Abstract
This book is an up-to-date reference on all aspects of anticoagulation and hemostasis in neurosurgery. After an opening section on basic principles and drug classes in current use, detailed consideration is given to coagulation issues relevant to all patients, not just neurosurgical ones. The coverage includes, for example, deep vein thrombosis, pulmonary embolism, and disseminated intravascular coagulation. A variety of important issues specific to neurosurgical practice are then addressed, and the book concludes with a summary of current guidelines and best practices. As the population ages and prophylactic anticoagulation for different cardiac disorders is validated by cooperative trials, it is becoming ever more important that practicing neurosurgeons have the requisite knowledge to manage optimally the opposing processes of anticoagulation and hemostasis. Nevertheless, only a modest amount of structured information is at present available on the subject, with the consequence that decision making is too often insufficiently informed. This book, written by recognized experts, is designed to rectify this situation by bringing together the latest knowledge from across the entire discipline. It will be of daily value for neurosurgeons and trainees worldwide and will also be of interest to emergency room physicians, surgeons in general, critical care physicians, neurologists, and hospital medicine specialists.

Abstract
Aspirin has been in clinical use for over 100 years, making it one of the oldest commonly used drugs in medicine, and is now predominantly used as an antiplatelet agent. Major new antiplatelet drug classes have recently become available for the clinician that offer the promise of major improvements in the treatment and prevention of arterial thrombotic disease. In the field of anticoagulation, the vitamin K antagonists such as warfarin have not been available for as long as aspirin in the antiplatelet field, and have also had very little in the way of competition until recently; although a number of parenteral anticoagulants have been available, the vitamin K antagonists have been the only orally active anticoagulant drugs. The last few years have seen the arrival of new drug classes, the orally active direct thrombin inhibitors and factor Xa antagonists, which offer a number of advantages, but not without important drawbacks. In the face of these important advances in both antiplatelet and anticoagulant therapy, clinicians often feel confused, even overwhelmed, as to what is now available and which drugs are useful (and indicated) in which circumstances. Antiplatelet and Anticoagulant Therapy is designed with such people in mind. It is written in an easy-to-follow format and with many illustrations to aid clarity and the assimilation of information. Each chapter is written by established authorities in their fields who are also experienced in explaining often complex concepts. The result is a unique book which is not only comprehensive but also clear and useful for the busy medical practitioner.

Abstract
There have been many changes in the field of coagulation during the past decade. New concepts of epidemiology of risk factors for thrombosis now help clinicians predict who is more likely to form clots after surgery, or after being placed on oral contraceptives. New anticoagulants have the potential to redefine how patients with atrial fibrillation and venous thrombosis are managed. There are new forms of recombinant clotting factors which have changed our approach to hypofibrinogenemia and von Willebrand's disease. Newer antiplatelet agents are available and their use in patients receiving cardiac stents has mushroomed. The management of thrombosis in the setting of pregnancy has changed over the past decade, as well as the way clinicians approach women with multiple miscarriages. An entire new class of compounds, the thrombopoietins, are available to treat individuals with immune thrombocytopenic purpura (ITP). The Coagulation Consult covers major topics of interest to hematologists who are asked to consult on individuals with coagulation related diseases, and encompasses the field's most recent developments. This "case-directed" book describes state-of-the-art approaches to patients with bleeding and clotting disorders, as well as laboratory tests for coagulation. Chapters include different vignettes, focus on typical clinical consult questions, and lay out specific types of treatment. Practicing clinicians being confronted with a coagulation consult, students, residents, fellows and attending physicians will find this unique text an invaluable resource for some of the newer areas of coagulation science, therapy and pharmacology.

Abstract
This book describes in detail the clinical presentation, diagnosis, and management of a wide range of congenital bleeding disorders. It will assist readers in overcoming the significant challenges involved in clinical and laboratory diagnosis and in providing effective clinical care that makes optimal use of new products, including recombinant factor concentrate. The coverage ranges from hemophilia A and B and von Willebrand disease to rare bleeding disorders such as congenital factor V, factor X, factor XI, and factor XIII deficiency and inherited platelet function disorders. The exceptional attention to rarer conditions is of particular importance given the considerable risk of overlooking them during diagnosis, with potential consequences for disease-related morbidity and mortality. The authors are acknowledged specialists in the field from across the world who have particular expertise in the disorder that they discuss. The book will be of value to hematologists, oncologists, pediatricians, laboratory specialists and technicians, general physicians, and trainees.

Since the very beginning of critical care medicine, much attention has been paid to the risk of bleeding, but only recently the risk of thrombosis has been recognized in critical patients; in fact, current laboratory investigationsáaimed to monitor blood coagulation are more focused on the assessment of the hemorrhagic than of the thrombotic risk. Since mid-90s, many studies have detected powerful pro-thrombotic actions of endogenous mediators in sepsis and other inflammatory conditions, includ…

Since mid-90s, many studies have detected powerful pro-thrombotic actions of endogenous mediators in sepsis and other inflammatory conditions, including trauma and postoperative status. Unfortunately, the related complication are both hard to detect and extremely harmful. The volume provides a full coverage of the hemocoagulative problems in anesthesia and intensive care, taking into consideration the physiology and monitoring of hemostasis, its relation with sepsis, and the specific aspects of the various critical conditions - from trauma to burns, to heart and transplant surgery or acute renal failure

Abstract
Coagulation testing is the basis for the diagnosis of bleeding and thrombotic disorders, as well as the mainstay of anticoagulant monitoring and management. This handbook provides practical information and guidance on topics relevant to directing a coagulation laboratory, filling a void in the literature. The book cites consensus recommendations of authoritative bodies whenever possible, and otherwise refers to reviews, chapters, and research articles from experts. Since the first edition, all chapters have been updated, and an entirely new chapter is included on pharmacogenomics and pharmacogenetics. Laboratory Hemostasis: A Practical Guide for Pathologists will aid pathologists, clinical laboratory scientists, and other physicians serving as laboratory directors to understand and carry out their responsibilities. It will also assist residents and fellows in learning the basics of coagulation testing and serve as a useful day-to-day reference for coagulation laboratory supervisors, technologists, and technicians. Finally, clinicians may find aspects of the book helpful in understanding the role of the coagulation laboratory in patient evaluation and monitoring.
Contents
Role and Responsibilities of the Laboratory Director -- Collection of Coagulation Specimens -- Instrumentation for the Coagulation Laboratory -- Validation of Hemostasis Assays, Analyzers, and Reagents -- Hemostasis Screening Assays -- Testing for Inherited Bleeding Disorders -- Testing for Acquired Platelet Disorders -- Acquired Coagulation Disorders and TTP -- Testing for Inherited and Acquired Thrombotic Disorders -- Monitoring of Anticoagulant Therapy -- Pharmacogenetic Testing for Anticoagulant and Antiplatelet Therapies -- Coagulation Testing and Transfusion Medicine.
