Abstract
Hospitals worldwide are currently suffering an epidemic of infections, generated in and spreading in those instituitions whose very "raison d'etre" is to protect and improve the health of their patients. This paradox comes at a time of possibly unprecedented investment in preventative measures (infection control). To better control these infections, however, we need to understand their causes, many of which are related, paradoxically, to the success of antibiotic therapy. It is possible that the golden era of antibiotics led subconsciously to a loss of emphasis on the importance of hygiene in the control of HAI. Moreover, the majority of HAIs are due to multiply antibiotic resistant bacteria whose emergence has been hastened by over-enthusiastic antibiotic prescribing. Accumulating evidence also suggests that antibiotics not only select for resistance but can actually increase the numbers of HAIs and even their virulence. This comprehensive compilation, written by eminent international researchers, explores these hypothesis in the context of recent epidemiology and evidence for the success of various antibiotic stewardship measures in controlling and even reversing resistance. It contains chapters by acclaimed authors on all the major causes of HAI, including MRSA, VRE, Clostridium difficile and multi resistant gram negative infections which are increasingly pan-resistant and untreatable; the nearest thing we have yet experienced to a "Doomsday bug". This book is an excellent source of information for students, researchers and practitioners in infection control, hospital epidemiology, antimicrobial prescribing and microbiology.

Focusing exclusively on the older patient with poor bone quality, this unique book presents the indications, contraindications and common techniques, as well as the risks, benefits and outcomes—for utilizing arthroplasty for the treatment of fractures in this population, sensibly divided into four thematic sections. The incidence and burden of fragility fractures is presented in part one, with considerations of the influences of osteoporosis on both treatment and healing. The subsequent three sections cover upper extremity fractures, lower extremity fractures, and peri-articular nonunions, respectively. However, far from providing a simple "how-to" for the techniques discussed, the relevant current literature is presented as well as the common techniques employed in fracture management, allowing the reader to select the best approach for the specific patient presentation. In addition, chapters are included here that cover degenerative joint conditions not typically treated with total joint replacement. The treatment of fragility fractures is constantly evolving, and the aging population is consistently expanding, creating a strong need for clinicians who have experience with and exposure to the use of arthroplasty techniques as an option in their successful treatment. Abstract
This book systematically focuses on central sleep apneas, analyzing their relationship especially with heart failure and discussing recent research results and emerging treatment strategies based on feedback modulation. The opening chapters present historical background information on Cheyne-Stokes respiration (CSR), clarify terminology, and explain the mechanics and chemistry of respiration. Following a description of the physiology of respiration, the pathophysiology underlying central apneas in different disorders and particularly in heart failure is discussed. The similarities and differences of obstructive and central apneas are then considered. The book looks beyond the concept of sleep apnea to daytime CSR and periodic breathing during effort and contrasts the opposing views of CSR as a compensatory phenomenon or as detrimental to the failing heart. The diagnostic tools currently in use for the detection of CSR are thoroughly reviewed, with guidance on interpretation of findings. The book concludes by describing the various forms of treatment that are available for CSR and by explaining how to select patients for treatment.

Abstract
The availability and diffusion of high-performance technologies has strengthened the role of CT angiography and MR angiography as simple, reliable techniques for the characterization and treatment planning of the principal diseases of the cardiovascular system, an understanding of which has become essential for all radiologists. The aim of this book is to provide concise but thorough information and guidance on the main methods and examination techniques used when performing high-quality CT angiography and MR angiography studies in a broad range of clinical settings. The lucid text is supported by a large number of case studies and is rich in advice on image interpretation and practical suggestions for the evaluation and reporting of the examinations. This volume presents a comprehensive overview of childhood trauma, considering the psychopathological definition and its neurobiological implications as well as its impact on different psychiatric disorders. The focus on childhood trauma rather than that occurring in adulthood is important due to its general “neuro-psyco-socio” and its specific biological implications, since trauma during childhood impacts directly on neurodevelopment. It has been suggested that early life stress increases vulnerability to psychiatric disorders; however, the exact mechanisms of this association are not yet completely understood. Although childhood trauma could be considered too unspecific to be an important risk factor for individual psychiatric disorders since it seems to occur across the board, it impacts differently on different psychiatric disorders, and it can modulate their clinical expression. Therefore, the assessment of early trauma needs to be included in the clinical evaluation of patients with psychiatric disorders. The volume will be an invaluable tool for psychiatrists, helping them to select suitable pharmacological, psychotherapeutic and rehabilitative treatments.

The opening chapters describe the biological principles of current accelerated techniques at the molecular and cellular levels and introduce guidelines on how to select the best acceleration approach based on each patient's needs. Clinicians are then guided step by step through the application of MOPs, case selection, and treatment planning. It is explained how MOPs can be incorporated into daily mechanotherapy for the treatment of different malocclusions and how to take advantage of the catabolic and anabolic effects of the procedure to expand the boundary of orthodontic and orthopaedic corrections.
