Abstract
Adventure and extreme sports have developed significantly and gained enormous popularity over the past two decades, and are now performed by adventurous elite athletes as well as the recreational adventure sportsman. These sports, by definition, involve element of increased risk, and are usually performed in beautiful, exciting and remote locations or in extreme environments, far away from medical assistance. As more and more people are enjoying adventure sports, unfortunately increased numbers are becoming injured as a result. Future research is progressing alongside the sport development, to allow the sport mechanisms, injury patterns and predisposing factors to be better understood. It is the hope of all researchers to make the sports safer without detracting from their adventurous nature. This book combines the Editors love to the sports with their life as sports surgeons. The aim and scope of this text is to bring the sports medicine' involved in each of these sports into one volume. Each sport and discipline is explained together with their subtle similarities and differences, the common injury mechanisms, patterns of injury and treatment options and finally, the mental and physiological aspects of it all. In this book the Editors hope to deliver useful valuable information to surgeons, physicians, physical therapists and whoever treats or has interest in these amazing activities.

Abstract
When it was developed, the Ilizarov device represented a revolutionary advance that made it possible to correct previously untreatable conditions through the stimulation of bone growth based on the principle of distraction osteogenesis. The device subsequently gained popularity among surgeons throughout the world since it can be used for the treatment of fractures that have failed to heal satisfactorily and for deformity correction. The technique is, however, complex and requires specialist knowledge if it is to be applied optimally.This is the second edition of a well-received book that focuses primarily on external fixation using the Ilizarov device but also considers other devices employed for the purpose. The opening chapters include discussion of biomechanical principles, use of a system of coordinates to allow safer insertion of K-wires and half pins, preoperative preparation, and principles of frame construction. External fixation of a variety of fractures in different pathologic settings is then clearly explained in a series of detailed chapters with the aid of high-quality illustrations. Numerous case reports are included to illustrate the results of different treatment methods. In addition, principles of postoperative management are described and advice is provided on correction of errors and treatment of complications. Since the first edition the text has been thoroughly updated, with inclusion of contributions from leading world experts. This volume will serve as an indispensable manual both for trainee orthopedic surgeons embarking on a steep learning curve and for more experienced surgeons requiring advice and guidance in demanding cases.

Best Evidence Structural Interventions for HIV Prevention reports on successful HIV prevention initiatives from across the globe, representing countries in Europe, Asia, Africa, and the Americas at diverse stages of the epidemic. Spotlighting major worldwide objectives--decreasing risks in IV drug use, commercial sex work, and non-commercial sexual activity and promoting HIV screening, voluntary counseling and testing (VCT), and antiretroviral therapy--these rigorously evaluated interventions are analyzed at individual and community levels. Here, experts in the field discuss the various tools available and explain how best to use them when confronted by different pathologies, drawing upon their personal experience. The text presents approaches and surgical techniques such as the trans-cerebellomedullary fissure approach and its variation to the fourth ventricle, as well as the cerebellomedullary cistern, infratentorial lateral supracerebellar approach to the fifth cranial nerve in the upper cerebellopontine angle, infrafloccular approach to the root exit zone of the seventh cranial nerve, transcondylar fossa approach through the lateral part of the foramen magnum, and the stitched sling retraction technique utilized during microvascular decompression procedures for trigeminal neuralgia and hemifacial spasm. It also describes in detail the bridging veins of the posterior fossa, especially the petrosal vein, and bridging veins to the tentorial sinuses, which can block approaches to the affected area. The original Japanese version of this work was published 8 years ago, and has established itself as a trusted guide, especially among young neurosurgeons who need to study various surgical approaches and techniques. In the course of being translated into English, some sections have been revised and new information has been added. Abstract
This contributed volume regenerates the significance of and respect for natural phenomenon in the background of the risks and complications seen in ART such as ovarian hyperstimulation and multiple pregnancies. Balancing the clinical practice between the beneficial natural physiology and the prolific use of stimulation drugs for ART may help eliminate risks that may prove clinically, financially and psychologically expensive. It underscores the fact that "greater" is not always synonymous with "better" with regard to the oocyte yield. While highlighting the risks involved with the use of high-dose gonadotropins, the book presents a stratification of patients who might benefit from the "soft" minimal and natural cycle IVF stimulation protocols that may be used. Additionally, clinicians can understand the significance of cryopreservation technology, now a useful adjunct to minimal stimulation protocols, with its manifold future advantages.
