range of therapies are considered, from the humanized monoclonal anti-IgE antibody omalizumab, widely approved as add-on treatment for inadequately controlled disease, through to emerging biologics for which evidence supportive of efficacy is accumulating, including anti-IL-5, anti-IL-4, and anti-IL-13 therapies. One aspect to emerge is the variability in individual response, which suggests a need for characterization of different asthma subtypes to permit the effective implementation of phenotype-targeted treatments. This book will be of interest for pulmonologists, clinical immunologists, and physicians seeking sound information on these therapies, but also for scientists and pharmacologists wishing to enhance their knowledge of the therapeutic implications of the cellular and molecular mechanisms that underlie severe, uncontrolled asthma.

This book offers a step-by-step guide to mental health assessment for bariatric surgery patients. A general introduction explains the concepts of bariatric psychology and psychiatry, their relevance in contemporary bariatric surgery, and reasons to include psychologists and psychiatrists in multidisciplinary teams taking care of bariatric patients. The following four chapters address the aspects of mental health that are investigated by bariatric psychology. The psychological processes analyzed here play a major role in influencing patients’ perception of the outcomes of bariatric surgery and in determining their commitment to lifestyle changes and follow-up programs. The second part of the book includes nine chapters addressing the clinical conditions relevant to bariatric psychiatry. For each condition, the major focus is on the impact of psychopathology on bariatric surgery outcomes (weight loss, weight regain, quality of life) and the impact of surgery on its course (remission, worsening, de novo onset). Each chapter in this part includes a discussion of the diagnostic instruments (i.e., structured interviews, clinician-rated tests, and patient-rated tests) that should be used to obtain a valid assessment of the patient’s mental status. Separate chapters focus on psychiatric complications (e.g., suicide and addiction transfer) and psychological problems related to quality of life (e.g., body image dissatisfaction) that may emerge postoperatively. Data on these postsurgery conditions has been reported only recently and, therefore, no published book deals with them. The final chapter offers an overview of unsolved issues in bariatric psychology and psychiatry and reviews emerging research findings that are likely to change assessment and care of bariatric patients’ mental health in the near future. Given its scope—and its wealth of tables, diagrams, mnemonics, and key fact boxes—the book will be an invaluable reference tool for clinicians.

Abstract
Diffusion MRI allows indirect estimation of the perfusion and cellularity of normal and abnormal tissues and has conventionally been used in the brain in order to evaluate ischemia, tumors, and other conditions. Application in other parts of the body has been hampered by the occurrence of motion and susceptibility artifacts. However, recent advances in MR technology are overcoming these hurdles, allowing the use of diffusion MRI in organs outside the brain. In this book, the authors present cases drawn from daily clinical practice to illustrate the role of diffusion sequences, along with other morphological and functional MRI information, in the work-up of a variety of frequently encountered oncological and non-oncological diseases. Breast, musculoskeletal, whole-body, and other applications are covered in detail, with careful explanation of the pros and cons of diffusion MRI in each circumstance. Quantification and post-processing are discussed, and advice is provided on how to acquire state of the art images, and avoid artifacts, when using 1.5- and 3-T magnets. Applications likely to emerge in the near future, such as for screening, are also reviewed. The practical approach adopted by the authors, combined with the wealth of high-quality illustrations, ensures that this book will be of great value to practitioners.

Abstract
This open access book about the Zadeh Project demonstrates and explores a core question in clinical ethics: how can ethics consultants be accountable in the face of a robust plurality of ethical standpoints, especially those that underwrite practices and methods for doing ethics consultation as well as those viewpoints and values encountered in daily clinical ethics practice? Underscoring this question is the recognition that the field of clinical ethics consultation has arrived at a crucial point in its maturation. Many efforts are underway to more formally “professionalize” the field, with most aimed toward stabilizing a specific set of institutional considerations. Stretched between these institutional and practical initiatives resides a crucial set of of ethical considerations, chief among them the meaning and scope of responsibility for clinical ethics consultants. Developed around a long-form case scenario, the Zadeh Project provides a multi-layered series of “peer-reviews”: critique of the actions of the case scenario’s ethics consultant; reflection on clinical ethics method; examination of the many ways that commitments to method and practice can, and do, intersect, overlap, and alter one another. The design and format of this book thus models a key element for clinical ethics practice: the need and ability to provide careful and thoughtful explanation of core moral considerations that emerge among diverse standpoints. Specifically designed for those studying to become and those who are ethics consultants, this book, with its innovative and multi-layered approach, allows readers to share a peer-review-like experience that shows accountability to be what it is, an ethical, not merely procedural or administrative, undertaking.

Abstract
Drawing from the work of academics and practitioners from ten states across the country, this edited volume showcases and synthesises the diversity and richness of efforts to understand and act on the social determinants of health in India, the conditions in which we are born, grow, live work and age. Such an effort is salient in the current era of Sustainable Development Goals (SDG), which have foregrounded the issue of equity and the need for a comprehensive, multi-sectoral agenda for health and development. In India, particularly in the last decade, there have been myriad efforts to more critically theorise and intervene in areas with bearing on health, like conflict, nutrition or urbanisation, or to address the concerns of vulnerable groups like women, children and the elderly. From these efforts emerge lessons of convergence for academic and policymaking institutions in India who are looking to operationalise and bring life to the SDG agenda in India and other Low and Middle Income Country settings. The book comprises eleven chapters and six short commentaries that appear in conversation with each other, as well as an annexure of validated, ready to use indicators for monitoring of social determinants of health.
