Abstract
An essential, comprehensive, and practical guide to understanding, measuring, and modifying patients' adherence behavior to optimize treatment outcomes, this book covers all major aspects of adherence in dermatology. The first section describes current knowledge on the magnitude and impact of nonadherence; the second outlines measuring adherence in the context of databases, trials and practice. The third section addresses adherence in key skin diseases and the fourth covers numerous strategies to improve adherence by mitigating specific patient barriers. All sections emphasize ways to optimize communication with patients: the foundation for producing successful health behavior change. Low adherence helps explain why, despite effective treatments existing for all major dermatologic conditions, many patients still fail to improve. When clinicians endeavour to maximize adherence, with the aid of evidence-based strategies, these drugs will often realise their potential. This book devotes special attention to understanding why individuals may be nonadherent, so that each patient can receive a suitable treatment regimen, with a personalized plan of action. This book is designed for clinicians at all levels, serving as both introductory training for medical students and residents, and an update on the field for experienced practitioners. Researchers and policymakers will also benefit from its coverage of research methods and the impact of nonadherence in specific diseases. Adherence in Dermatology bridges the gap between outcomes researchers and clinicians by explaining the latest findings in plain language, with examples from everyday dermatologic practice.

Abstract
Each chapter of this new book on advanced lower GI endoscopy and endoluminal surgery focuses on the thought process and step-wise technical approach to the condition and procedure listed. By using this unique method, practitioners ranging from surgeons-in-training, gastroenterologists-in-training and those early in their career to senior colorectal specialists and gastroenterologist who want to incorporate or improve their advanced endoscopic skills will be able utilize techniques and learn from this gathering of experts. The guiding principle of this work is to create a resource for surgeons and gastroenterologists that extends beyond the currently available texts, and that surgeons and gastroenterologists can turn to when wanting to "brush up" on techniques, find a useful "tip or trick" for a complex patient, or simply learn a reproducible methods for advanced endoscopic procedures. This unique book highlights current knowledge, demonstrates standards of medical care, and provides clear step-by-step reproducible techniques even for the most advanced procedures. Beyond the simple application of technical knowledge the book addresses the deeper questions about the optimal "next step" in dealing with more complex patients. International experts also address future challenges and innovations in lower gastrointestinal endoscopy. Finally, it focuses on specific "tips and tricks" that experts in the field have learned. The format follows that of both a "how to" manual as well as an algorithm-based guide to allow the reader to understand the thought process behind the proposed treatment strategy. Throughout the text, each author provides an ongoing narrative of his/her individual techniques along with color illustrations and diagrams to "personally" take the reader through the crucial steps of the procedure, and key points of patient care inherent to that topic. Additionally, where appropriate, links to online videos will give the reader an up-front look into technical aspects of EMR, ESD, endoscopic stent placements, CELS, as well as NOTES. This book will be useful to colorectal surgeons, general surgeons, and gastroenterologists who want to learn or improve their skills in lower gastrointestinal endoscopy and advanced endoscopic interventions. Furthermore, this book will be of particular interest to the surgeons-in-training, and gastroenterologist-in-training that are often called upon to manage a variety of colorectal conditions through an endoscopic approach.

Abstract
This unique resource offers an in-depth, comprehensive look at different types of mental health needs of transgender and gender diverse youth, how these intersect with gender identity, gender expression, and sexual orientation, and provides practical information on how to ethically, responsibly, and sensitively care for these patients. Affirmative Mental Health Care for Transgender and Gender Diverse Youth: A Clinical Guide begins with three introductory chapters which contain practical information regarding assessment, psychological interventions, and the potential medical and surgical interventions that are indicated for youth with gender identity concerns. The remaining chapters are illustrated by multiple cases build around overarching chapter themes. Each case chapter opens with broad questions applicable to clinical practices, while the cases themselves focus on a particular co-occuring mental health condition. The case chapters are structured with intersectionality in mind, including elements of ethnic, racial, and cultural diversity, and the patients range over the full developmental spectrum, from pre-pubertal children to older adolescents. cases range in complexity as well, to provide readers with the tools they need to evaluate patients, and to assist in the decision of which presenting factors to prioritize in treatment at which time. Ending each chapter are clinical take-home messages, closing with additional practical knowledge that can be applied to other cases providers may see in their own practices. Written by expert clinicians in the field, Affirmative Mental Health Care for Transgender and Gender Diverse Youth: A Clinical Guide is an ideal resource not only for child and adolescent psychiatrists, but for clinicians across all mental health disciplines working with gender non-conforming youth, and who are interested in providing informed, affirmative, and intersectional care.

