Calipered kinematic alignment was proposed by Dr. Stephen M. Howell to enable the accurate and individualized placement of total knee components coincident within ± 0.5 mm of the patient’s pre-arthritic alignment. The aim was to restore the patient's native joint kinematics and thereby address the well documented 20% dissatisfaction when total knee replacement is performed using traditional, or mechanical alignment techniques.

Abstract
This book adopts a novel approach: procedures. Instead of discussing the diagnostic categories of neurological syndromes, it focuses on the symptoms of common neurological presentations (especially in the emergency room or on the ward), and the diagnostic hypotheses that can be validated or rejected case by case as a result. Each chapter covers one of the main symptoms of emergencies in neurology – from transient consciousness disturbances to focal deficits, acute muscular disorders, respiratory insufficiency in neurological disorders, headaches, delirium, seizures and epileptic status, para-and tetraplegia, and head trauma, to acute functional disorders – and includes tables and figures to allow readers to gain a quick and easy, yet comprehensive overview of the topic. The book guides readers through various scenarios generated by the onset of symptoms, the clinical tools for differential diagnostics, and the principles of acute and post-acute phase therapy, managed by decision algorithms supported by the most recent scientific evidence. The resulting precedural profile – created through the collaboration of over 80 specialists in neurology or other disciplines – makes this text a valuable tool for neurologists, neurology residents and allied professionals in their daily clinical practice, as well as medical students.

Abstract
Neuroendocrine neoplasms comprise a large family of proliferative lesions that involve almost every part of the body. Our understanding of their cells of origin as well as the pathology, pathophysiology and genetics of these neoplasms has made tremendous advances in the last few decades. While they are often discussed as separate entities in textbooks of gastroenteropancreatic pathology and pulmonary pathology, their scope is much broader. The book conveys the similarities and differences of these fascinating tumors that may be found from the hypothalamus and pituitary to the rectum, and in soft tissue as well as in many organs. Written by experts in the field, the authors emphasize their structural, functional, predictive and prognostic features and attempt to provide the clinical context that allows improved diagnosis and therapy while building on the genetics that clarifies patterns of inheritance and predisposition to tumor development through precursor lesions. The Spectrum of Neuroendocrine Neoplasia provides a broad overview of neuroendocrine neoplasms using a practical approach to diagnosis, histological classification and therapy, and presents the most important and significant developments of the technologies used to diagnose, classify and treat them.

Abstract
This ambitious and comprehensive manual represents an essential contribution to our current understanding of interactions between heart and brain, a research topic generating growing interest. Despite the increasing awareness that neural mechanisms are the primary cause of cardiac disease and its progression, therapy continues to focus on end-organ protection and does not approach the neural core of the problem. Growing public health problems such as heart failure are still treated with autonomic drugs that are 30-40 years old and simply act on cardiac receptors. However, it has now been shown that the progression of ischemic heart disease to heart failure is mainly due to abnormal central responses to incipient cardiac disease, with neural activation the primary cause rather than the consequence of cardiac remodeling. Written by leading international experts in their respective research areas, it presents a variety of perspectives on the core topic: from social and philosophical to gender-related aspects. Designed for a broad readership, it includes dedicated sections for cardiologists, psychiatrists, neurologists and psychotherapists looking for a more insightful and targeted approach to neuro-cardiovascular disease.

Abstract
This atlas is a concise but comprehensive guide to the diverse patterns of response to immunotherapy as observed on Positron Emission Tomography/Computed Tomography (PET/CT) and other conventional imaging modalities, including CT and Magnetic Resonance Imaging (MRI). The purpose for this publication is to fill the gap between the growing clinical relevance and utilization of immunotherapy in medical oncology, mainly based on checkpoint inhibitors, and the need for experienced imagers with reliable tools assessing response to treatment. A series of disease-oriented chapters will present the imaging findings during immunotherapy in the major oncological settings, with helpful comparison of functional (PET/CT) and morphological (CT/MRI) patterns of response in individual cases. To complete the atlas, a dedicated chapter will focus on major pitfalls and immune-related adverse events (irAEs) affecting image interpretation during the course of immunotherapy. The concluding chapter will lastly examine the available data and potential developments of immuno-PET, which is considered as the novel frontier of research in this oncological scenario. The atlas will be of high value for radiologists and nuclear medicine specialists at all levels of experience.

