Abstract

Drawing on 20 years of experience, the authors elucidate the clinical potential of the approach while also highlighting current pitfalls and limitations. The opening two chapters discuss the 3D imaging workflow and its integration into daily clinical routine and comprehensively describe cone-beam CT virtual diagnosis. The stepwise 3D virtual planning of orthognathic surgery and transfer of the 3D virtual treatment plan to the patient in the operating room are then thoroughly explained, and the unprecedented potential of 3D virtual evaluation of treatment outcome, documented. Finally, after provision of all this essential background information, the closing chapter illustrates the application of the 3D virtual approach in different types of maxillofacial deformity. Abstract
Poor adherence to therapy is one of the main obstacles to treatment effectiveness in schizophrenia. It is the main determinant of relapse, hospitalization, symptom persistence, and poor psychosocial functioning and outcome. Adherence to treatment is affected by various factors related to the disease characteristics, to the patient him- or herself, to the treatment, and to the therapeutic relationship. Some of these factors are modifiable, and both pharmacological and non-pharmacological strategies have been developed for this purpose.This book addresses the different aspects of adherence to treatment in schizophrenia and related disorders in a systematic but easy-to-use manual format. Chapters focus on a full range of issues, including pharmacological and non-pharmacological strategies to enhance adherence and continuity of care, relevant psychological factors, the importance of the patient-doctor relationship, and the need for an alliance with other care-givers.

The authors have developed a treatment based on the integration of object relations theory, family systems, attachment, developmental neurobiology and cognitive behavioral approaches that focuses on clearing blockages to normal identity development and adaptive functioning. While most adolescents build satisfying interpersonal relationships, are successful in school and work, and begin romantic relationships, there is a minority of adolescents who do not succeed in this and are at a high risk of developing problems in school, work, and relationships, problems with affect regulation as well as engaging in a wide range of self-destructive behaviors. In addition to a description of the disorder and assessment, this manual offers extensive clinical examples and concrete interventions, with phase-specific treatment components, including a clear treatment frame, psychoeducation, environmental interventions (with a "Home Plan" that addresses self-care behaviors, responsibilities and improved boundaries that fosters the development of better relationships between the adolescent and family), and parenting strategies, all in the service of creating a space for the individual work with the adolescent.

By using this unique method, practitioners ranging from surgeons-in-training, gastroenterologists-in-training and those early in their career to senior colorectal specialists and gastroenterologist who want to incorporate or improve their advanced endoscopic skills will be able utilize techniques and learn from this gathering of experts. The guiding principle of this work is to create a resource for surgeons and gastroenterologists that extends beyond the currently available texts, and that surgeons and gastroenterologists can turn to when wanting to "brush up" on techniques, find a useful "tip or trick" for a complex patient, or simply learn a reproducible methods for advanced endoscopic procedures. This unique book highlights current knowledge, demonstrates standards of medical care, and provides clear step-by-step reproducible techniques even for the most advanced procedures. Beyond the simple application of technical knowledge the book addresses the deeper questions about the optimal "next step" in dealing with more complex patients. International experts also address future challenges and innovations in lower gastrointestinal endoscopy. Finally, it focuses on specific "tips and tricks" that experts in the field have learned. This new, revised edition of Measuring the Skin reflects these advances by presenting the current theory and practice of noninvasive investigation and measurement of the skin and its appendices in health and disease. Skin functions and their measurement are then discussed in detail, with sections on mechanical protection, photoprotection, barrier function, immune function, thermoregulation, and sensory function.
