Abstract
This book presents all the current evidence in assessing and making decisions for patients. The multi-disciplinary team of authors pool their knowledge to bring the most up-to-date information regarding the underlying diseases, diagnostics and non operative therapies found in hepatobiliary and pancreatic surgery. The chapters reflect the constant efforts made by the authors to evaluate the current evidence, make the correct decisions and provide the best clinical care for their patients. Difficult Decisions in Hepatobiliary and Pancreatic Surgery: An Evidence-Based Approach reflects the challenging decisions faced by busy, thoughtful surgeons specializing in these fields of surgery. It is a timely resource for practicing surgeons, surgeons in training as well as educators that describes the recommended ideal approach, rather than customary care, in selected clinical situations.

Abstract
This book provides a comprehensive overview of dislocation of the temporomandibular joint (TMJ), covering all relevant aspects, from aetiology and diagnosis to management. The treatment-oriented chapters describe nonsurgical, surgical and arthroscopic interventions, and the book closes by examining the role of total alloplastic reconstruction of the TMJ in patients with chronic jaw joint dislocation. Each chapter commences with a summary of key points and the clearly written text is supported by numerous clinical photographs, as well as by videos to which the reader will have online access. All of the authors are recognized experts in the topics that they discuss. Dislocation of the TMJ constitutes a medical emergency since prompt treatment is required in order to prevent the harmful sequelae that may arise once spasm of the muscles of mastication has become established, preventing return of the condyle into the glenoid fossa. If the condition is left untreated, radica l corrective surgery may be required to restore jaw function and eliminate pain. In presenting state of the art knowledge on TMJ dislocation, this book will be of value for all involved in the management of the condition.
Contents
1. Introduction: Etiology, Presentation & Diagnosis -- 2. Imaging of the TMJ -- 3. Acute TMJ Dislocation & Technique of Manual Reduction -- 4. Chronic TMJ Dislocation -- 5. Conservative Management -- 6. Surgical Management: Clearing the Path -- 7. Surgical Management: Obstructing the Path -- 8. Arthroscopic Management -- 9. Role of Total Joint Replacement in the Management of TMJ Dislocation.

Abstract
Measures of Personality and Social Psychological Constructs provides a systematic standardized evaluation of approximately 100 scales in 25 areas. Each chapter is written by an expert in the respective area and includes: the variable scale measures, a description of the scale, reliability & validity information, and examples from the scale. In addition, there will be a discussion of which scales are best used for what purpose and population. Relative to the earlier book, this work will cover twice as many areas (25 relative to 12), and focus only on the most promising scales used for that area, rather than trying to be comprehensive in covering all scales. Coverage will be expanded per scale covered, and more overall topics covered, but with fewer scales covered per topic.

Abstract
This book offers an overview of the clinical applications of PET/MR imaging through a case-based format. Hybrid PET/MRI provides functional and anatomical information via one setting offering superior imaging quality with lower radiation dose being administered to the patient. The cases in this book focus on the use of this technique in the diagnosis of oncologic, neurologic, cardiovascular, infectious and inflammatory, and pediatric diseases. Each case is presented with the patient history, protocols, interpretation of findings, and pearls and pitfalls accompanied by high quality PET/MR images. The major strength of this book is the discussion of both MRI and PET findings pertinent to each particular case. It expands the discussion of oncologic applications of this modality through aávariety of cases that highlight staging, treatment response, and follow up. Illustrating a spectrum of PET/MRI clinical applications, PET/MR Imaging: A Case-Based Approach is a valuable resource for radiologists, nuclear medicine physicians, and residents.

Abstract
This in-depth text addresses how to approach and treat the chronic pain patient struggling with problematic opiate use. It discusses the approach for patients who may be at high risk of problematic use, such as those with a history of mental illness or substance use disorder. The first part covers diagnosis and treatment, focusing on common best practices that practitioners can adapt to any practice. Two of the chapters detail alternative and replacement therapies for opiates. Two more cover special issues in the treatment of women and older patients. The second part reviews the ethical, legal, regulatory, and policy issues surrounding the treatment of patients with comorbid pain and addiction. The book includes strategies for documentation that mitigate the risk of legal issues or ethical boundary crossings. The last part of the book addresses treating comorbid pain and opiate abuse in different medical settings and the treatment of co-occurring mental illnesses and substance abuse. Treating Comorbid Opioid Addiction and Dependence in Chronic Pain is of great value to psychiatrists, pain physicians, primary care providers, social workers, drug rehabilitation centers, and other behavioral health professionals.

Abstract
The fifth revised edition of this highly successful book presents the most extensive enhancement since Using and Understanding Medical Statistics was first published 30 years ago. Without question, the single greatest change has been the inclusion of source code, together with selected output, for the award-winning, open-source, statistical package known as R. This innovation has enabled the authors to de-emphasize formulae and calculations, and let software do all of the ‘heavy lifting’. This edition also introduces readers to several graphical statistical tools, such as Q-Q plots to check normality, residual plots for multiple regression models, funnel plots to detect publication bias in a meta-analysis and Bland-Altman plots for assessing agreement in clinical measurements. New examples that better serve the expository goals have been added to a half-dozen chapters. In addition, there are new sections describing exact confidence bands for the Kaplan-Meier estimator, as well as negative binomial and zero-inflated Poisson regression models for over-dispersed count data. The end result is not only an excellent introduction to medical statistics, but also an invaluable reference for every discerning reader of medical research literature.
Contents
Basic concepts -- Tests of significance -- Fisher's test for 2 x 2 contingency tables -- Approximate significance tests for contingency tables -- Some warnings concerning 2 x 2 tables -- Kaplan-Meier or 'actuarial' survival curves -- The log-rank or Mantel-Haenszel test for comparing survival curves --An introduction to the normal distribution -- Analyzing normally distributed data -- Linear regression models for medical data -- Binary logistic regression -- Regression models for count data -- Proportional hazards regression -- The analysis of longitudinal data -- Analysis of variance -- Data analysis -- The question of sample size -- The design of clinical trials -- Further comments regarding clinical trials -- Meta-analysis -- Epidemiological applications -- Diagnostic tests -- Agreement and reliability.
