For each procedure, a list of common indications is given, covering 95% of the situations in which particular procedures will be used, as well as a list of essential steps. This is then followed by a list intended to prompt the surgeon for particular details to note and dictate within the template, a list of possible complications that are typically associated with a particular surgical procedure, and templates of operative dictation that allow the surgeon to individualize their dictations. Ample space is also provided to allow each surgeon to add notes. Abstract
Treatment by means of dental implants has become increasingly common, but it is now recognized that cementation during the restorative phase can be the source of significant problems. Nevertheless, few dentists have a clear understanding of why related disease processes arise and there is a similar lack of awareness of the factors to be considered in cement selection, which all too often appears essentially arbitrary. This book examines in detail the issues associated with cementation in dental implantology, with a particular focus on residual excess cement and its consequences. It provides reliable guidance on cement selection and use on the basis of the latest scientific research. Among the topics addressed are microbial aspects of cement selection, new abutment designs, aesthetic considerations, margin placement, and the role of radiography. The relation of peri-implant disease to residual excess cement is explored in depth, and alternatives to the cementation process are also considered. All of the chapters have been written by leading experts in restorative and surgical dental implantology. The information supplied isádesigned to change the way in which the dentist thinks and practices by replacing assumptions and reliance on anecdotes with evidence-based knowledge.

Abstract
This text includes stabilization techniques for the entire spinal column, ranging from the cranio-cervical junction to the pelvis. The information is presented in an easily digestible format that is suitable for those in school or training, yet includes pearls and insight that can be appreciated by even the most seasoned surgeon. The text is divided into major sections based on the anatomical regions of the spine: cervical, thoracic, and lumbosacral. An additional section is devoted to related surgical concepts and principles such as spinal biomechanics, computer image guidance, and deformity surgery. Each chapter has a uniform design including background, indications, patient selection, preoperative considerations, surgical technique, technical pearls, and strategies for complication avoidance. Abstract
This book provides detailed descriptions of fundamental techniques that may be employed for extremity reconstruction and distraction osteogenesis in accordance with the principles established by Gavriil Abramovich Ilizarov. Techniques of proven value for deformity correction, limb lengthening, reconstruction of post-traumatic and post-osteomyelitis bone defects, non-union surgery, and fracture fixation with external fixators are thoroughly described step by step with the aid of a wealth of illustrative material. In addition, indications and preoperative planning are clearly explained. Throughout, care is taken to highlight important technical tips and tricks as well as clinical pearls and pitfalls.
