Abstract
This open access book offers essential information on values-based practice (VBP): the clinical skills involved, teamwork and person-centered care, links between values and evidence, and the importance of partnerships in shared decision-making. Different cultures have different values; for example, partnership in decision-making looks very different, from the highly individualized perspective of European and North American cultures to the collective and family-oriented perspectives common in South East Asia. In turn, African cultures offer yet another perspective, one that falls between these two extremes (called batho pele). The book will benefit everyone concerned with the practical challenges of delivering mental health services. Accordingly, all contributions are developed on the basis of case vignettes, and cover a range of situations in which values underlie tensions or uncertainties regarding how to proceed in clinical practice. Examples include the patient’s autonomy and best interest, the physician’s commitment to establishing high standards of clinical governance, clinical versus community best interest, institutional versus clinical interests, patients insisting on medically unsound but legal treatments etc. Thus far, VBP publications have mainly dealt with clinical scenarios involving individual values (of clinicians and patients). Our objective with this book is to develop a model of VBP that is culturally much broader in scope. As such, it offers a vital resource for mental health stakeholders in an increasingly inter-connected world. It also offers opportunities for cross-learning in values-based practice between cultures with very different clinical care traditions.

Part 1 begins with a review of the human response to infection including key definitions and general concepts important in the field of vaccinology. Next, detailed chapters on currently available strategies that provide passive and active immune protection against infectious diseases are presented, including descriptions for how these biologic products are produced. Given the complexity of the manufacturing processes used in vaccine production and the frequency with which questions arise regarding the presence of various additives and excipients found in vaccines, a full chapter has been dedicated to the topic. Part 1 concludes with a chapter outlining the regulatory processes and timelines needed for the development and testing of new vaccine candidates. Part 2 of the book includes separate chapters on each of the 35 infections that are currently vaccine preventable. The chapters are presented in alphabetical order according to disease. The reader will find that certain terms and concepts important to the study of vaccines appear in many or even all of the chapters. Familiarity with the definition of several of these terms as they are used in vaccinology and in human clinical vaccine trials is key to developing an authoritative understanding of the field so they are introduced first here.

Abstract
This book presents rational diagnostic approaches to common areas of cytopathology, such as thyroid, lymph node and effusion cytology. It discusses differential diagnoses for each site, and describes a systematic approach to narrow down differential diagnoses in a logical manner based on cytomorphology and the judicious application of ancillary tests. Further, it introduces discriminatory panels of immunohistochemical tests, with an emphasis on patient-centred approaches with active clinicopathologic correlations. In addition, it provides practical recommendations for optimizing tissue triage for ancillary testing, in terms of both diagnostic and therapy-related testing. The book also includes sample diagnostic reports to help readers formulate appropriate comments and to aid clinicians in specific clinical scenarios, as well as test cases for readers to apply their diagnostic and specimen triage algorithms. The book equips readers to apply logical approaches to sound cytopathology reporting in daily clinical practice, guiding them through specimen collection and triage to diagnostic workup based on morphologic and clinical features, and writing rational and clinically useful diagnostic cytology reports with a focus on clinicopathologic correlation. As such it is relevant for practising cytopathologists and pathology trainees as well as for cytologists (cytotechnologists) and other clinicians involved in cytopathology diagnostic processes.

Abstract

ILM peeling, ILM insertion, and ILM flap techniques are explained, and detailed descriptions are provided of the ILM surgical procedures currently performed in conditions such as macular hole, epiretinal membrane, diabetic macular edema, myopic tractional maculopathy, retinal detachment, and optic pit maculopathy. In addition, a chapter is devoted to the postoperative anatomical and functional changes to the macula after ILM surgery in order to help readers both to understand the mechanism of metamorphopsia and to reduce this symptom. The human hypothalamus can be astutely referred to as the crossroad of endocrinology, psychiatry, neurology and neurosurgery. Because of its involvement in myriad physiologic functions and the varied ways disorders involving it can manifest, hypothalamic disease can initially come to medical attention in widely disparate settings and with widely different clinicians. Therefore, the detection and proper care of hypothalamic dysfunction and disease often requires carefully coordinated multidisciplinary care. This volume fills a significant void in the medical professional community, comprehensively presenting the scope of hypothalamic structure, function, dysfunction and disease to cater to the various clinical, teaching and research professionals that have a stake in this part of the human brain. This open access book marks the first historical overview of the autism rights branch of the neurodiversity movement, describing the activities and rationales of key leaders in their own words since it organized into a unique community in 1992. Sandwiched by editorial chapters that include critical analysis, the book contains 19 chapters by 21 authors about the forming of the autistic community and neurodiversity movement, progress in their influence on the broader autism community and field, and their possible threshold of the advocacy establishment. The actions covered are legendary in the autistic community, including manifestos such as “Don’t Mourn for Us”, mailing lists, websites or webpages, conferences, issue campaigns, academic project and journal, a book, and advisory roles. These actions have shifted the landscape toward viewing autism in social terms of human rights and identity to accept, rather than as a medical collection of deficits and symptoms to cure.

Abstract
This book illustrates the characteristics of imaging after treatment in brain gliomas. It describes in detail the modifications to brain tissue, both healthy and pathological, that can manifest after surgery, radiotherapy or chemotherapy treatment. These modifications are discussed in terms of both how they occur in the immediate post-treatment period, and in the long term. The imaging methods used include CT with and without the addition of contrast medium, but above all MRI, which involves the use of routine basic sequences and mainly advanced study techniques such as diffusion, perfusion, spectroscopy and cortical activation. The aim of the text is to equip neuroradiologists with adequate expertise in post-treatment examinations reporting, allowing them to perform an effective differential diagnosis between the persistency or recurrence of illness and the effects of short or long-term treatment. The book is divided into a general section, which addresses the classification of cerebral tumors, the surgical treatment options, radiotherapy and chemotherapy protocols, and a section on clinical cases that employs rich iconography, making it quick and easy to consult. This second edition has been updated to reflect the new WHO classification system from 2016, new surgical, radiotherapy and chemotherapeutic treatment options, and (in the iconography section) the new sequences available from the manufacturers of RM scanners.
