Abstract
Each chapter of this new book on advanced lower GI endoscopy and endoluminal surgery focuses on the thought process and step-wise technical approach to the condition and procedure listed. By using this unique method, practitioners ranging from surgeons-in-training, gastroenterologists-in-training and those early in their career to senior colorectal specialists and gastroenterologist who want to incorporate or improve their advanced endoscopic skills will be able utilize techniques and learn from this gathering of experts. The guiding principle of this work is to create a resource for surgeons and gastroenterologists that extends beyond the currently available texts, and that surgeons and gastroenterologists can turn to when wanting to "brush up" on techniques, find a useful "tip or trick" for a complex patient, or simply learn a reproducible methods for advanced endoscopic procedures. This unique book highlights current knowledge, demonstrates standards of medical care, and provides clear step-by-step reproducible techniques even for the most advanced procedures. Beyond the simple application of technical knowledge the book addresses the deeper questions about the optimal "next step" in dealing with more complex patients. International experts also address future challenges and innovations in lower gastrointestinal endoscopy. Finally, it focuses on specific "tips and tricks" that experts in the field have learned. The format follows that of both a "how to" manual as well as an algorithm-based guide to allow the reader to understand the thought process behind the proposed treatment strategy. Throughout the text, each author provides an ongoing narrative of his/her individual techniques along with color illustrations and diagrams to "personally" take the reader through the crucial steps of the procedure, and key points of patient care inherent to that topic. Additionally, where appropriate, links to online videos will give the reader an up-front look into technical aspects of EMR, ESD, endoscopic stent placements, CELS, as well as NOTES. This book will be useful to colorectal surgeons, general surgeons, and gastroenterologists who want to learn or improve their skills in lower gastrointestinal endoscopy and advanced endoscopic interventions. Furthermore, this book will be of particular interest to the surgeons-in-training, and gastroenterologist-in-training that are often called upon to manage a variety of colorectal conditions through an endoscopic approach.

Abstract
This unique book is intended for all health professionals caring for older people with diabetes such as specialist and general nurses, doctors, primary care practioners and dieteticians. Although there is an increasing body of work about personalised care, no publications were identified that encompass the focus and scope of the proposed book. The global population is ageing and increasing age is a key risk factor for diabetes. Older people with diabetes are often vulnerable, have complex care needs and often have cognitive changes, which makes personalising care challenging for health professionals. Thus, this is an internationally relevant book filling a gap in the current literature. This is a practical and updated book that will use an engaging and easy to read narrative style. It challenges readers to reflect in and on their practice. It encompasses people with diabetes' and authors' stories, which are known to have a special interest to readers, make it easier to apply the information to practice, enhance learning, and hence the relevance and value of the book. It is relevant to advocacy organisations as well as managers and service planners. Researchers and scientists may find relevant information on grant and ethics applications, research protocols, plain language statements for potential participants and operationalising research protocols.

Abstract
This third edition text provides a completely revised and updated new version of this unique, modern, practical text that covers the strategic evaluation, specific approaches, and detailed management techniques utilized by expert colorectal surgeons caring for patients with complex problems--whether they result from underlying colorectal disease or from complications arising from previous surgical therapy. The format follows that of both a "how to" manual as well as an algorithm-based guide to allow the reader to understand the thought process behind the proposed treatment strategy. By making use of evidence-based recommendations, each chapter includes not only background information and diagnostic/therapeutic guidelines, but also provides a narrative by the author on his/her operative technical details and perioperative "tips and tricks" that they utilize in the management of these complex surgical challenges. Distinctive to this book, is the reliance on experts in the field including past presidents of the ASCRS, as well as multiple other national and internationally recognized surgeons, to lend their personal insight into situations where data may be more sparse, but individual and collective experience is paramount to making sound decisions and thereby optimizing patient outcomes. The text includes chapters on the assessment of risk and methods utilized to minimize perioperative complications. In addition, it incorporates sections covering the medical and surgical therapies for abdominal, pelvic and anorectal disease. Moreover, the technical challenges of managing complications resulting from the original or subsequent operations is addressed. The underlying focus throughout the text is on providing pragmatic and understandable solutions that can be readily implemented by surgeons of varying experience to successfully treat complex colorectal problems. However, it also goes beyond the technical aspects of colorectal surgery and includes special sections highlighting the essence of a surgeon; covering aspects involving the medical-legal, ethical, and economic challenges confronting surgeons. Throughout the text, each author provides an ongoing narrative of his/her individual surgical techniques along with illustrations and diagrams to "personally" take the reader through the crucial steps of the procedure, and key points of patient care inherent to that topic. Additionally, where appropriate, links to online videos give the reader an up-front look into technical aspects of colorectal surgery.

Abstract
This text gathers the weaknesses revealed during recent infections outbreaks and organizes them into a guide for combating the trends in emerging infections as they relate to hospital preparedness. As the first book to exclusively explore infectious emergencies, the text begins by reviewing potential pathogens and the clinical issues that may threaten hospital safety before delving into the best operational guidelines for commanding a staff under extreme circumstances, including incident command, communication, transport, maintenance, and a myriad of other topics that can remain manageable with proper protocol. Written by experts in the field, this text is the only one that offers the most effective clinical responses to a crisis at every level of care, including special population, laboratory techniques, care of the deceased, behavioral support, and medical documentation. The text concludes by focusing on the reality of care by introducing true examples from the field and the lessons gained from these cases. Bioemergency Planning is a vital resource for infectious disease specialists, hospitalists, epidemiologists, internal medicine physicians, nurses, social workers, public health officials, and all medical professionals who need to be prepared to respond to an infection outbreak.

