Abstract
This book provides step-by-step instructions on how to operate with 27G instrumentation in a wide range of surgical indications, including vitreous floaters, macular holes, dropped nucleus, retinal detachment, diabetic retinopathy, submacular hemorrhage, retinopathy of prematurity and much trauma. All surgeries are approached in cookbook fashion, with initial coverage of the "ingredients" (devices and instruments) and then meticulous description of preparation and performance with supporting photographs, drawings, and videos. In addition, the differences and benefits in comparison with 23G and 25G vitrectomy are highlighted. Small-gauge vitrectomy has radically changed the ways in which vitrectomy is performed. 27G vitrectomy is the most recent and most exciting development in small-gauge vitrectomy. The healing time is faster but the small diameter prolongs the duration of surgery. This obstacle has been overcome with powerful new vitrectomy machines and revolutionary vitreous cutters with two cutting blades making 27G vitrectomy as fast as 25G. This new equipment has expanded the indications for 27G surgeries immensely from detachment surgery to retinopathy of prematurity and the advent of new instruments may make 27G the gold standard in the future. This book, written by authors with huge experience in 27G vitrectomy, will enable surgeons to fully exploit its advantages.

Abstract
This is the second edition of a well-received book that reflects the state of the art in diagnosis and treatment of acute abdominal disorders in the pregnant patient. It addresses a wide range of conditions, whether associated with or incidental to pregnancy, ranging from very rare to more common ones, such as acute appendicitis and acute cholecystitis. It offers an update on recommendations, guidelines and scenarios to provide fundamental support for all clinicians who might encounter such cases. The book highlights the importance of a rapid diagnosis to avoid serious consequences for both the mother and the fetus. Furthermore, it sheds light on the different imaging modalities of rare pathologies that can occur during pregnancy, helping clinicians and radiologists to better define underlying cases. This new edition has been almost completely rewritten, and includes an additional section focusing on urologic emergencies, preterm labor and intra-abdominal pressure, as well as new figures and tables. It is equally valuable for general and abdominal surgeons, gynecologists and obstetricians, as well as emergency physicians, who may be the first specialists to have clinical contact with this group of patients. From the reviews of the first edition: "This book contains most of the information needed to address the problem of acute abdomen in pregnancy and brings state of the art principles and practice of emergency, abdominal surgery, obstetrics, gynecology and maternal-fetal medicine. The book should prompt clinicians, primarily surgeons and obstetricians to make faster and more precise diagnosis, earlier surgical interventions with the optimal outcome for the mother and child, by a multidisciplinary approach." (Translation into English; Original review: Prof. Dr. Tomislav Luetiç, Lijec Vjesn, Vol. 137 (5-6), May-June, 2015) "This book provides a detailed description of the various clinical scenarios in which we encounter acute abdominal conditions in pregnant patients. ÇŒ This is an excellent resource for practicing acute care surgeons and obstetricians/gynecologists who are most likely to encounter these patients. It also is a very useful resource for general practitioners, since they frequently are the first healthcare professionals who encounter pregnant patients with these conditions." (Prashant Khullar, Doody's Book Reviews, February, 2015).

Abstract
This textbook reviews the novel techniques employed in corneal transplantation. It will assist fellows and corneal surgeons in using these techniques to best effect and in selecting patients for surgical procedures, taking into account the benefits and risks. Corneal transplantation has been performed for more than 100 years. Until 15 years ago the state-of-the art type of transplantation was penetrating keratoplasty. Since the start of this millennium, however, important advances have been made in developing new surgical techniques. Today, the vast majority of keratoplasty procedures are performed as delicate lamellar procedures, either with the assistance of fine microkeratomes or femtosecond lasers or using very advanced surgical dissection procedures. Corneal Transplantation provides detailed information on these and other advances, which have helped patients undergoing keratoplasty to achieve a much faster visual recovery and a more stable eye with less risk of rejection episodes.

