The components of the system of involuntary mental hospitalization are addressed, as are different methods of treating mental illness. Dr. William E. Simon moderates the debate between Dr. Ellis and Dr. Szasz. They discuss among other subjects, racism, the women's movement, psychoanalysis, primal scream therapy, the Son of Sam, chemotherapy.

Cultural Competence in Health Care offers a wealth of this crucial knowledge with emphasis on day-to-day clinical work. - Since their first application in 1982, Totally Implantable Venous Access Devices (TIVADs) have become increasingly important in the clinical practice, as more intensive chemotherapy and parenteral treatments have come into use. At this time, there is objective evidence that TIVADs are a safe, effective strategy for long-term venous access; they play a significant role throughout the management of the oncology patient, as they are needed in the initial phases for active treatments as well as in the last stages for palliative measures, making possible repeated administration of chemotherapeutic vesicant agents, nutrients, antibiotics, analgesics, and blood products. According to a number of prospective studies, use of TIVADs is associated with a significant complication rate (10% to 25% of all patients). Evidence-based data support that most complications are directly related to inappropriate technique of placement and/or nursing care, sometimes leading to TIVAD loss, significant morbidity, increased duration of hospitalization, and additional medical cost. A group of world-renowned experts - both in the clinical and research fields contributed to this volume, whose aim is to provide clinicians, nurses and medical students with a multidisciplinary, full update on these devices, as long term central venous access can no be longer considered a routine matter, and serious complications can be maintained at a very low level only if strict adherence to a well-defined protocol of surgical technique and of catheter care is maintained.

- Since the first endovascular repair of abdominal aortic aneurysm was reported in the early 1990s, there has been an explosion in the volume and complexity of endovascular and hybrid procedures for the treatment of aortic diseases. Endovascular techniques and technologies have evolved from the initial devices that allowed treatment of only the most straightforward infrarenal aneurysms with appropriately long and straight necks to the scenario that pertains today, wherein a customized endovascular solution is almost always available for even the most complex aortic aneurysm. There is growing evidence in favor of the endovascular treatment of these aneurysms, which extend above the visceral aortic segment, as well as for the treatment of thoracic aortic dissection and aneurysm. These new endoaortic surgical procedures have been proven to shorten hospitalization, reduce morbidity and mortality, speed recovery, and hasten return to normal life, while their evolution and conceptual design would obviously not be possible without the simultaneous explosion in medical imaging technologies that has taken place over a similar time period. The Atlas of Advanced Endoaortic Surgery has been compiled to engage its readers in a comprehensive yet concise manner, with numerous illustrations of real clinical cases. - There have been news reports with the tales of many community acquired bacterial infections that lead to serious hospitalization. The last resort drug for these infections is vancomycin; an FDA approved glycopeptide antibiotic for the treatment of several bacterial infections, including infections caused by susceptible staphylococcus, streptococcus, enterococcus, and diphtheroid organisms. Around the world in 1997, initial reports of reduced vancomycin susceptibility in clinical isolates of staphylococcus aureus generated significant concern in the medical community. There has been uncertainty regarding optimal laboratory tests and the clinical relevance of reduced vancomycin susceptibility in S. aureus; changes in the Clinical and Laboratory Standards Institute (CLSI) breakpoints for vancomycin against S. aureus; and increasing concern regarding the efficacy of vancomycin for the treatment of infection due to gram-positive bacteria, such as S. aureous. Vancomycin can be administered either intravenously or orally, but in order to treat systemic infections, vancomycin must be administered intravenously. Identifying the changes in genome content or expression, which are associated with the acquisition of the different types of vancomycin resistance, is an important step in the process of understanding the molecular basis of resistance with the potential to provide leads both for new drug targets and diagnostic biomarkers. However, considering that vancomycin has a slow bactericidal activity, clinicians have to adequately prescribe vancomycin to avoid the acquisition of resistance under therapy and to facilitate the cure.

On the basis of clinical experience with more than 7,000 patients with various types of tumors treated over a period of nearly 20 years at the National Institute of Radiological Sciences, step-by-step procedures and technological development of this modality are highlighted. The book is divided into two sections, the first covering the underlying principles of physics and biology, and the second section is a systematic review by tumor site, concentrating on the role of therapeutic techniques and the pitfalls in treatment planning. Readers will learn of the superior outcomes obtained with carbon-ion therapy for various types of tumors in terms of local control and toxicities. It is essential to understand that the carbon-ion beam is like a two-edged sword: unless it is used properly, it can increase the risk of severe injury to critical organs. In early series of dose-escalation studies, some patients experienced serious adverse effects such as skin ulcers, pneumonitis, intestinal ulcers, and bone necrosis, for which salvage surgery or hospitalization was required. To preclude such detrimental results, the adequacy of therapeutic techniques and dose fractionations was carefully examined in each case. In this way, significant improvements in treatment results have been achieved and major toxicities are no longer observed. With that knowledge, experts in relevant fields expand upon techniques for treatment delivery at each anatomical site, covering indications and optimal treatment planning. With its practical focus, this book will benefit radiation oncologists, medical physicists, medical dosimetrists, radiation therapists, and senior nurses whose work involves radiation therapy, as well as medical oncologists and others who are interested in radiation therapy.
