Abstract
Cultural Competence in Health Care A Guide for Professionals Wen-Shing Tseng, University of Hawaii, Honolulu Jon Streltzer, University of Hawaii, Honolulu Between a growing pan-ethnic patient base, the need to solve inequalities of care, and the clinical desirability of viewing clients in their entirety, applied cultural knowledge is emerging as a major goal of clinical and public health practice. Cultural Competence in Health Care offers a wealth of this crucial knowledge with emphasis on day-to-day clinical work. Authors Tseng and Streltzer, well-known experts on transcultural medicine and therapy, start by differentiating between disease and illness and advise on a range of practical matters, from discussing culturally sensitive subjects to negotiating compromises between traditional healing and established medicine, so that readers will gain both useful working techniques and valuable empathy with clients. Case studies highlight medical issues as they apply to various minority (and majority) groups, and eight insightful chapters reveal cultural dimensions expressed in: Concepts of illness and illness behaviors. Assessment, interviews, and symptom presentation. Hospitalization, medication, family involvement, consent issues. Highly charged medical areas, e.g., pain management, transfusions, reproductive issues, organ donation. Frequently stigmatized conditions such as AIDS, STDs, cancer, and psychiatric disorders. Care by non-medical professionals, including social workers, physical therapists, and nutritionists. Cultural Competence in Health Care is equally suited to use by practitioners and students in medicine, psychology, nursing, social work, and affiliated fields. And since it's one of the only practice-oriented books on the topic, cross-cultural courses in training programs will find it especially useful.

Abstract
Since their first application in 1982, Totally Implantable Venous Access Devices (TIVADs) have become increasingly important in the clinical practice, as more intensive chemotherapy and parenteral treatments have come into use. At this time, there is objective evidence that TIVADs are a safe, effective strategy for long-term venous access; they play a significant role throughout the management of the oncology patient, as they are needed in the initial phases for active treatments as well as in the last stages for palliative measures, making possible repeated administration of chemotherapeutic vesicant agents, nutrients, antibiotics, analgesics, and blood products. According to a number of prospective studies, use of TIVADs is associated with a significant complication rate (10% to 25% of all patients). Evidence-based data support that most complications are directly related to inappropriate technique of placement and/or nursing care, sometimes leading to TIVAD loss, significant morbidity, increased duration of hospitalization, and additional medical cost. A group of world-renowned experts - both in the clinical and research fields contributed to this volume, whose aim is to provide clinicians, nurses and medical students with a multidisciplinary, full update on these devices, as long term central venous access can no be longer considered a routine matter, and serious complications can be maintained at a very low level only if strict adherence to a well-defined protocol of surgical technique and of catheter care is maintained.

Abstract
Since the first endovascular repair of abdominal aortic aneurysm was reported in the early 1990s, there has been an explosion in the volume and complexity of endovascular and hybrid procedures for the treatment of aortic diseases. Endovascular techniques and technologies have evolved from the initial devices that allowed treatment of only the most straightforward infrarenal aneurysms with appropriately long and straight necks to the scenario that pertains today, wherein a customized endovascular solution is almost always available for even the most complex aortic aneurysm. There is growing evidence in favor of the endovascular treatment of these aneurysms, which extend above the visceral aortic segment, as well as for the treatment of thoracic aortic dissection and aneurysm. These new endoaortic surgical procedures have been proven to shorten hospitalization, reduce morbidity and mortality, speed recovery, and hasten return to normal life, while their evolution and conceptual design would obviously not be possible without the simultaneous explosion in medical imaging technologies that has taken place over a similar time period. The Atlas of Advanced Endoaortic Surgery has been compiled to engage its readers in a comprehensive yet concise manner, with numerous illustrations of real clinical cases. It provides a useful tool for practitioners as they plan and execute treatment of patients with these various aortic pathologies, and serves as a useful reference as this segment of the field continues to evolve.

Abstract
There have been news reports with the tales of many community acquired bacterial infections that lead to serious hospitalization. The last resort drug for these infections is vancomycin; an FDA approved glycopeptide antibiotic for the treatment of several bacterial infections, including infections caused by susceptible staphylococcus, streptococcus, enterococcus, and diphtheroid organisms. Around the world in 1997, initial reports of reduced vancomycin susceptibility in clinical isolates of staphylococcus aureus generated significant concern in the medical community. There has been uncertainty regarding optimal laboratory tests and the clinical relevance of reduced vancomycin susceptibility in S. aureus; changes in the Clinical and Laboratory Standards Institute (CLSI) breakpoints for vancomycin against S. aureus; and increasing concern regarding the efficacy of vancomycin for the treatment of infection due to gram-positive bacteria, such as S. aureous. Vancomycin can be administered either intravenously or orally, but in order to treat systemic infections, vancomycin must be administered intravenously. Identifying the changes in genome content or expression, which are associated with the acquisition of the different types of vancomycin resistance, is an important step in the process of understanding the molecular basis of resistance with the potential to provide leads both for new drug targets and diagnostic biomarkers. However, considering that vancomycin has a slow bactericidal activity, clinicians have to adequately prescribe vancomycin to avoid the acquisition of resistance under therapy and to facilitate the cure.

