Abstract
This book is an up-to-date reference on all aspects of anticoagulation and hemostasis in neurosurgery. After an opening section on basic principles and drug classes in current use, detailed consideration is given to coagulation issues relevant to all patients, not just neurosurgical ones. The coverage includes, for example, deep vein thrombosis, pulmonary embolism, and disseminated intravascular coagulation. A variety of important issues specific to neurosurgical practice are then addressed, and the book concludes with a summary of current guidelines and best practices. As the population ages and prophylactic anticoagulation for different cardiac disorders is validated by cooperative trials, it is becoming ever more important that practicing neurosurgeons have the requisite knowledge to manage optimally the opposing processes of anticoagulation and hemostasis. Nevertheless, only a modest amount of structured information is at present available on the subject, with the consequence that decision making is too often insufficiently informed. Aspirin has been in clinical use for over 100 years, making it one of the oldest commonly used drugs in medicine, and is now predominantly used as an antiplatelet agent. Major new antiplatelet drug classes have recently become available for the clinician that offer the promise of major improvements in the treatment and prevention of arterial thrombotic disease. In the field of anticoagulation, the vitamin K antagonists such as warfarin have not been available for as long as aspirin in the antiplatelet field, and have also had very little in the way of competition until recently; although a number of parenteral anticoagulants have been available, the vitamin K antagonists have been the only orally active anticoagulant drugs. The last few years have seen the arrival of new drug classes, the orally active direct thrombin inhibitors and factor Xa antagonists, which offer a number of advantages, but not without important drawbacks. In the face of these important advances in both antiplatelet and anticoagulant therapy, clinicians often feel confused, even overwhelmed, as to what is now available and which drugs are useful (and indicated) in which circumstances. Antiplatelet and Anticoagulant Therapy is designed with such people in mind. It is written in an easy-to-follow format and with many illustrations to aid clarity and the assimilation of information. Each chapter is written by established authorities in their fields who are also experienced in explaining often complex concepts. The result is a unique book which is not only comprehensive but also clear and useful for the busy medical practitioner.

There have been many changes in the field of coagulation during the past decade. New concepts of epidemiology of risk factors for thrombosis now help clinicians predict who is more likely to form clots after surgery, or after being placed on oral contraceptives. New anticoagulants have the potential to redefine how patients with atrial fibrillation and venous thrombosis are managed. There are new forms of recombinant clotting factors which have changed our approach to hypofibrinogenemia and von Willebrand's disease. Newer antiplatelet agents are available and their use in patients receiving cardiac stents has mushroomed. The management of thrombosis in the setting of pregnancy has changed over the past decade, as well as the way clinicians approach women with multiple miscarriages. An entire new class of compounds, the thrombopoietins, are available to treat individuals with immune thrombocytopenic purpura (ITP). The Coagulation Consult covers major topics of interest to hematologists who are asked to consult on individuals with coagulation related diseases, and encompasses the field's most recent developments. This "case-directed" book describes state-of-the-art approaches to patients with bleeding and clotting disorders, as well as laboratory tests for coagulation. Chapters include different vignettes, focus on typical clinical consult questions, and lay out specific types of treatment.
