Abstract
This book provides a clear framework for conducting participatory research with children and young people supported with practical examples from international research studies. Our aim is to encourage more participatory research with children and young people on all matters that affect their lives. This book illustrates innovative ways of being participatory and sheds new light on involvement strategies that play to children's and young people's competencies. Participatory research is based on the recognition of children and young people as active contributors rather than objects of research. Participatory researchers support and value the voices of children and young people in all matters that concern them. Core to participatory research practice is a strengths-based approach that aims to promote the active engagement of children and young people in all stages of research, from inception to implementation. Engagement of children and young people requires the use of creative, participatory methods, tools and involvement strategies to reveal children's competencies. This book shares knowledge about creative participatory techniques that can enable and promote children's ways of expressing their views and experiences. The book provides guidance on appropriate techniques that reduce the power differential in the adult-child relationship and which optimise children's abilities to participate in research. This book is targeted at researchers, academics, practitioners who need guidance on what tools are available, how the tools can be used, advantages and challenges, and how best to involve children in all stages of a research project. It will provide several examples of how children can have an active participatory role in research. There is increasing interest in involving children as co-researchers but little guidance on how this can be done. This book fills a gap in the current books by addressing all of these issues outlined above and by providing worked examples from leading researchers and academics. It will have wide appeal across a range of different disciplines.
Contents

Being participatory through the use of app-based research tools -- 9. Participatory research: Does it genuinely extend the sphere of children and young people's participation?

Abstract
This book proposes and investigates a universal framework, and accompanying documentation system, to facilitate and catalogue benefit-risk decisions; a valuable addition to the benefit-risk toolbox. Over the past decade, pharmaceutical companies and regulatory agencies have been reviewing the benefit-risk assessment of medicines with a view to developing a structured, systematic, standardized approach. Examining the evaluation of such an approach by several mature regulatory authorities ensures that the reader gains a unique insight into the ongoing debate in this area. The field of benefit-risk assessment continues to evolve at a rapid pace due to political and societal pressure, as is reflected in the recent FDA PUDFA agreement as well as in the EMA 2015 Roadmap. Rather than provide a comprehensive snap-shot of this constantly changing environment, this book evaluates selected current approaches to benefit-risk assessment. The strengths and weaknesses of publicly available documents in communicating benefit-risk decisions to stakeholders are reviewed, and these evaluations are used to inform development of a prospective framework that could be used to harmonise procedures globally.
Contents
Overview -- Approaches to utilising decision-making framework -- Benefit-risk assessment of medicines by pharmaceutical companies and regulatory authorities -- Development of a universal benefit-risk framework and template -- Implementation of the benefit-risk assessment template by mature agencies -- Implementation of the benefit- risk summary template by a maturing agency: a case study -- Communicating benefit-risk decisions by US FDA, EMA, TGA and Health Canada -- Conclusions and future directions.

Abstract
This book examines all aspects of the progress being made towards the development of highly specific and sensitive biomarkers that will overcome the shortcomings of clinical assessments in periodontics. The opening chapters present the basic anatomic features of periodontal tissues, outline the nature and pathogenic mechanisms of periodontal diseases, and discuss both conventional and novel methods of diagnosis with reference to their specificity and sensitivity. The potential role of biomarkers in periodontal diagnosis is then presented in the light of the published data. The biological samples in which potential biomarkers are sought are evaluated comparatively, drawing attention to their strengths and weaknesses, and the available technologies for biomarker studies are reviewed. The potential benefits of biomarkers are also discussed with regard to the possible bidirectional interactions between periodontal diseases and systemic health. Finally, the past, present, and future of periodontics are examined from a broader perspective. Readers will find the book to be an ideal summary of the state of the art in the field as biomarkers emerge that promise to facilitate periodontal diagnostics and permit timely, personally tailored interventions.

Abstract
This book takes a fresh look at how caring for Medicare beneficiaries can lead to success in today's healthcare marketplace. Using examples from highly successful practice models, the book will take the reader through the key components necessary to run a profitable geriatric practice. The author, who co-founded what became one of the largest geriatric private practices in the country, discusses each part of the continuum of care from the perspective of the provider-client relationship, highlighting the importance of business cultures in this environment. The text also features an overview of payment models, starting with traditional fee-for-service, insurance (Medicare HMOs), and finishing with the concept of bundled payment models that have been heralded as the future of physician reimbursement. Finally, this book focuses on the practical aspects of business planning, budgeting and hiring and concludes with chapters about the key challenges faced by a geriatric practice, focusing on provider productivity, coding and billing, and dealing with potential competition in the marketplace. Abstract
Carcinomas of the larynx and hypopharynx present unique challenges when it comes to their assessment and management. Management involves the essential dilemma of optimizing cure rates while also ensuring that potential disability due to compromised airway, voice and swallowing is minimized. Treatment decisions involve careful disease assessment, judicious multispecialty decision-making, and subsequent meticulous implementation of the selected treatment modalities. The critical nature of each of these steps has led to dramatic advances in assessment techniques (radiology, endoscopy and photography, stroboscopy, narrow band imaging, contact endoscopy) and also in treatment methods (radiation therapy, combined concurrent chemo-radiation, combined sequential chemo-radiation, trans-oral CO2 laser and robotic surgery, partial laryngectomy, total laryngectomy, extended total laryngectomy). As such, treatment decisions have become increasingly complex with the twin objectives of maximizing cure as well as function preservation and are based on tumor stage, tumor location, systemic co-morbidities, nutritional status, patient priorities, and social and economic factors. This multidisciplinary book has been written by a team of editors with considerable experience and expertise in these critical areas and a deep understanding of teamwork and the strengths and limitations of individual technologies and treatments. It presents a homogeneous and unbiased view that is applicable to all specialties involved in the care of larynx and hypopharynx cancers.

