As a contribution to the search for suitable and sustainable solutions to finance rising medical care expenditures, the book proposes a typology of healthcare financing and insurance schemes, based on the dimensions of basic vs. supplementary services and mandatory vs. voluntary coverage, to analyse the design and the complex interactions between various financing and insurance arrangements in several OECD countries. This study provides a better understanding of the strengths and weaknesses of the financial and organisational structures of different countries' healthcare financing and insurance schemes. Its main contributions are the development of a novel and rigorous theoretical framework analysing the economic rationales for the optimal design of healthcare financing and insurance schemes, and an empirical and institutional analysis investigating the consequences for efficiency and affordability of the complex interactions between basic and supplementary sources of financing. The distinction between basic and supplementary health care insurance, and how they are regulated, is of fundamental importance to health care financing. This book makes a welcome contribution to the literature on this subject, providing insights into how the problem is addressed in a number of countries. Forced migration has yet to be sufficiently addressed from the perspective of health policy and systems research, resulting in limited knowledge on system-level interventions and policies to improve the health of forced migrants. The contributions within this edited volume seek to rectify this gap in the literature by compiling the existing knowledge on health systems and health policy responses to forced migration with a focus on asylum seekers, refugees, and internally displaced people. It also brings together the work of research communities from the fields of political science, epidemiology, health sciences, economics, psychology, and sociology to push the knowledge frontier of health research in the area of forced migration towards health policy and systems-level interventions, while also framing potential routes for further research in this area. Health Policy and Systems Responses to Forced Migration offers unique and interdisciplinary theoretical, empirical, and literature-based perspectives that apply a health policy and systems approach to health and healthcare challenges among forced migrants. Focusing on the health care systems of individual Canadian provinces and territories, our new series, Health System Profiles, examines the social, political, economic, and epidemiological context of health care policy in each Canadian province. Geriatric urology represents one of the largest overall sectors of care provided within the specialty of urology. As the incidence and prevalence of many clinical urologic conditions increase significantly in older adults, the practice of urology involves a need for advanced knowledge of the basic principles of geriatrics. Geriatric Urology outlines important topics in the care of elderly urology patients. The first section covers the biology of aging, changes in the genitourinary system, and common diseases including urologic conditions that may serve as warning signs of other disorders. Section two covers geriatric syndromes and implications for urologic care including frailty, polypharmacy, dementia, , and wound healing. Urologic conditions in older adults are explored in detail including urinary incontinence, pelvic organ prolapse, urinary tract infection, nocturia, and benign prostate diseases. Other topics covered include obtaining informed consent, development of advance directives, palliative medicine and care of the dying patient.

Healthcare Changes and the Affordable Care Act: A Physician Call to Action provides guidance, examples, and information on processes and timelines for physicians based on the implementation of The Affordable Care Act (ACA) that was established in 2010. This volume focuses on how geriatricians and other healthcare professionals can be engaged in responding to the roll-out of the ACA in their communities, and through this engagement assume leadership roles in local hospitals, healthcare organizations, and medical societies to advance quality improvement and new models of care for older adults. In-depth chapters provide an update on quality improvement efforts at the state level as well as changes in Medicaid financing and the significant impact this will have for older adults, particularly dual-eligibles. Many elements of the ACA are yet to be rolled out and many healthcare decisions are yet to be made. This first-of-its-kind volume addresses the myriad of issues relating to--and reviews the plethora of responses to--premature births in the United States, both in national context and compared with other countries. In addition to current clinical data, it examines how preterm births in the U.S. fit in with larger social concerns regarding poverty, racial disparities, reproductive rights, gender expectations, and the business of health care. Comparisons with preterm birth phenomena in Canada, the U.K., and other Western European countries illustrate cultural narratives about motherhood, women's status, differences across social welfare and abortion policies, and across health care financing and delivery sytems, and how these may affect outcomes for newborns. The book sorts out these intersecting complexities through the following critical lenses: Clinical: causes, treatments, and outcomes of preterm birth Population: the distribution of preterm births Cultural: how we understand preterm birth Health care: delivering care for high-risk pregnant women and preterm infants Ethical: moral decision-making about preterm births Preterm Birth in the United States synthesizes a wide knowledge base for maternal and child health professionals across diverse disciplines, including public health, social work, nursing, medicine, and health policy.