Abstract
This book highlights the importance of investigating for ADHD in adults with attention difficulties, poor memory and executive function impairments. The authors advocate a neurodevelopmental assessment approach in all phases of life, and explain how to perform such assessments. The identification of ADHD in adults with other psychiatric and neurological disorders will lead to a better response to treatments, and as a result reduce its social, economic and personal burden. ADHD can no longer be considered solely a pediatric condition, as it occurs in a significant proportion of adults worldwide. However, ADHD in adults is often unrecognized and untreated. Diagnosing the disorder in adulthood is a challenge, due not only to the different clinical presentation in this phase of life, but also to the fact that other conditions may mask the symptoms. ADHD constitutes a hidden comorbidity with a detrimental impact of those affected, and a poor response (or even non-response) to several treatments. Intended for psychiatrists, neurologists, specialists in gerontology and neuropsychology, this book is an indispensable resource for all mental health practitioners who want to optimize the treatment of patients affected by psychiatric and neurological disorders who respond poorly to standard treatments.

Abstract
This excellently illustrated book adopts an evidence-based approach to evaluate the efficacy of different techniques for the imaging and treatment of patellofemoral pain, instability, and arthritis. The aim is to equip practitioners with an informative guide that will help them to manage disorders of the patellofemoral joint by casting light on the many issues on which a consensus has been lacking. The opening chapters supply essential background information and explain the role of various imaging modalities, including radiography, CT, MRI, and bone scan. The various conservative and surgical treatment approaches for each of the three presentations – pain, instability, and arthritis – are then described and assessed in depth, with precise guidance on indications and technique. Postoperative management and options in the event of failed surgery are also evaluated. Throughout, careful attention is paid to the literature in an attempt to establish the level of evidence for each imaging and treatment method. The new edition has been thoroughly updated, with inclusion of additional chapters, in order to present the latest knowledge on biomechanics, diagnosis, surgical techniques, and rehabilitation.

Abstract
The book examines an integrated approach for addressing decisions about the location of healthcare facilities. Supported by Geographic Information Systems (GIS) and Multi-Criteria Decision Analysis (MCDA), the approach provides comprehensive information on territory, taking into account the spatial dimensions. Due to the multiple criteria involved, site selection for urban facilities is a crucial topic in planning decision processes, especially for healthcare facilities. Healthcare provision policies generally fail to address the distribution of facilities within cities, entrusting decisions to various stakeholders. Moreover current evaluation tools focus on the intrinsic performances of healthcare structures, disregarding the extrinsic characteristics, namely those related to the location. Starting with a cross-disciplinary literature review, the book describes a multi-methodological approach for decision-making regarding the location of healthcare facilities, and presents an innovative evaluation tool that simultaneously considers functional, locational, environmental and economic issues, providing a comprehensive overview of the areas under investigation.
Contents
1. The Location Problem. Addressing Decisions About Healthcare Facilities -- 2. Structuring the Decision Problem. A Spatial Multi-methodological Approach -- 3. Approaching the Location of Healthcare Facilities: How to Model the Decision Problem -- 4. Modelling the Spatial Decision Problem. Bridging the Gap Between Theory and Practice: SitHealth Evaluation Tool -- 5. Policy Implications. How to Support Decision-Makers in Setting and Solving Complex Problems.

Abstract
This book presents the state of the art in atrophic rhinitis and analyses its specific characteristics in non-allergic rhinitis. Little has been written on this condition, which represents a topical issue in modern clinical practice, and as such many clinicians tend to underestimate its impact on patients’ quality of life. This book covers the definition, aetiology, clinical presentation, diagnosis and management, providing specialists with a better understanding of this condition and of the treatment options. It describes in detail the latest diagnostic tools, such as the innovative computational fluid-dynamics studies, nasal cytology, trigeminal stimulation tests, olfactometry, subjective questionnaires, with a focus on the psychological aspects, which have not yet been systematically investigated. Further, it discusses therapeutic strategies, both medical and surgical, reviewing the literature, from the first historical procedures to recent conservative techniques, such as lateral nasal wall reconstruction with various materials, and septal perforation repair. Illustrating all the options currently available, the book is an invaluable resource, not only for otolaryngologists, but also for plastic surgeons, clinical allergists and rheumatologists.

Abstract
This book provides a comprehensive overview of the importance of molecular imaging in multiple myeloma, with detailed explanation of its clinical impact. Other important features are the definition of criteria that will aid PET/CT interpretation; identification and explanation of the most frequent pitfalls; a brief overview of the advantages and limitations of DWI MR imaging, still an experimental technique in multiple myeloma; and examination of the possible role of emerging PET tracers. When appropriate, clinical cases are used to illustrate key teaching points. All physicians involved in oncological imaging should regularly reassess and update their routine practice in the evaluation of multiple myeloma patients. This is especially true now, given the recent clarification by the International Myeloma Working Group (IMWG) of the criteria for bone damage requiring therapy and the emerging data supporting the role of the newer functional imaging techniques in predicting outcome and/or evaluating response to therapy. In this challenging context, Molecular Imaging in Multiple Myeloma will be of high value for nuclear medicine physicians, radiologists, and hematologists.