Abstract
Regarded as the authoritative reference and text, this handbook presents the most effective, widely studied approaches to couple therapy. The distinguished coeditors bring together other leading experts, most of whom developed the approaches they describe. Adhering closely to a uniform structure to facilitate study and comparison, chapters cover the history, theoretical and empirical underpinnings, and techniques of each model. The volume also describes cutting-edge applications for particular relationship contexts (such as blended families, LGBT couples, and separated couples) and clinical problems (such as partner aggression, psychological disorders, and medical issues). New to This Edition: Chapters on interpersonal neurobiology and intercultural relationships; Chapters on couple therapy for PTSD, functional analytic couple therapy, and the integrative problem-centered metaframeworks approach; Many new authors.*Extensively revised with the latest theory and research.

Abstract
This is a book about primary care clinicians and the clinical uncertainty endemic to their work. Even when seemingly straightforward, each patient raises unique questions regarding how best to listen to their complaints, empathize with their suffering, or respond to their silences. This book is also about addressing uncertainty in primary care practice and engaging it. Engagement requires knowledge, explicit and tacit, placed in the service of a single patient's problem. It also requires carefully managed communication, facilitating dialogue with the patient and encouraging shared problem-solving. Most importantly, this book is about collaborative engagement with case-based uncertainty in the setting of small groups of clinicians. Sommers and Launer contend that the medical profession's tradition of working independently should be augmented with an explicitly shared, collegial one of jointly creating wisdom through practice-based learning. An international panel of expert clinicians and educators provides: Perspectives on clinical uncertainty in the medical literature; A taxonomy of clinical uncertainty with patient examples; Analysis of the educator role to support clinicians in engaging uncertainty; A compendium of small group methods for collaborative engagement with clinical scenarios; Analysis of the special challenges of collaborative engagement. A mind-opening manifesto, Clinical Uncertainty in Primary Care will equip primary care clinicians, educators, public health and behavioral health professionals with resources for infusing practice with meaning through collegial collaboration. From the Foreword: "Lucia Sommers and John Launer, with the accompanying input of their contributing authors, have done a deeply insightful and close to exhaustive job of defining clinical uncertainty. They identify its origins, components, and subtypes; demonstrate the ways in which and the extent to which it is intrinsic to medicine--and present a cogent case for its special relationship to primary care practice--Clinical Uncertainty in Primary Care not only presents a model of collegial collaboration and support, it also implicitly legitimates it." Renee Fox, Annenberg Professor Emerita of the Social Sciences, University of Pennsylvania.

Abstract
Complexities in Colorectal Surgery: Decision-Making and Management provides a unique, modern, practical guide that covers the strategic evaluation, specific approaches, and detailed management techniques utilized by expert colorectal surgeons caring for patients with complex problems whether they result from underlying colorectal disease or from complications arising from previous surgical therapy. The text is formatted as both a "how-to" manual as well as an algorithm-based guide allowing the reader to understand the thought process behind the proposed treatment strategies. By making use of evidence-based recommendations, each chapter will include not only diagnostic and therapeutic guidelines, but also a narrative by the author on his/her operative technical details and perioperative "tips and tricks" that they utilize in the management of these complex surgical challenges. This test includes chapters on the assessment of risk and nutritional intervention methods utilized to minimize perioperative complications. In addition, sections on medical and surgical therapies for abdominal, pelvic and anorectal disease are incorporated. Moreover, the technical challenges of managing complications resulting from the original or subsequent operations is addressed. The underlying focus throughout the text is to provide pragmatic and understandable solutions that can be readily implemented by surgeons of varying experience to successfully treat complex colorectal problems. The text also goes beyond the technical aspects of colorectal surgery and includes special sections highlighting the essence of a surgeon. These include chapters describing the importance of the first encounter, building patient rapport, and demonstrating confidence and competence while showing humility and avoiding arrogance. Additionally, aspects involving the medical-legal, ethical, and economic challenges confronting surgeons are covered.

Abstract
Ability to learn from errors is an essential aspect of the quest to improve treatment quality and patient safety. This book consists of 33 cases in anesthesiology that are based on real life situations and illuminate avoidable complications and mishaps. The cases are presented in a novel manner in that they are embedded within narratives. The reader comes to each case cold, without any clue as to the content, and each case comprises a narrative and a factual component that are interwoven. The narrative parts provide the reader with information and tips regarding the clinical problems and tasks that the protagonist must face and try to solve. The idea is to engage the reader emotionally while reading and to entertain him or her while learning. All cases conclude with short debriefing sections which include possible strategies to prevent similar errors or mishaps.

Abstract
Contemporary psychodynamic theory profoundly impacts our understanding of the development of psychopathology in young patients. However, while much has been written about this theory's influence on the psychotherapy techniques used with adults, little attention has been paid to its practical clinical applications in psychotherapeutic work with children and adolescents. This book explains how the new concepts derived from contemporary psychodynamic theory have made it necessary to revise our understanding of and approach to young patients in psychotherapy. It reviews the recent contributions from contemporary two-person relational psychodynamic theory and employs detailed case examples to bring to life this theory's practical applications in child and adolescent psychotherapy. Psychotherapists and students of psychotherapy will find this book a valuable source of information on contemporary psychodynamic theory and a useful resource for introducing a contemporary style into their practice, co-constructing with the patient a narrative to achieve the desired goals.