Abstract
This book is a comprehensive resource on psychotropic medications, detailing the latest methods for defining their characteristics, their use in different patient populations, and drug-drug interactions; an important collection of information for clinicians, students, researchers, and members of the pharmaceutical industry alike. The first section provides the foundational principles of these drugs. Mathematical modeling of parameters that affect their entryto, and exit from, the central nervous system (CNS) compartment are presented on an individual basis and then applied to target populations with specific disease states. Methods and characteristics that inform the transfer of these drugs from the laboratory bench to use in patient care are discussed, including imaging techniques, genetics and physiological barriers, such as the blood-brain barrier. The second section describes the characteristics of specific agents, nominally arranged intodifferent therapeutic categories and with reference crossover use in different disease states. The pharmacologic characteristics of different drug formulations are explored in the context of their ability to improve patient adherence. The third section focuses on drug-drug interactions.Psychotropic medications from different categories are frequently prescribed together, or alongside medications used to treat comorbid conditions, and the information provided is directly relevant to the clinic, as a result. The clinical application of pharmacokinetics and pharmacodynamics of CNS agents has made significant progress over the past 50 years and new information is reported by numerous publications in psychiatry, neurology, and pharmacology. Our understanding of the interrelationship between these medications, receptors, drug transporters, as well as techniques for measurement and monitoring their interactions, is frequently updated. However, with information presented on a host of different platforms, and in different formats, obtaining the full picture can be difficult. This title aims to collate this information into a single source that can be easily interpreted and applied towards patient care by the clinical practitioner, and act as a reference for all others who have an interest in psychopharmacological agents.

Abstract
This book provides a valuable resource for surgeons and health care providers who care for patients with colorectal disease at all stages of their careers. In line with previous editions, it aims to build upon the collective experience and expertise from national and international experts in the field, providing a completely revamped, up-to-date tome covering the wide breadth of colorectal disease organized around the “pillars” of colorectal surgery including perioperative care (including endoscopy); anorectal disease; benign disease (including inflammatory bowel disease); malignancy; pelvic floor disorders; and a “miscellaneous” section that covers aspects both inside and beyond the operating room that are pertinent to providers at every level. In addition, each chapter contains several Key Concepts that succinctly depict the major learning objectives for individual sections and are in line with the Core Curriculum for Colon and Rectal Surgery provided by Association of Program Directors in Colon and Rectal Surgery and the key topics used by the American Board of Colon and Rectal Surgery. The ASCRS Manual of Colon and Rectal Surgery 3rd Edition serves as a relevant and practical tool to provide information, recommendations, and ultimately help improve the care and outcome for our patients.

Abstract
This third edition text provides a completely revised and updated new version of this unique, modern, practical text that covers the strategic evaluation, specific approaches, and detailed management techniques utilized by expert colorectal surgeons caring for patients with complex problems--whether they result from underlying colorectal disease or from complications arising from previous surgical therapy. The format follows that of both a "how to" manual as well as an algorithm-based guide to allow the reader to understand the thought process behind the proposed treatment strategy. By making use of evidence-based recommendations, each chapter includes not only background information and diagnostic/therapeutic guidelines, but also provides a narrative by the author on his/her operative technical details and perioperative "tips and tricks" that they utilize in the management of these complex surgical challenges. Distinctive to this book, is the reliance on experts in the field including past presidents of the ASCRS, as well as multiple other national and internationally recognized surgeons, to lend their personal insight into situations where data may be more sparse, but individual and collective experience is paramount to making sound decisions and thereby optimizing patient outcomes. The text includes chapters on the assessment of risk and methods utilized to minimize perioperative complications. In addition, it incorporates sections covering the medical and surgical therapies for abdominal, pelvic and anorectal disease. Moreover, the technical challenges of managing complications resulting from the original or subsequent operations is addressed. The underlying focus throughout the text is on providing pragmatic and understandable solutions that can be readily implemented by surgeons of varying experience to successfully treat complex colorectal problems. However, it also goes beyond the technical aspects of colorectal surgery and includes special sections highlighting the essence of a surgeon; covering aspects involving the medical-legal, ethical, and economic challenges confronting surgeons. Throughout the text, each author provides an ongoing narrative of his/her individual surgical techniques along with illustrations and diagrams to "personally" take the reader through the crucial steps of the procedure, and key points of patient care inherent to that topic. Additionally, where appropriate, links to online videos give the reader an up-front look into technical aspects of colorectal surgery.

Abstract
Atlas of Esophagus and Stomach Pathology provides an image-based resource for those studying normal histology of the upper gastrointestinal tract, as well as the microscopic manifestations of developmental abnormalities, toxic insults, infectious diseases, inflammatory and autoimmune conditions, and neoplasia in the esophagus and stomach. Because modern gastrointestinal pathology practice centers on specimens obtained during endoscopic examination, the atlas focuses on biopsy pathology, providing "real-world" microscopic images and ancillary diagnostic studies for most commonly-encountered abnormalities and diseases affecting these two organs. The book is supplemented with endoscopic and special study images. Authored by nationally and internationally recognized pathologists, Atlas of Esophagus and Stomach Pathology is a valuable tool for both pathologists-in-training seeking to make "new acquaintances", and practicing surgical pathologists in need of a quick visual reference in recalling "old friends" in the world of diagnostic gastrointestinal pathology.
