Adenovirus epithelial keratitis and Thygeson's superficial punctate keratitis (TSPK) are apparently disparate diseases. While adenovirus eye infections are very common, caused by a known agent, and highly infectious, TSPK is comparatively rare, of unknown causation, and seemingly non-contagious. Despite these differences, the two diseases are often confused in clinical practice owing to the similarity between adenovirus epithelial infiltrates and 'coarse' epithelial TSPK lesions. This book presents the morphological features, dynamics, and sequelae of adenovirus and Thygeson's keratitides captured at high magnification in the living human cornea. It thereby fills the existing void between conventional photographs and slit-lamp observations. Case reports demonstrate the importance of patient history in differential diagnosis, illustrate the need for familiarity with early manifestations of adenovirus infections, and assist in the diagnosis of rare variants of TSPK. Furthermore, the detailed observations on the natural course of the diseases ensure that the book will serve not only as a diagnostic tool but also as a reference when evaluating the effects of potential new treatments.

Abstract

The best known of these are the Broca and Wernicke regions, and these have become synonyms for the location of productive and receptive language functions respectively. This Broca-Wernicke model has proved to be such a powerful concept that is remains the predominant view in modern clinical practice. What is fascinating, however, is that there is little evidence for this strictly localist view on language functions. Modern neuroscience and numerous clinical observations in individual patients show that language functions are represented in complex and ever-changing neural networks. It is fair to say that the model is wrong, and that Broca's and Wernicke's areas in their classic forms do not exist. This is a fascinating paradox: why do neurologists and neurosurgeons continue to use these iconic language models in everyday decision-making? In this book, the author uses his background as a neurosurgeon and a neuroscientist to provide some answers to this question. The book acquaints clinicians and researchers with the many different aspects of language representation in the brain. It provides a historical overview of functional localisation, as well as insights into the misjudgements that have kept the localist doctrine alive. It creates an awareness of the need to integrate clinical observations and neuroscientific theories if we want to progress further in clinical language research and patient care.

Paul Gerson Unna's "Abtropfung" theory that nevi develop through migration of nevus cells from the epidermis to the dermis prevailed for almost a century until the "Hochsteigerung" theory postulated the reverse pattern of migration based on newly acquired observations from histopathology and studies of embryogenesis. Most recently, however, new insights gained from epidemiology, cross-sectional and longitudinal studies of nevi, dermoscopy and confocal microscopy, and cellular and molecular studies have brought into question both of the aforementioned theories. This book provides a comprehensive guide to current knowledge about nevogenesis by presenting these latest advances and in addition discusses issues yet to be resolved. It will assist practicing physicians in effectively managing patients with a variety of nevi and will also be of great value to researchers in the field. This book presents in vivo captured high-magnification images of two conditions: keratoconjunctivitis sicca (KCS, or dry eye), an extremely common disease of the ocular surface, and filamentary keratopathy, a relatively rare phenomenon most commonly associated with KCS. Although most of the images of KCS originated from patients with Sjœgren's syndrome, they fairly represent the broad spectrum of ocular surface changes seen in the condition. The appearance of the ocular surface and of the mucus component discernible in the tear film is clearly depicted, and long-term observations, less common KCS cases, and images showing iatrogenic epithelial damage are included. The images of filamentary keratopathy clearly reveal the components of the ocular surface appendices, termed filaments, and assist in explaining the mechanisms underlying the formation of these filaments. The photographs show phenomena captured in various illumination modes, without staining and after staining with diagnostic dyes, and the photographic sequences illustrate their dynamics. The images reflect the in vivo situation. Once aware of the various phenomena, anyone working with standard diagnostic equipment - the slit lamp and the diagnostic dyes- will be able to detect almost all of them. The book will be invaluable for all who deal with ocular surface diseases, including general practitioners, medical eye specialists, ocular surgeons, optometrists, opticians, and rheumatologists.

Single-access laparoscopic surgery (SALS), performed using a single transumbilical access, has recently proved to bring advantages over other forms of laparoscopic surgery due to the reduction in the number of incisions and the consequent decrease in morbidity, less blood loss and postoperative pain. Furthermore, cosmesis may be improved because incision within the umbilicus renders surgery virtually scarless. Nevertheless, as with all new surgical concepts, questions may be raised regarding safety, usefulness, and appropriateness. This book analyzes the use of SALS for different indications and abdominal surgical procedures based on a careful literature review of the experience of the most important surgical teams employing this approach and also the authors' personal observations. The coverage is broad, encompassing, for example, the role of SALS in appendectomy, repair of abdominal wall defects, cholecystectomy, gastric surgery, nephrectomy, splenectomy, hemicolectomy, and pancreatic, liver, and gynecological surgery.

The obesity pandemic continues to increase on a world-wide basis with over 70% of the United States population being either overweight or obese. Hematologic malignancies have recently been identified among the obesity associated malignancies spanning the lifespan from childhood to the elderly and include leukemia, myeloma, lymphoma and others. In addition to the potential etiologic association between obesity and hematologic malignancies, the presence of obesity has profound effects on therapy by impacting pharmacokinetics of chemotherapeutic agents, dose, adipocyte metabolism and drug distribution. These may be particularly important in hematopoietic stem cell transplantation. Another important aspect of the association of obesity with hematologic malignancies is the increased incidence of obesity in children who successfully complete therapy for acute lymphoblastic leukemia. This and other observations indicate important relations between the hematopoietic systems and adipose tissue metabolism. Energy Balance in Hematologic Malignancies will provide an important volume in the series on Energy Balance and Cancer and serve as a basis for better understanding etiology, mechanisms, therapeutic implications and experimental approaches. In total, 940 cases of CRS were diagnosed in this population and these patients were subjected to detailed analysis. The opening chapters address the definition and classification of CRS, epidemiology and pathogenesis, covering molecular and cellular mechanisms, radioadaptation, and the role of tissue reactions. The pathoanatomy of CRS during the development and recovery stages is discussed for all organ systems. Clinical manifestations of CRS at the different stages are then described in detail and the dynamics of hematopoietic changes are thoroughly examined. In the following chapters, principles of diagnosis (including assessment of the exposure doses to critical organs) and differential diagnosis from a wide range of other conditions are discussed and current and potential treatment options, described. The medical and social rehabilitation of persons with CRS is also covered. This book, which casts new light on the condition, will be of value for all practitioners and researchers with an interest in CRS.