Abstract
Manfred Georg Krukemeyer first describes magnetic drug targeting, by which is meant the focused application of drugs, e.g. the introduction of a cytostatic, and which is aimed at optimising its locally therapeutic effect. The author thus achieves a high concentration of cytostatic at the site of the tumour. The particles have to be large enough to be attracted by a magnetic field and so find their way into the tumour, where they release their active ingredient. The aim of the hypothesis formed by one of the author's study groups was to show in a scientific experiment that in this case cytostatics coupled with iron particles [Fe3O4] can be introduced into a tumour. The focused introduction of drugs—magnetic drug targeting—results in faster arrival of the active ingredient at the target site together with a reduced quantity of active substances. For this reason magnetic drug targeting is also interesting from an economic point of view. Contents: Sarcomas and their Pathology; Magnetic Drug Targeting; Animal Testing and Patient Examination; Tumour Therapy in Humans; Target groups; Researchers, teachers and students in surgical and oncology departments Doctors working in the fields of surgery and oncology. About the author: Dr. Manfred Georg Krukemeyer is a specialist surgeon currently holding a senior position at a large German hospital enterprise.

Abstract
For the first time in a thousand years, Americans are experiencing a reversal in lifespan. Despite living in one of the safest and most secure eras in human history, one in five adults suffers from anxiety as does one-third of adolescents. Nearly half of the US population is overweight or obese and one-third of Americans suffer from chronic pain – the highest level in the world. In the United States, fatalities due to prescription pain medications now surpass those of heroin and cocaine combined, and each year 10% of all students on American college campuses contemplate suicide. With the proliferation of social media and the algorithms for social sharing that prey upon our emotional brains, inaccurate or misleading health articles and videos now move faster through social media networks than do reputable ones. This book is about modern health – or lack of it. The authors make two key arguments: that our deteriorating wellness is rapidly becoming a health emergency, and two, that much of these trends are rooted in the way our highly evolved hardwired brains and bodies deal with modern social change. The co-authors: a PhD from the world of social science and an MD from the world of medicine – combine forces to bring this emerging human crisis to light. Densely packed with fascinating facts and little-told stories, the authors weave together real-life cases that describe how our ancient evolutionary drives are propelling us toward ill health and disease. Over the course of seven chapters, the authors unlock the mysteries of our top health vices: why hospitals are more dangerous than warzones, our addiction to sugar, salt, and stress, our emotionally-driven brains, our relentless pursuit of happiness, our sleepless society, our understanding of risk, and finally, how world history can be a valuable tutor. Through these varied themes, the authors illustrate how our social lives are more of a determinant of health outcome than at any other time in our history, and to truly understand our plight, we need to recognize when our decisions and behavior are being directed by our survival-seeking hardwired brains and bodies.

Abstract
This book discusses globalization and its impact on human health. The population of the world grew from 1 billion in 1800 to 7 billion in 2012, and over the past 50 years the mean temperature has risen faster than ever before. Both factors continue to rise, as well as health inequalities. Our environment is changing rapidly, with tremendous consequences for our health. These changes produce complex and constantly varying interactions between the biosphere, economy, climate and human health, forcing us to approach future global health trends from a new perspective. Preventive actions to improve health, especially in low-income countries, are essential if our future is going to be a sustainable one. After a period of undeniable improvement in the health of the world's population, this improvement is likely to slow down and we will experience--at least locally--crises of the same magnitude as have been observed in financial markets since 2009. There is instability in health systems, which will worsen if preventive and buffering mechanisms do not take on a central role. We cannot exclude the possibility that the allied forces of poverty, social inequalities, climate change, industrial food and lack of governance will lead to a deterioration in the health of large sectors of the population. In low-income countries, while many of the traditional causes of death (infectious diseases) are still highly prevalent, other threats typical of affluent societies (obesity, diabetes, cardiovascular diseases) are increasing. Africa is not only affected by malaria, TB and HIV, but also by skyrocketing rates of cancer. The book argues that the current situation requires effective and coordinated multinational interventions guided by the principle of health as a common good. An entirely competition-driven economy cannot--by its very nature--address global challenges that require full international cooperation. A communal global leadership is called for. Paolo Vineis is Chair of Environmental Epidemiology at Imperial College. His current research activities focus on examining biomarkers of disease risk as well as studying the effects of climate change on non-communicable diseases. "From morality to molecules, environment to equity, climate change to cancer, and politics to pathology, this is a wonderful tour of global health: consistently presented in a clear, readable format. Really, an important contribution." Professor Sir Michael Marmot Director, Institute of Health Equity University College London Author of "The Health Gap". "This book is a salutary and soundly argued reminder that the "common good" is not simply what remains after individuals and groups have appropriated the majority of societal resources: it is in fact the foundation on which any society rests and without which it collapses." Rodolfo Saracci Senior Visiting Scientist International Agency for Research on Cancer, Lyon, France.

