Abstract
This textbook reviews the novel techniques employed in corneal transplantation. It will assist fellows and corneal surgeons in using these techniques to best effect and in selecting patients for surgical procedures, taking into account the benefits and risks. Corneal transplantation has been performed for more than 100 years. Until 15 years ago the state-of-the art type of transplantation was penetrating keratoplasty. Since the start of this millennium, however, important advances have been made in developing new surgical techniques. Today, the vast majority of keratoplasty procedures are performed as delicate lamellar procedures, either with the assistance of fine microkeratomes or femtosecond lasers or using very advanced surgical dissection procedures. Corneal Transplantation provides detailed information on these and other advances, which have helped patients undergoing keratoplasty to achieve a much faster visual recovery and a more stable eye with less risk of rejection episodes.

Abstract
Manfred Georg Krukemeyer first describes magnetic drug targeting, by which is meant the focused application of drugs, e.g. the introduction of a cytostatic, and which is aimed at optimising its locally therapeutic effect. The author thus achieves a high concentration of cytostatic at the site of the tumour. The particles have to be large enough to be attracted by a magnetic field and so find their way into the tumour, where they release their active ingredient. The aim of the hypothesis formed by one of the author's study groups was to show in a scientific experiment that in this case cytostatics coupled with iron particles [Fe3O4] can be introduced into a tumour. The focused introduction of drugs—magnetic drug targeting—results in faster arrival of the active ingredient at the target site together with a reduced quantity of active substances. For this reason magnetic drug targeting is also interesting from an economic point of view. This book provides step-by-step instructions on how to operate with 27G instrumentation in a wide range of surgical indications, including vitreous floaters, macular holes, dropped nucleus, retinal detachment, diabetic retinopathy, submacular hemorrhage, retinopathy of prematurity and much trauma. All surgeries are approached in cookbook fashion, with initial coverage of the "ingredients" (devices and instruments) and then meticulous description of preparation and performance with supporting photographs, drawings, and videos. In addition, the differences and benefits in comparison with 23G and 25G vitrectomy are highlighted. Small-gauge vitrectomy has radically changed the ways in which vitrectomy is performed. 27G vitrectomy is the most recent and most exciting development in small-gauge vitrectomy. The healing time is faster but the small diameter prolongs the duration of surgery. This obstacle has been overcome with powerful new vitrectomy machines and revolutionary vitreous cutters with two cutting blades making 27G vitrectomy as fast as 25G. This new equipment has expanded the indications for 27G surgeries immensely from detachment surgery to retinopathy of prematurity and the advent of new instruments may make 27G the gold standard in the future. This book, now in an extensively updated and expanded third edition, offers important insights into polymyalgia rheumatica (PMR) and giant cell arteritis (GCA), providing an overview of their pathophysiology, genetics, immunology, and endocrinological mechanisms but above all presenting the state of the art in diagnosis and treatment. Both PMR and GCA are diseases of advanced age. Early diagnosis and treatment can save one of the most precious senses vision. In this book, the role of imaging techniques is thoroughly examined, with particular attention to ultrasonography and FDG PET. All aspects of management of both diseases are discussed, including the current use of glucocorticoids, the efficacy of modern biological treatment, such as infliximab, tocilizumab, and rituximab, and the available monitoring tools. Diagnostic criteria for PMR and GCA are outlined, with discussion of differential diagnosis. Among the other topics addressed are disease classification, the histopathology of GCA, the ACR criteria for temporal arteritis, the value of temporal artery biopsy in GCA, serum cytokine profiles, chronobiology, and sine syndromes. The reader will find that the book aids in achieving early diagnosis and providing faster and more effective disease management. Abstract
For the first time in a thousand years, Americans are experiencing a reversal in lifespan. Despite living in one of the safest and most secure eras in human history, one in five adults suffers from anxiety as does one-third of adolescents. Nearly half of the US population is overweight or obese and one-third of Americans suffer from chronic pain – the highest level in the world. In the United States, fatalities due to prescription pain medications now surpass those of heroin and cocaine combined, and each year 10% of all students on American college campuses contemplate suicide. With the proliferation of social media and the algorithms for social sharing that prey upon our emotional brains, inaccurate or misleading health articles and videos now move faster through social media networks than do reputable ones. This book is about modern health – or lack of it. The authors make two key arguments: that our deteriorating wellness is rapidly becoming a health emergency, and two, that much of these trends are rooted in the way our highly evolved hardwired brains and bodies deal with modern social change. The co-authors: a PhD from the world of social science and an MD from the world of medicine – combine forces to bring this emerging human crisis to light. Densely packed with fascinating facts and little-told stories, the authors weave together real-life cases that describe how our ancient evolutionary drives are propelling us toward ill health and disease. Over the course of seven chapters, the authors unlock the mysteries of our top health vices: why hospitals are more dangerous than warzones, our addiction to sugar, salt, and stress, our emotionally-driven brains, our relentless pursuit of happiness, our sleepless society, our understanding of risk, and finally, how world history can be a valuable tutor.
