Cette brochure explique ce que sont les incidents liés à la sécurité des patients, ce qu'il faut faire si vous constatez de tels incidents, comment les signaler, ce qu'est la divulgation, de quelle façon elle peut vous aider et comment elle a lieu. Une liste de ressources pour de plus amples renseignements est incluse.
This is a French translation of the English pamphlet 1448, "Patients First: Disclosure of Patient Safety Incidents". People expect safe health care. Health care providers work hard every day to give safe, excellent care. Despite our best efforts to keep you safe, there are risks to being in the hospital. For example, a patient may fall, or get an infection or a pressure injury (bedsore), or be given the wrong dose of medication. These events are called patient safety incidents. This pamphlet explains what patient safety incidents are, what to do if you notice a patient safety incident, how patient safety incidents are reported, what disclosure is and how it will help you, and how disclosure takes place. A list of resources for further information is included.

If you use home parenteral nutrition (HPN), it is very important to be prepared for an emergency (like a weather-related emergency or a natural disaster). It is important to plan before an emergency to make sure you will be able to keep doing your therapy without any interruptions. This pamphlet explains what an HPN emergency plan is, what emergency supplies you should have ready, HPN during a power outage, how to clean your hands with sanitizer, how to run your HPN when your pump battery has run out (the gravity method), and HPN during an evacuation. The pamphlet also includes a Health Emergency Card, where you can write down your medical information, emergency contacts, type of vascular access and location, and your HPN order. Keep a copy of your Emergency Health Card in your HPN emergency pack at all times. Space to write down your individual flow rates for pump tubing via gravity is also included.

This pamphlet is a French translation of the English pamphlet 1942, "Advance Care Planning: Making Your Personal and Medical Wishes Known". Advance care planning, or personal care planning, is the process of reflecting on what is important to you, to help you plan for your future medical and personal care. This pamphlet gives information about advance care planning, personal directives (PDs), delegates, what will happen if you do not have a PD, where to keep your completed PD and who should have a copy of it, when you should update your PD, and some treatments you should consider when making a PD. Information about Medical Assistance in Dying (MAID) is included. A list of resources, as well as information and tools available to help you, are provided. A blank Nova Scotia Health Authority Personal Directive form is included.