Abstract
"Integrated Care in Psychiatry: Redefining the Role of Mental Health Professionals in the Medical Setting" is a landmark title in the field, offering a clear, detailed, and cohesive call by leading experts for coordinated care for patients with concurrent psychiatric and medical conditions. The renowned editors and authors argue that what is slowly occurring, and greatly needed at a faster pace, is nothing less than a sea change in the way that psychiatric care will be delivered. The current, mostly segregated, medical and psychiatric model of care has led to the development of competing medical and psychiatric subcultures that have resulted in a lack of dialog among health providers, administrators, and payers -- and thus in less than optimal patient outcomes. To remedy this problem, the book offers a practical, insightful road map to achieving the central tenet of health reform truly coordinated, patient-centered care where the care experience for the patient, the medical care itself, and the cost outcomes improve as the system changes from fee-for-service to population-based health. An invaluable reference for all clinicians, policy makers, payers, administrators, and others interested in the debate surrounding healthcare systems, Integrated Care in Psychiatry: Redefining the Role of Mental Health Professionals in the Medical Setting is a major contribution to the literature and a gold standard resource"--Provided by publisher.

Abstract
This book, now in an extensively updated and expanded third edition, offers important insights into polymyalgia rheumatica (PMR) and giant cell arteritis (GCA), providing an overview of their pathophysiology, genetics, immunology, and endocrinological mechanisms but above all presenting the state of the art in diagnosis and treatment. Both PMR and GCA are diseases of advanced age. Early diagnosis and treatment can save one of the most precious senses vision. In this book, the role of imaging techniques is thoroughly examined, with particular attention to ultrasonography and FDG PET. All aspects of management of both diseases are discussed, including the current use of glucocorticoids, the efficacy of modern biological treatment, such as infliximab, tocilizumab, and rituximab, and the available monitoring tools. Diagnostic criteria for PMR and GCA are outlined, with discussion of differential diagnosis. Among the other topics addressed are disease classification, the histopathology of GCA, the ACR criteria for temporal arteritis, the value of temporal artery biopsy in GCA, serum cytokine profiles, chronobiology, and sine syndromes. The reader will find that the book aids in achieving early diagnosis and providing faster and more effective disease management. General readers with an interest in rheumatology, internal medicine, oftalmology and gerontology will find this book particularly valuable.

Abstract
The approach to total hip and total knee arthroplasty is changing around the world. There is now a strong emphasis on a more rapid recovery and on more outpatient surgery. Each step of the process is now being modified with more thorough pre-operative evaluations, streamlined surgical approaches, rapid mobilization and faster return to normal activities. These changes place new pressures on all of the health care providers in the system. This unique text--the first of its kind--is a practical guide for each member of the clinical team with updates by the leading authorities from around the country. Each chapter addresses a specific issue and will outline the important new items that need to be addressed, very similar to a handbook. Topics covered include the implementation of new payment models and outcome measurements, recommendations to streamline the pre-, peri- and post-operative protocols, the utility and application of outpatient joint replacement programs, multimodal pain management and post-acute rehabilitation strategies. There is no publication at present that addresses all of these items together in one convenient place. Rapid Recovery in Total Joint Arthroplasty is written and edited by experienced clinicians and surgeons, sharing their years of experience to create a practical, up-to-date text useful for everyday work.
