Abstract
This book systematically focuses on central sleep apneas, analyzing their relationship especially with heart failure and discussing recent research results and emerging treatment strategies based on feedback modulation. The opening chapters present historical background information on Cheyne-Stokes respiration (CSR), clarify terminology, and explain the mechanics and chemistry of respiration. Following a description of the physiology of respiration, the pathophysiology underlying central apneas in different disorders and particularly in heart failure is discussed. The similarities and differences of obstructive and central apneas are then considered. The book looks beyond the concept of sleep apnea to daytime CSR and periodic breathing during effort and contrasts the opposing views of CSR as a compensatory phenomenon or as detrimental to the failing heart. The diagnostic tools currently in use for the detection of CSR are thoroughly reviewed, with guidance on interpretation of findings. The book concludes by describing the various forms of treatment that are available for CSR and by explaining how to select patients for treatment.

This open access book details tools and procedures for data collections of hard-to-reach, hard-to-survey populations. Inside, readers will discover first-hand insights from experts who share their successes as well as their failures in their attempts to identify and measure human vulnerabilities across the life course. Coverage first provides an introduction on studying vulnerabilities based on the Total Error Survey framework. Next, the authors present concrete examples on how to survey such populations as the elderly, migrants, widows and widowers, couples facing breast cancer, employees and job seekers, displaced workers, and teenagers during their transition to adulthood. In addition, one essay discusses the rationale for the use of life history calendars in studying social and psychological vulnerability while another records the difficulty the authors faced when trying to set-up an online social network to collect relevant data. Overall, this book demonstrates the importance to have, from the very beginning, a dialogue between specialists of survey methods and the researchers working on social dynamics across the life span. This is the 3rd, and much expanded edition, of a text that was first published by Rubin and Crocker in 1989 (Lea and Fibiger). The second edition was published by Rubin and Crocker in 2006 (Paul Brookes) and has been used as a formal required text in training programs for physicians, nurses and nurse practitioners as well as by administrators who are responsible for programs serving people with IDD. This book is considered the "Bible" in the field of health care for people with IDD since 1989 when the first edition came out.

Abstract
This practice-oriented book provides a comprehensive and up-to-date review of the history, surgical anatomy, etiology, pathogenesis, clinical presentation, and treatment of primary, secondary, and tertiary hyperparathyroidism. The coverage is wide ranging, encompassing, for example, innovations in both medical and surgical treatment, current indications for parathyroidectomy, the role and performance of minimally invasive surgery, the value of intraoperative parathyroid hormone monitoring, and guidance on reoperations. Individual chapters are devoted to particular conditions and disease settings, including multiple endocrine neoplasia types 1 and 2 and parathyroid carcinoma, with provision of information on genetic testing, clinical manifestations, and therapy. All aspects of secondary hyperparathyroidism in predialysis and dialysis patients are discussed. The book is endorsed by the Italian Society of Surgery. This is the first comprehensive review of the edge-to-edge technique for mitral repair, which was introduced into the surgical armamentarium in 1991 by one of the authors, Ottavio Alfieri. The surgical technique employed in edge-to-edge repair today provides the basis for the only reliable method of percutaneous correction of degenerative mitral regurgitation. The book traces the evolution of the edge-to-edge technique from a surgical to a percutaneous approach, documenting indications, contraindications, technical aspects, controversies, and clinical outcomes. Detailed attention is devoted to the MitraClip procedure, which reproduces the Alfieri stitch percutaneously and has to date been used in more than 20,000 patients worldwide. The state of the art in MitraClip therapy is described, with clear explanation of when and how to use the MitraClip system. The reader will learn all aspects of percutaneous edge-to-edge repair, which is recommended for use in carefully selected patients in the new ESC/EACTS guidelines for the management of valvular heart disease and in the ESC guidelines on heart failure. The group at San Raffaele Hospital, Milan, has accumulated by far the largest experience in the world with the technique and over the years has been very active in its promotion and scientific evaluation. This book reflects that depth of knowledge and will be of great interest to cardiac surgeons and interventional and clinical cardiologists.
