Abstract
The field of structural heart disease interventions is experiencing a stage of rapid growth with the development of percutaneous mitral valve repair therapies and percutaneous aortic valve replacement therapies. The MitraClip represents a new therapeutic frontier for mitral regurgitation and is expected to revolutionize repair of the mitral valve. For the first time in the world, physicians now have the capability to repair the mitral valve percutaneously without the need for a sternotomy or cardiopulmonary bypass. However, as with all new endeavors, there is a steep learning curve, not just for the individual operator but also for the entire field. The MitraClip technology and technique is technically complex and is guided by echocardiography and fluoroscopy. It is an imaging intensive procedure that requires deep 3-dimensional understanding of normal and abnormal cardiac anatomy. Not all mitral regurgitation cases are suitable and not all cases are straightforward. It is therefore crucially important for physicians in this specialty to communicate their experience, successes, failures and complications with each other. Written by the leaders in this breakthrough field, the Atlas of Percutaneous Edge-to-Edge Mitral Valve Repair is the first of its kind for the new percutaneous MitraClip procedure. The Editors have designed the book to be a comprehensive reference manual and to provide cardiologists with true examples of basic and complex cases of percutaneous mitral valve repair using the technology. In addition, this book provides step-by-step guides to the procedure and complications as well as steps to recover from them are discussed. The Atlas is anticipated to be a very valuable reference for both novice and experienced interventional cardiologists and echocardiologists. Abstract
This atlas is a concise but comprehensive guide to the diverse patterns of response to immunotherapy as observed on Positron Emission Tomography/Computed Tomography (PET/CT) and other conventional imaging modalities, including CT and Magnetic Resonance Imaging (MRI). The purpose for this publication is to fill the gap between the growing clinical relevance and utilization of immunotherapy in medical oncology, mainly based on checkpoint inhibitors, and the need for experienced imagers with reliable tools assessing response to treatment. A series of disease-oriented chapters will present the imaging findings during immunotherapy in the major oncological settings, with helpful comparison of functional (PET/CT) and morphological (CT/MRI) patterns of response in individual cases. To complete the atlas, a dedicated chapter will focus on major pitfalls and immune-related adverse events (irAEs) affecting image interpretation during the course of immunotherapy. The concluding chapter will lastly examine the available data and potential developments of immuno-PET, which is considered as the novel frontier of research in this oncological scenario. The atlas will be of high value for radiologists and nuclear medicine specialists at all levels of experience.

Abstract
Biobank research and genomic information are changing the way we look at health and medicine. Genomics challenges our values and has always been controversial and difficult to regulate. In the future lies the promise of tailored medical treatments and pharmacogenomics but the borders between medical research and clinical practice are becoming blurred. We see sequencing platforms for research that can have diagnostic value for patients. Clinical applications and research have been kept separate, but the blurring lines challenges existing regulations and ethical frameworks.Then how do we regulate it? This book contains an overview of the existing regulatory landscape for biobank research in the Western world and some critical chapters to show how regulations and ethical frameworks are developed and work. How should international sharing work? How design an ethical informed consent? An underlying critique: the regulatory systems are becoming increasingly complex and opaque. The international community is building systems that should respond to that. According to the authors in fact, it is time to turn the ship around. Biobank researchers have a moral responsibility to look at and assess their work in relation to the bigger picture: the shared norms and values of current society. Research ethics shouldn't only be a matter of bioethicists writing guidelines that professionals have to follow. Ethics should be practiced through discourse and regulatory frameworks need to be part of that public discourse. Ethics review should be then not merely application of bureaucracy and a burden for researchers but an arena where researchers discuss their projects, receive advice and practice their ethics skills.

Abstract
How long can humans live? This open access book documents, verifies and brings to life the advance of the frontier of human survival. It carefully validates data on supercentenarians, aged 110+, and semi-supercentenarians, aged 105-109, stored in the International Database on Longevity (IDL). The chapters in this book contribute substantial advances in rigorously checked facts about exceptional lifespans and in the application of state-of-the-art analytical strategies to understand trends and patterns in these rare lifespans. The book includes detailed accounts of extreme long-livers and how their long lifespans were documented, as well as reports on the causes of death at the oldest ages. Its key finding, based on the analysis of 1,219 validated supercentenarians, is that the annual probability of death is constant at 50% after age 110. In contrast to previous assertions about a ceiling on the human lifespan, evidence presented in this book suggests that lifespan records in specific countries and globally will be broken again and again as more people survive to become supercentenarians.

This book provides a historical analysis of the Frontier Nursing Services in the Eastern Appalachians of the United States, as well as a review of the oral history tradition of former frontier and non-frontier nurses. The data was gathered from 2003 to 2007, and the historical part covers the years 1900 to 1970. The objective of the study presented here was to conduct interviews with former frontier and non-frontier nurses in order to better understand their family and personal relationships, and the experiences that motivated their career choices. These interviews also give a voice to the working and middle-class women of the FNS. The emerging themes include moral inhabitability in work/education environments, the generational mix, nurse-physician and male-female relationships at the workplace, the role of technology, humanitarian versus financial rewards, and the public image of nurses. In addition, the book examines how the FNS shifted from a community/grass-roots structure to the corporate/business model of healthcare delivery employed today. In closing, it stresses the importance of explorig past nursing in order to better grasp present nursing. It also represents a testament to the professional work and vital contributions of frontier nurses.

Abstract
Forced migration has yet to be sufficiently addressed from the perspective of health policy and systems research, resulting in limited knowledge on system-level interventions and policies to improve the health of forced migrants. The contributions within this edited volume seek to rectify this gap in the literature by compiling the existing knowledge on health systems and health policy responses to forced migration with a focus on asylum seekers, refugees, and internally displaced people. It also brings together the work of research communities from the fields of political science, epidemiology, health sciences, economics, psychology, and sociology to push the knowledge frontier of health research in the area of forced migration towards health policy and systems-level interventions, while also framing potential routes for further research in this area. Health Policy and Systems Responses to Forced Migration offers unique and interdisciplinary theoretical, empirical, and literature-based perspectives that apply a health policy and systems approach to health and healthcare challenges among forced migrants. A timely work describing how localized hospital-based health technology assessment (HB-HTA) complements general, "arms-length" HTA agency efforts, and what has been the collective global impact of HB-HTA across the globe. While HB-HTA has gained significant momentum over the past few years, expertise in the field, and information on the operation and organization of HB-HTA, has been scattered. This book serves to bring this information together to inform those who are currently working in the field of HTA at the hospital, regional, national or global level. In addition, this book is intended for decision-makers and policy-makers with a stake in determining the uptake and decommissioning of new and established technologies in the hospital setting. HTA has traditionally been performed at the National/Regional level by HTA Agencies, typically linked to governments. Yet hospitals are the main entry door for most health technologies (HTs). Hospital decision-makers must undertake multiple high stakes investment and disinvestment decisions annually for innovative HTs, usually without adequate information. Despite the existence of arms-length HTA Agencies, inadequate information is available to hospital decision-makers either because relevant HTA reports are not yet released at the time of entry of new technologies to the field, or because even when the report exists, the information contained is insufficient to clarify the contextualized informational needs of hospital decision makers. Therefore, there has recently been a rising trend toward hospital-based HTA units and programs. These units/programs complement the work of National/Regional HTA Agencies by providing the key and relevant evidence needed by hospital decision makers in their specific hospital context, and within required decision-making timelines. The emergence of HB-HTA is creating a comprehensive HTA ecosystem across health care levels, which creates better bridges for knowledge translation through relevance and timeliness.