Abstract
This book clearly explains when and how different rehabilitation techniques should be applied in the aging patient, thereby enabling readers to identify and apply those rehabilitation strategies that will maximize quality of life and functional independence in individual cases. It is specifically designed for ease of consultation and rapid retrieval of the information most relevant to clinical practice. Prominence is given to the benefits of a multidisciplinary approach to rehabilitation, with discussion of a very wide range of aspects of rehabilitation in different disease settings. The breadth of coverage is illustrated by the attention paid to less commonly addressed topics such as visual and hearing rehabilitation, the role of robotics and 3D imaging techniques, variations in approach among health care systems, and rehabilitation in end-of-life care. The authors are international academic experts in their fields, guaranteeing a high scientific standard throughout. This manual will be an invaluable tool and source of knowledge for geriatricians and physiatrists but will also appeal to a wider range of clinicians, practitioners, and students.

Abstract
This structured dynamic book outlines, step by step, an evidence-based systematic approach to the sonographic evaluation of the pelvis in women with suspected endometriosis. This "how to" guide is intended for those with basic ultrasonography skills who want to further develop their capabilities in performing the relevant sonographic techniques to identify endometriosis. Detailed schematics, and corresponding high-resolution ultrasound images and intuitive videos support readers in expanding their technical skills and bridging the gaps in their knowledge of endometriosis ultrasound. The International Deep Endometriosis Analysis (IDEA) group consensus statement was the culmination of the work of 29 authors from 5 continents. With the publication of How to Perform Ultrasonography in Endometriosis the authors intend to provide the basis for quality improvement and benchmarking of ultrasound in the world of endometriosis. This book not only offers sonologists, radiologists and sonographers valuable insights into the field of endometriosis ultrasound, but also enables them to develop their practical skills in assessing women with chronic pelvic pain.

Abstract
This book provides essential insights into how the approach to nursing care in ICU patients has markedly changed over recent years. It shows how the focus has progressively moved away from the technical approach that characterized early ICUs to a wider personalization of patient care that also highlights general problems such as basic hygiene and general comfort. It also demonstrates that, at the same time, the nurses' role has become more professionalized, with increasing competences in assessing and managing patients' problems and measuring related outcomes. It is structured in four units: Unit 1 presents the essential elements of accurate vital-function and basic-needs assessments for ICU patients, using both instrumental monitoring and specially validated assessment tools. Unit 2 addresses basic care in ICU patients, particularly hygiene and mobilization, reflecting recent developments in nursing that focus on the importance of these activities. Unit 3 highlights the main nursing outcomes in ICU patients, particularly focusing on risk prevention and complication management. Lastly, Unit 4 discusses advances in ICU nursing, from clinical, organizational and research perspectives.

Abstract
This book presents state of the art knowledge and practice in the rapidly developing field of bronchiectasis not due to cystic fibrosis (referred to below simply as bronchiectasis). The focus is especially on diagnosis and existing and emerging therapies, but the book also covers a wide range of other key topics, from pathophysiology, histopathology, and immunology through to pulmonary rehabilitation, nursing care, and management in primary care and pediatric settings. While bronchiectasis was formerly regarded as an "orphan" disease, international data reveal an increase in its prevalence in recent years. Accordingly, there has been renewed interest in the disease, resulting in more clinical research and the development of new treatments. The impact of bronchiectasis on healthcare systems is substantial and it has a clear attributable mortality. In covering all aspects of the disease, this book will be of interest to respiratory, internal medicine, and infectious disease fellows and specialists, final-year medical students, and nurses and physiotherapists. The authors are leading experts and chairs of the steering committee of EMBARC, the first truly international bronchiectasis network.

Abstract
This book presents up-to-date knowledge on infertility in the context of polycystic ovary syndrome (PCOS) and provides clear evidence-based guidance on its treatment. The book opens by discussing anovulation, oocyte quality, and the endometrium in women with PCOS, infertility and subfertility cofactors, and the impact of PCOS phenotypes on fertility. All aspects of management are then thoroughly addressed. The available medical treatments for PCOS-related infertility—including antiestrogens, aromatase inhibitors, insulin-sensitizing drugs, and gonadotropins—are reviewed, and other potential therapeutic approaches, such as acupuncture and laparoscopic ovarian drilling, are assessed. Careful attention is also devoted to the role of lifestyle interventions. The use of controlled ovarian stimulation in infertile PCOS patients undergoing intrauterine insemination or in vitro fertilization is examined in detail, as are the benefits of in vitro maturation of oocytes. This book will be of value to all who are involved in the care of women with PCOS and related infertility issues.