Abstract
This book aims to provide an overview of the latest theoretical insights from research on sociocultural aspects of mental health and connect these with clinical insights from practical mental health care provision. Using strengths-based, resiliency-oriented and family-centered approaches can enrich clinical practice in refugee mental health, but clinicians need to translate the emerging evidence into concrete steps and interventions. This requires additional skills for the assessment and management of mental health conditions in refugee children and families. The chapters in this book are written by a diverse group of authors using global, multi-disciplinary approaches. The chapters provide examples from various contexts including refugees who are displaced to neighboring countries, refugees ‘on the move’, and refugees and asylum seekers in resettlement settings. This book is therefore a unique resource for clinicians, researchers and policy makers working on mental health issues of refugee children and adolescents around the world.

Abstract
Adolescent homelessness is a growing problem that results in a variety of health challenges. This text is a practical resource designed to promote effective interdisciplinary health and social care interventions targeting adolescents who are homeless or at risk for homelessness. It is based on extensive interdisciplinary experience, reviews of pertinent research and insights and contributions of leading professionals who are directly involved in the care of these young people. Divided into four main sections, Section 1: (Chapters 1-7) section one is a review of the structure and professional involvement of program models targeting youth experiencing or at risk for homelessness to encourage broader understanding and utilization of principles and practices underlying effective programs and identify replicable components. Section 2: (Chapters 8-16) Section two is clinically focused with recommendations for working with adolescents and youth experiencing homelessness and interventions for common and significant medical and mental health conditions, and substance use disorders. Section 3: (Chapter 17) Reviews international agreements regarding stabilization and care of refugee youth and families, description of experiences of refugee children and youth in developed countries, and an outline of conditions from which refugee youth and families have left. Abstract
This book covers all aspects of low field MRI, describing its advantages, problems, and prerequisites. Individual chapters are devoted to site planning, safety considerations, coils, imaging technique, image quality optimization, the imaging of different anatomic regions, and likely future developments. The factors that must be borne in mind when selecting a low field system are clearly identified, and detailed attention is paid to the applications for which such a system is adequate. The focus on high field systems has led to a situation where only a few systems with field strengths lower than 0.5 T survive. Some of these systems possess high field features such as multichannel coils and strong gradients; furthermore, sequence technology and image processing techniques taken from higher field strength systems have resulted in impressive imaging capabilities. While 1.5-T systems will probably continue to remain the standard, low field systems offer advantages such as the feasibility of dynamic joint examinations, improvement of T1 contrast, reduction of "missile effects", and decreased radiofrequency exposure. Low field strength MRI consequently has the potential to contribute to optimal patient management and, given comparable image quality, its application may become an issue of patient safety. This book will be an invaluable asset to all who are involved in planning and/or running a low field strength MRI facility.

Abstract
This is a book about primary care clinicians and the clinical uncertainty endemic to their work. Even when seemingly straightforward, each patient raises unique questions regarding how best to listen to their complaints, empathize with their suffering, or respond to their silences. This book is also about addressing uncertainty in primary care practice and engaging it. Engagement requires knowledge, explicit and tacit, placed in the service of a single patient's problem. It also requires carefully managed communication, facilitating dialogue with the patient and encouraging shared problem-solving. Most importantly, this book is about collaborative engagement with case-based uncertainty in the setting of small groups of clinicians. Sommers and Launer contend that the medical profession's tradition of working independently should be augmented with an explicitly shared, collegial one of jointly creating wisdom through practice-based learning. An international panel of expert clinicians and educators provides: Perspectives on clinical uncertainty in the medical literature; A taxonomy of clinical uncertainty with patient examples; Analysis of the educator role to support clinicians in engaging uncertainty; A compendium of small group methods for collaborative engagement with clinical scenarios; Analysis of the special challenges of collaborative engagement. A mind-opening manifesto, Clinical Uncertainty in Primary Care will equip primary care clinicians, educators, public health and behavioral health professionals with resources for infusing practice with meaning through collegial collaboration. From the Foreword: "Lucia Sommers and John Launer, with the accompanying input of their contributing authors, have done a deeply insightful and close to exhaustive job of defining clinical uncertainty. They identify its origins, components, and subtypes; demonstrate the ways in which and the extent to which it is intrinsic to medicine--and present a cogent case for its special relationship to primary care practice--Clinical Uncertainty in Primary Care not only presents a model of collegial collaboration and support, it also implicitly legitimates it." Renee Fox, Annenberg Professor Emerita of the Social Sciences, University of Pennsylvania.

Abstract
Cognitive Screening Instruments: A Practical Approach provides a practical and structured overview of some of the most commonly used and easily available cognitive screening instruments applicable in the outpatient clinic and bedside setting. Dementia and cognitive disorders are now recognised as an increasing public health problem, both in terms of patient numbers and cost, as populations age throughout the world. Despite this, many patients with dementia never receive a formal diagnosis, with implications for their appropriate care and management. Diagnostic tests which identify cases of dementia therefore have an important role. Expert authors from around the world equip the reader of Cognitive Screening Instruments: A Practical Approach with clear instructions on the usage of each screening instrument, its strengths and weaknesses, the time required for administration, and rules on scoring, such as how to correct for variations in the patient's age or education, and suggested cut-off scores. Cognitive Screening Instruments: A Practical Approach is a handy, illustrated guide and a valuable diagnostic aid for practitioners working closely with patients with dementia and mild cognitive impairment. This volume will be of use both to clinicians and to professionals in disciplines allied to medicine who are called upon to assess patients with possible cognitive disorders, including neurologists, old age psychiatrists, neuropsychologists, primary care physicians, dementia support workers, and members of memory assessment teams.
