Abstract
This book addresses the unique healthcare needs of adults with chronic childhood illnesses. It presents a model of primary and secondary prevention for emerging adulthood: primary prevention in which all young adults are screened for high-risk behaviors and health needs and secondary prevention in which young adults with chronic childhood conditions are optimized through coordinated care, connections to community resources and social/family support. This book is organized in five parts. Part I provides a detailed overview of the health care transition from pediatrics to adult medicine from both a policy and practice perspective. In Part II, the concept of emerging adulthood as a developmental period is explored and strategies for providing improved comprehensive care for this age group are discussed. Part III reviews specific chronic childhood conditions, such as attention-deficit/hyperactivity disorder, autism, cystic fibrosis, and diabetes mellitus, and offers clinical cases and summary reports that can be used as a quick guides to each condition. In Part IV, additional clinical considerations that are not necessarily condition-specific but are highly relevant to the care of young adults with chronic childhood conditions are examined. Part V describes the socio-legal issues involved in caring for this population. Care of Adults with Chronic Childhood Conditions provides primary care providers with a new framework for the care of young adults and identifies opportunities to influence patient health outcomes over a life trajectory.

Abstract
This text outlines the importance of biopsychosocial factors in improving medical care, and illustrates evidence-based, state-of-the-art interventions for patients with a variety of medical conditions. Each chapter is focused on a particular health concern or illness, which is described both in terms of prevalence and frequent psychological and psychiatric comorbidities that may present to clinicians working with these populations. Consistent with evidence-based care, information on the efficacy of the treatments being described is presented to support their continued use. To accommodate the needs of clinicians, we describe population specific approaches to treatment, including goal settings, modules and skills as well as strategies to assess and monitor progress. To facilitate learning, each chapter contains one or more case examples that explicate the skills described to convey change within a behavioral medicine protocol. Each chapter also includes resources in the form of books and websites to gain additional knowledge and detail as needed. Palliative Care for Chronic Cancer Patients in the Community: Global Approaches and Future Applications offers a much-needed account of the current state of global palliative care in the community – the good, the bad and, especially, the hopeful. This extraordinary compendium, with chapters form 46 countries, edited by Professor Michael Silbermann, tells the remarkable stories of palliative care - from grass-root efforts by volunteers in resource-constrained countries such as India to the full integration of palliative care in high-resource countries such as the United States, the United Kingdom, New Zealand and Australia. What is so unique about this book is that it captures the enormous range and diversity of palliative care efforts: some led by nurses (Iran), some led by governments (Turkey) and many led by local charities and non-governmental agencies. Readers of this textbook can also learn about the challenges of a government-led palliative care initiative in the post-genocide era in Rwanda or the attempts to provide palliative care services in the conflict zones of Iraq, Palestine and Sudan. Contextual issues requiring cultural sensitivity in Afghanistan, Latin America and the Middle East present a real picture of both the exceptional leadership and environmental hardships that palliative care advocates face in their journeys forward. A practical, highly illustrative and easy to read reference for interventional cardiologists, vascular surgeons, cardiac/endovascular catheterization laboratory personnel, interventional radiologists, technical staff, industry professionals, investigators, and researchers on peripheral arterial CTO interventions. Peripheral CTO lesions are present in nearly 50% of all peripheral arterial disease (PAD) endovascular interventions. There is tremendous growth in the performance and learning of peripheral arterial CTO procedures. There are several ongoing physician courses and many others that are being prepared; however, there is a significant lack of standardized selection, strategies, techniques and equipment necessary to achieve a high degree of reproducible success with low complication rates during the performance of these complex and technically challenging endovascular procedures. The book is not an academic tour de force description of the topic but rather a practical one with many illustrations and step-by-step explanations of the various procedure steps, what can go wrong, and troubleshooting.
