Abstract
This book describes the surgical bariatric procedures most frequently performed worldwide and examines their evolution in recent years both within Italy and internationally. For each operation, indications, the surgical technique, potential complications, and the outcomes with respect to weight and obesity-associated comorbidities are presented. In view of the significant failure rate revealed by studies on the long-term results of bariatric surgery, the problem of weight regain and revision surgery are also discussed in detail, covering the different types of revision, conversion to other procedures, and the main outcomes. In addition, individual chapters focus on selected topics of importance. The role of bariatric surgery in the cure of type 2 diabetes ("diabetes surgery") is discussed and the debate over the significance of gastroesophageal reflux disease and hiatal hernia for choice of procedure is summarized. Finally, the most common endoluminal procedures, which have been gaining in importance, are described and other bariatric operations, outlined.

To date, diet programs and medical therapies for the treatment of obesity have had limited success. Bariatric surgery, however, provides a means of effective weight loss for many of those with morbid obesity. Most of these weight loss procedures are performed with a variety of techniques that continue to evolve. Each technique is associated with unique challenges and complications and it is important for the clinician to be knowledgeable about the endoscopic management of these patients. Additionally, as endoscopic technology evolves it may offer more than just the diagnosis and treatment of complications. Endoscopic therapy may soon allow less invasive bariatric revision procedures as well as a variety of primary obesity therapies for various patient populations. Bariatric Endoscopy reviews the management of obesity, normal post-surgical anatomy, endoscopic and medical management of post-surgical complications, and future endoscopic therapies for obesity management. Abstract
The present book intends to provide a comprehensive guide on the field of robotic bariatric surgery. It covers all the stages to fulfill credentialing for performing robotic surgery, from an introduction to the robot device until comprehensive descriptions of surgical procedures. Also, robotic surgery is presented as an institutional program and we describe how to establish a robotic program in a hospital environment. The currently accepted and most common procedures – sleeve gastrectomy, gastric bypass and duodenal switch – are described in detail, with a step-by-step description of the techniques, followed by a wealth of photos and videos for each case. Special attention is given to the employment of robotic bariatric surgery in exceptional conditions, such as in super-obese patients, reoperations and revisional procedures. Critical issues, for the success of the robotic surgical interventions, such as anesthesia, are also addressed. Finally, the outcomes of robotic bariatric surgery are described, including long-term weight loss, improvement and resolution of comorbidities and improvement in quality of life. Bariatric Robotic Surgery is the first book specially devoted to this modality of surgical intervention. It is a fundamental tool for surgeons, residents and fellows who want to start a robotic bariatric surgery program. Abstract
The increasing prevalence of morbid obesity has led the World Health Organization to coin the descriptive term "globesity" to reflect the worldwide nature of the problem. Providing health care to these patients, especially when surgery is required, can be extremely challenging owing to the specific needs in respect of logistics, facilities, and professional expertise. Appropriate care has too often been unachievable and unaffordable outside of established bariatric centers, but such centers themselves usually have insufficient capacity and resources to cope with growing demand. This book aims to provide guidance and helpful tips and tricks on how to deal with obese patients within a general surgery setting. Epidemiology, organizational and logistical aspects, nursing issues, patient assessment, anesthesiology, and surgical practicalities are expertly covered in the opening chapters. Techniques of relevance to the general surgeon are described according to anatomic region, covering the head and neck; cardiothoracic and vascular system; upper and lower GI tract; pancreas, liver and adrenal glands; urinary tract and kidneys; the reproductive system; and the abdominal wall. Results achieved by bariatric surgery worldwide are reviewed, and the book closes with a chapter devoted to plastic and reconstructive surgery. The Globesity Challenge to General Surgery highlights a need for global rethinking on public health as regards resource allocation and patterns and standards of care, improving outcomes through greater affordability.

Intragastric balloons are now considered one of the safest and most effective medical procedures for weight reduction. Additionally, the reduced potential for side effects and low invasiveness (compared to bariatric surgeries) are important considerations. This book is edited by renowned surgeons and endoscopists from Brazil, a country known for its reputation on the field of bariatric endoscopy and surgery. Such reputation is especially valid where the use of intragastric balloons is concerned. These devices, approved by the FDA only in 2015, have been used in Brazil since the 1990s, attesting to Brazilian expertise in the field. This book describes the various types of intragastric balloons used for the treatment of obesity, an alternative to bariatric surgery and other more invasive methods. Morbid obesity is an epidemic as more than 2/3 of the United States population is obese and as such, has a high burden of weight-related co-morbid diseases. Bariatric surgery has proven to be effective and durable for treatment of severe obesity. Technological advances including applications of laparoscopy and endolumenal techniques have rapidly advanced this field. Data and outcomes examining treatments have also improved and as providers, we have a wide spectrum of therapeutic options to treat patients. As techniques and outcomes have evolved, access to a comprehensive yet focused resource regarding bariatric surgery is currently limited. The proposed textbook is designed to present a comprehensive and state-of the-art approach to the current and future status of Bariatric interventions, which has changed significantly since the first edition of the Manual. Updates in this version will include the rapidly expanding field of endoluminal bariatric procedures, with a focus on new devices and theories of mechanisms. New data regarding laparoscopic approaches to treat obesity, as well as improved longer-term data outcomes will be reviewed. Newer surgical approaches to treat metabolic disease and obesity are included, as well as proposed mechanisms of action and efficacy. Additional new sections include sections on the application of robotic technologies, special circumstances including transplantation and pregnancy, and telemedicine and social media in bariatric surgery. Sections will address the evolution in specific treatments available to patients, initial evaluation and selection of procedures for individual patients, the latest surgical and endoscopic techniques being employed to treat patients including data on outcomes, and future directions for therapy. In particular and unique amongst references, a major focus of this text will be on both the bariatric and metabolic bases of therapies and outcomes. The SAGES Manual A Practical Guide to Bariatric Surgery, Second Edition aligns with the new SAGES UNIVERSITY MASTERS Program. The Manual supplements the Bariatric Surgery Pathway from Competency to Proficiency to Mastery.
