Abstract
Over the course of the last decade, political and mental entities at large have embraced global mental health: the idea that psychiatric health is vital to improved quality of life.  Physicians globally have implemented guidelines recommended by the National Institute of Mental Health (NIMH) in 2007, thereby breaking down barriers to care and improving quality of life in areas where these practices have been implemented.  Programs for training and education have expanded as a result.  Clinicians benefit more from both local resources in some regions as well as in international collaboration and technological advancements.  Even amidst all of these positive outcomes, clinicians still face some stumbling blocks.  With worldwide statistics estimating that 450 million people struggle with mental, neuropsychiatric, and neurological disorders—25 percent of the world’s non-communicable disease burden—rising to these challenges prove to be no small feat, even in wealthy Western nations.   Various articles and books have been published on global mental health, but few of them thoroughly cover the clinical, research, innovative, and social implications as they pertain to psychiatry; often, only one of these aspects is covered.  A comprehensive text that can keep pace with the rapidly evolving literature grows more and more valuable each day as clinicians struggle to piece together the changes around the world that leave open the possibility for improved outcomes in care. This textbook addresses the themes that are at the forefront of neonatal clinical care and research, based on natural divisions in care during pregnancy, and postnatally by gestational age at birth. The book offers a unique approach, in that it proposes discussion of important general principles underpinning neonatal care that are not addressed in most general neonatology textbooks, such as ethical issues, counselling, effective training methods, quality and safety, among other subjects. These are fundamental aspects and challenges that need to be appreciated by senior clinicians. A chapter authored by parents describing their perspectives of neonatal intensive care is unique and will be highly educational, with the potential to influence the way in which individuals view and deliver neonatal care. The authors discuss common and important conditions, to promote adoption of sound evidenced based practice where this is available. However, where evidence is limited, as is the case in many areas of neonatal practice, the authors aim to encourage critical thinking and evidence appraisal, which are necessary skills for busy clinicians wishing to filter evidence to guide delivery of care. Administrators and faculty in medical, nursing and health science programs are witnessing a substantial increase in the number of students with disabilities entering their programs. Concurrently, the benefits of diversity in healthcare are becoming increasingly apparent and important. Provider-patient concordance is a known mechanism for reducing health care disparities. By developing a workforce that mirrors the patient population, we can appropriately inform disability care, reducing health care disparities while embracing the tenets of the Americans With Disabilities Act (ADA), namely equal opportunity, full participation, independent living, and economic self-sufficiency for qualified individuals with disabilities. One in five individuals will experience disability at some point in their lives, making this the largest minority in the US. A commitment to disability inclusion for qualified students should be a high-level goal of nursing, medical, and other health science programs. To support this goal, leaders in these areas must develop robust programs and an understanding of the best practices for inclusion. This first-of-its-kind title is designed to help deans, program directors, faculty, student affairs personnel and disability resource professionals thoughtfully plan for the growing population of health-care professionals with disabilities. The content helps stakeholders contextualize disability inclusion in health-care education as a function of social justice and a mechanism of reducing health care disparities for patients. It offers pragmatic advice, grounded in research, best practice, and case law to address the highly nuanced approach to determining and implementing accommodations in a high-stakes clinical environment. This book captures cornerstone developments in a new body of knowledge and provides an expert resource on a “hot topic” in rectal surgery. Transanal minimally invasive surgery (TAMIS) was designed for local excision of select rectal neoplasms, however soon it became realized that the TAMIS technique could be used for applications beyond local excision, most notably for transanal total mesorectal excision (taTME). This new operative technique has revolutionized our approach to the distal rectum by allowing for improved access, especially in obese male patients with an android pelvis, and by minimizing abdominal wall access trauma. The endpoints of improved oncologic resection, as defined by mesorectal envelope completeness, negative circumferential resection margins, and negative distal margin, are assessed. This book details controversies, pitfalls, and future directions of taTME and TAMIS. Chapters are authored by those on the forefront of innovation with TAMIS and taTME, and each is considered an authority on the topic. Antibody-drug conjugates (ADCs) stand at the verge of a transformation. Scores of clinical programs have yielded only a few regulatory approvals, but a wave of technological innovation now empowers us to overcome past technical challenges. This volume focuses on the next generation of ADCs and the innovations that will enable them. The book inspires the future by integrating the field's history with novel strategies and cutting-edge technologies. While the book primarily addresses ADCs for solid tumors, the last chapter explores the emerging interest in using ADCs to treat other diseases. The therapeutic rationale of ADCs is strong: to direct small molecules to the desired site of action (and away from normal tissues) by conjugation to antibodies or other targeting moieties. However, the combination of small and large molecules imposes deep complexity to lead optimization, pharmacokinetics, toxicology, analytics and manufacturing. The field has made significant advances in all of these areas by improving target selection, ADC design, manufacturing methods and clinical strategies. The relatively recent concepts of structural and functional connectivity make it even more important to visualize the brain as a whole rather than looking at its individual parts. Abstract
This book is designed to re-establish the position of the oral cavity and its mucosa at the forefront of defence and maintenance of homeostatic mechanisms that protect against disease not just locally but also systemically. The oral mucosa is a unique collection of tissues that constitutes a highly active environment with its own unique microflora and homeostatic interaction with the innate and adaptive immune responses. As an immune tissue the oral mucosa was somewhat neglected in the past owing to the tendency to assume similarity to the gut mucosa. More recently it has become apparent that the oral mucosa is a complex environment and, like the esophageal mucosa, has more in common with vaginal tissue than with the gut. Furthermore, the ability of the oral mucosa to act as an immune inductive site has made it an attractive area of research in terms of desensitization for allergic reactions and possibly autoimmune responses. In this book, recognized experts in the field provide up-to-date coverage of all aspects of the structure and function of the oral mucosa, reflecting important recent advances in knowledge, including at the molecular level.

Abstract
This book critically examines the many complex entanglements between AIDS activism and HIV science. It takes readers on a medical anthropological expedition across time and space that highlights the stakes from the perspective of those most affected by the epidemic. Author Robert Lorway reveals how early in the HIV epidemic, amid inadequate government leadership, communities of people living with and directly affected by HIV and AIDS rose to become a vital force at the forefront of prevention responses. Yet now, more than three decades later, HIV prevention and treatment is increasingly being placed under the jurisdiction of clinical, epidemiological, and management scientific expertise. In this kind of context, where does activism figure into the possibility of more democratized collaborations between affected communities, scientists, and policy makers? Coverage draws upon the findings from an array of community research projects conducted in Canada, India, and Kenya over a 22-year period. It weaves together rich, original data sources that range from in-depth qualitative interviews, field notes, and primary and secondary archival document retrievals in these three regions. Offering a rich diversity in perspectives, this book tackles the broader themes related to global health policy, science, and transnational activism at the same time as it highlights the experiences and local arenas where debates about activism and science play out. In the end, Lorway questions the growing expectation for affected communities themselves to produce sound evidence to legitimize their advocacy projects. He calls for the planners and implementers of biomedically oriented HIV research and interventions to more meaningfully engage with communities in ways that de-monopolize decision making as a matter of ethics and improved scientific practice.
