But as parents and researchers have long known, there is a crucial dichotomy between adolescents' cognitive competence and their frequent inability to utilize that competence in everyday decision-making. This first-of-its-kind textbook marks a revolutionary effort to reform medical education nationally by providing a comprehensive, high-quality resource to serve as a foundation for lesbian, gay, bisexual, transgender, and queer (LGBTQ) health education across multiple disciplines. Addressing the decades-long unequal weight of medical education generally offered about the care of LGBTQ people, The Equal Curriculum was created to advance clinicians' competencies in optimizing the health of LGBTQ people. This textbook is designed to be integrated into health sciences curricula and offers pointed strategies to evaluate the integration of LGBTQ health topics. Starting with a brief overview, chapters 1 through 4 cover general content that is highly relevant to all health professionals working with LGBTQ people. Chapters 5 through 12 focus on specific patient populations and clinical specialties, and chapters 13 and 14 cover special topics. Key points in each chapter are highlighted to aid in the comprehension, and case vignettes are provided throughout the textbook, allowing learners to apply the content to clinical scenarios in order to evaluate how the application of relevant knowledge may impact health outcomes. Questions similar to National Board of Medical Examiners (NBME) style are provided in most chapters to assist in the application of content. The prevalence of disorders related to insulin resistance has continued to increase throughout the developed and developing world. The world's population has continued to become more obese and sedentary. Of great concern in the last decade is the extension of these deleterious lifestyle patterns to the pediatric population, leading to both obesity and the appearance of insulin resistance-related disorders in youth as well as adults. Insulin Resistance: Childhood Precursors and Adult Disease presents topics related to insulin resistance and its consequences across the lifespan. In the first section of the book examining epidemiology, the contributors review controversies over the definition of metabolic syndrome in adults and children, current knowledge regarding the epidemiology of insulin resistance in the pediatric population and the contributions of the prenatal and early childhood environment to the development of insulin resistance. The Second part of the book explores pathophysiology, with reviews of the techniques used to study insulin resistance, as well as current knowledge of the molecular and physiologic mechanisms of insulin resistance, including the contributions of adipose tissue and biochemical mediators. This section concludes with discussion of the relationship between insulin resistance and cardiovascular, endothelial, liver and gonadal disorders. The final section of the book explores the impact of exercise and weight loss medications on insulin resistance. Insulin resistance is likely to be the most important public health concern in many parts of the world in coming decades. This volume provides up-to-date reviews of these areas, providing the reader with a current perspective on issues in insulin resistance as they affect patients across the lifespan. This section concludes with discussion of the relationship between insulin resistance and cardiovascular disease and liver disorders. A third section, new to this second edition, explores insulin resistance in unique models: intrauterine growth restriction and girls with polycystic ovary syndrome and metabolic syndrome. The final section of the book explores the concepts of treatment through medications directed at insulin sensitivity, as well as exercise, weight loss medications and consequences of bariatric surgery. With the increasing proportion of persons above 65 years old and over with a 50% overall risk of developing cancer during our lifetimes, cancer rates continue to play a role in the utilization of healthcare services—particularly in the ICU. Currently, very few oncologic critical care texts, journals, subspecialty societies in the field, or knowledge about the current availability of oncologic/hematologic ICUs exist. As such, major knowledge gaps exist about outcomes (e.g., ICU utilization, mortality, costs); healthcare disparities (e.g., racial, geographic); and almost all aspects of intensive care delivery to the critically ill cancer patient. With the aging of the population and projected shortage of critical care providers over the next 25 years, identifying the outcomes of the available oncologic critical care resources is imperative. To address such disparities, this text seeks to close significant knowledge and educational gaps within the realms of medical care for critically ill cancer patients by serving as one of the first major reference works on oncologic critical care. Main objectives encompass closing existing critical care knowledge gaps to serve as a vehicle of education for current and successive generations of intensivists, advance practice providers, nurses, and other healthcare providers.
