Evidence-based Clinical Practice (EBCP) is the conscientious, explicit, and judicious use of current best external evidence in making decisions about the care of individual patients.In neurology, practice has shifted from a rich, descriptive discipline to one of increasingly diagnostic and therapeutic interventions. Providing a comprehensive review of the current best evidence, Neurology: An Evidence-Based Approach presents this type of evidence in a concise, user-friendly and easily accessible manner. The three co-editors of this important volume are linked in their passion for evidence-based clinical practice in the clinical neurological sciences, connected to a common historical origin at the University of Western Ontario (UWO), London, Ontario Canada and influenced directly by Evidence-Based Medicine teachings of McMaster University, Hamilton, Ontario Cananda. The book is organized in three sections: Basics of Evidence-Based Clinical Practice, with an introduction to the topic, a chapter on the evolution of the hierarchy of evidence, and another chapter on guidelines for rating the quality of evidence and grading the strength of recommendation. Abstract
The obesity pandemic continues to increase on a world-wide basis with over 70% of the United States population being either overweight or obese. Hematologic malignancies have recently been identified among the obesity associated malignancies spanning the lifespan from childhood to the elderly and include leukemia, myeloma, lymphoma and others. In addition to the potential etiologic association between obesity and hematologic malignancies, the presence of obesity has profound effects on therapy by impacting pharmacokinetics of chemotherapeutic agents, dose, adipocyte metabolism and drug distribution. These may be particularly important in hematopoietic stem cell transplantation. Another important aspect of the association of obesity with hematologic malignancies is the increased incidence of obesity in children who successfully complete therapy for acute lymphoblastic leukemia. This and other observations indicate important relations between the hematopoietic systems and adipose tissue metabolism. Energy Balance in Hematologic Malignancies will provide an important volume in the series on Energy Balance and Cancer and serve as a basis for better understanding etiology, mechanisms, therapeutic implications and experimental approaches. While this book should be of specific interest and utility to all pediatric and adult hematologists and oncologists dealing with hematologic malignancies, it should also provide an important resource for all investigators concerned, from a clinical or research basis, with the impact of obesity and exercise on hematologic malignancies.

While adenovirus eye infections are very common, caused by a known agent, and highly infectious, TSPK is comparatively rare, of unknown causation, and seemingly non-contagious. Despite these differences, the two diseases are often confused in clinical practice owing to the similarity between adenovirus epithelial infiltrates and 'coarse' epithelial TSPK lesions. This book presents the morphological features, dynamics, and sequelae of adenovirus and Thygeson's keratitides captured at high magnification in the living human cornea. It thereby fills the existing void between conventional photographs and slit-lamp observations. Case reports demonstrate the importance of patient history in differential diagnosis, illustrate the need for familiarity with early manifestations of adenovirus infections, and assist in the diagnosis of rare variants of TSPK. Furthermore, the detailed observations on the natural course of the diseases ensure that the book will serve not only as a diagnostic tool but also as a reference when evaluating the effects of potential new treatments.

This book provides a comprehensive guide to current knowledge about nevogenesis by presenting these latest advances and in addition discusses issues yet to be resolved. It will assist practicing physicians in effectively managing patients with a variety of nevi and will also be of great value to researchers in the field. Importantly, since nevi are associated with an increased risk of melanoma, understanding nevogenesis may help to unravel some of the mysteries of melanomagenesis.

This book presents in vivo captured high-magnification images of two conditions: keratoconjunctivitis sicca (KCS, or dry eye), an extremely common disease of the ocular surface, and filamentary keratopathy, a relatively rare phenomenon most commonly associated with KCS. Although most of the images of KCS originated from patients with Sjœgren's syndrome, they fairly represent the broad spectrum of ocular surface changes seen in the condition. The appearance of the ocular surface and of the mucus component discernible in the tear film is clearly depicted, and long-term observations, less common KCS cases, and images showing iatrogenic epithelial damage are included. The images of filamentary keratopathy clearly reveal the components of the ocular surface appendices, termed filaments, and assist in explaining the mechanisms underlying the formation of these filaments. The photographs show phenomena captured in various illumination modes, without staining and after staining with diagnostic dyes, and the photographic sequences illustrate their dynamics. The images reflect the in vivo situation. Once aware of the various phenomena, anyone working with standard diagnostic equipment - the slit lamp and the diagnostic dyes- will be able to detect almost all of them. The book will be invaluable for all who deal with ocular surface diseases, including general practitioners, medical eye specialists, ocular surgeons, optometrists, opticians, and rheumatologists.

Abstract
This book covers all aspects of chronic radiation syndrome (CRS) based on observations in a unique sample of residents of the Techa riverside villages in the southern Urals who were exposed to radioactive contamination in the 1950s owing to releases of liquid radioactive wastes from Mayak Production Association, which produced plutonium for weapons. In total, 940 cases of CRS were diagnosed in this population and these patients were subjected to detailed analysis. The opening chapters address the definition and classification of CRS, epidemiology and pathogenesis, covering molecular and cellular mechanisms, radioadaptation, and the role of tissue reactions. The pathoanatomy of CRS during the development and recovery stages is discussed for all organ systems. Clinical manifestations of CRS at the different stages are then described in detail and the dynamics of hematopoietic changes are thoroughly examined. In the following chapters, principles of diagnosis (including assessment of the exposure doses to critical organs) and differential diagnosis from a wide range of other conditions are discussed and current and potential treatment options, described. The medical and social rehabilitation of persons with CRS is also covered. This book, which casts new light on the condition, will be of value for all practitioners and researchers with an interest in CRS.

Abstract
The purpose of this book is to improve diagnostic yields of capsule endoscopy and double-balloon endoscopy, because those procedures can depict nonspecific findings that may not lead to a proper diagnosis. Another reason for the publication was recognition of the difficulty in distinguishing enteroscopic findings of ulcerative colitis from those of Crohn's disease. From a practical point of view, it is important to observe endoscopic pictures first, then to compare the images of other modalities, and finally to compare macroscopic pictures of resected specimens. For that reason, a large number of well-depicted examples of small intestinal lesions were assembled to clarify differences among small intestinal lesions that appear to exhibit similar findings and morphologies. Comparisons with radiographic findings comprise another important element in diagnosis. There are limitations in endoscopic observations of gross lesions of the small intestine, with its many convolutions. In Japan, many institutions still practice double-contrast imaging, which provides beautiful results. Because a single disorder may exhibit variations, this volume includes multiple depictions of the same disorders. Also included are lesions in active and inactive phases, as both appearances are highly likely to be encountered simultaneously in clinical practice. The number of illustrated findings therefore has been limited to strictly selected cases.