Abstract
This book serves as a practical guide for the use of carbon ions in cancer radiotherapy. On the basis of clinical experience with more than 7,000 patients with various types of tumors treated over a period of nearly 20 years at the National Institute of Radiological Sciences, step-by-step procedures and technological development of this modality are highlighted. The book is divided into two sections, the first covering the underlying principles of physics and biology, and the second section is a systematic review by tumor site, concentrating on the role of therapeutic techniques and the pitfalls in treatment planning. Readers will learn of the superior outcomes obtained with carbon-ion therapy for various types of tumors in terms of local control and toxicities. It is essential to understand that the carbon-ion beam is like a two-edged sword: unless it is used properly, it can increase the risk of severe injury to critical organs. In early series of dose-escalation studies, some patients experienced serious adverse effects such as skin ulcers, pneumonitis, intestinal ulcers, and bone necrosis, for which salvage surgery or hospitalization was required. To preclude such detrimental results, the adequacy of therapeutic techniques and dose fractionations was carefully examined in each case. In this way, significant improvements in treatment results have been achieved and major toxicities are no longer observed. With that knowledge, experts in relevant fields expand upon techniques for treatment delivery at each anatomical site, covering indications and optimal treatment planning. With its practical focus, this book will benefit radiation oncologists, medical physicists, medical dosimetrists, radiation therapists, and senior nurses whose work involves radiation therapy, as well as medical oncologists and others who are interested in radiation therapy.

Abstract
Poor adherence to therapy is one of the main obstacles to treatment effectiveness in schizophrenia. It is the main determinant of relapse, hospitalization, symptom persistence, and poor psychosocial functioning and outcome. Adherence to treatment is affected by various factors related to the disease characteristics, to the patient him- or herself, to the treatment, and to the therapeutic relationship. Some of these factors are modifiable, and both pharmacological and non-pharmacological strategies have been developed for this purpose. This book addresses the different aspects of adherence to treatment in schizophrenia and related disorders in a systematic but easy-to-use manual format. Chapters focus on a full range of issues, including pharmacological and non-pharmacological strategies to enhance adherence and continuity of care, relevant psychological factors, the importance of the patient-doctor relationship, and the need for an alliance with other care-givers.

Abstract
This title deals with internationally documented variations in medical practice and health service that exist across countries as well as regions across a specific country. Such variations raise critical concerns about the quality, equity and efficiency of health care resources across the world. Health services researchers have long been aware of large variations in the use of medical care across regions and medical providers. In the 1930s, the British pediatrician J.A. Glover observed that the rates of tonsillectomy in British schoolchildren varied widely, depending on the district where the students lived and the doctors who examined them. This volume provides a contextual landscape for the study of health care utilization through the lens of medical practice variations. It is grounded in the pioneering work by medical care epidemiologist, Dr. John Wennberg, who revealed wide variations in elective surgical rates across small areas in the U.S. and his findings that these variations were generally not explained by differences in population illness rates or patient preferences but rather, there were strong associations between supply of health care resources, such as hospital beds and physicians and health care utilization. This volume introduces the concept of medical practice variations and its early history, outlines established concepts and frameworks, with an overview of methods used to understand the variations in medical care. It makes the case for outcomes research in determining what works in health care and policy reforms to rationalize how care is delivered. Each chapter synthesizes the current published literature in the field and covers a description of medical practice variations in the area, determinants of these variations and outcomes. It outlines the most current research on specific types of utilization such as inpatient care, emergency services, elective surgery, primary care, obstetric and gynaec ological care, mental health care and end-of-life care, among others. Studies of variation in condition-specific care focus on common conditions such as acute myocardial infarction, congestive heart failure, stroke, diabetes and procedures such as cancer surgery and joint replacement. Special topics include health care spending and quality, shared decision making and disparities.

Abstract
This book is designed to present the clinical geriatric trends within general internal medicine and family practice, which practitioners often encounter in caring for their older adult patients. Chapters focus on increasingly difficult clinical decisions that practitioners have to make in caring for older adults, who often experience medical complications due to memory loss, physical disability, and multiple chronic conditions. Written by experts in geriatric medicine, each of these chapters start with the most up-to-date clinical geriatric research and provide specific examples or case studies on how to use this information to address the clinical needs of older adult patients. In addition, there is a set of concise "take-home points" for each chapter that are easy to commit to memory and implement in clinical care of aging patients. As the only book to focus on current trends in geriatric research and evidence-based eldercare practice, Clinical Trends in Geriatric Medicine is of great value to internists, family practitioners, geriatricians, nurses, and physician assistants who care for older adults.
Contents

Abstract
This book elucidates the etiology of heart failure (HF) to assist understanding in the clinical therapies available. It explains these in light of the better understanding of the pathophysiology of HF, which has translated into dramatic improvements in patient outcome. The chapters detail the causes, diagnosis and treatment of HF, and recognize that having become the leading cause of morbidity, mortality and hospitalization in the developed world, even patients with the most advanced HF have options that allow prolonged survival. This includes alternatives to cardiac transplantation, including mechanical circulatory support with a rapidly growing number of new, smaller, more reliable devices becoming available. Heart Failure: A Comprehensive Guide to Pathophysiology and Clinical Care will be critical reading for all involved in the management of these patients and provides a scientific and clinical reference for trainees in cardiovascular disease as well as physicians involved in caring for these patients.
