A practical treatment algorithm for clinicians treating patients with massive rotator cuff tears is also included. Each chapter opens with pearls and pitfalls covering the main key points for quick reference. The overarching theme of this book is that patients with similar imaging findings may demonstrate very different clinical presentations, and the final treatment recommendation should be made based on their complaints and expectations. It is unclear, in most circumstances, which single clinical test or indeed radiological investigation is the best for diagnosing a particular condition. In fact, the diagnosis of a particular shoulder condition rests on the four sub-components of clinical history, conventional examination, special tests and radiological investigations. It is often the case that all these sub-components are to be used in conjunction with each other as ǣclustersǥ rather than in isolation. Tumors of the parotid gland present particular problems for the surgeon, especially when they extend beyond the borders of the parotid tissue. Reconstruction of the postoperative defect is especially difficult, not only due to the complexity and density of the anatomical structures in the lateral region of the head and neck, but also because of the general condition of most patients, who tend to be elderly and to have accompanying disorders. The difficulty in managing extended parotid tumors is increased by the need to sacrifice the facial nerve, even if only partially.
