Abstract
This book is unique in providing a comprehensive overview of the human factors issues relevant to patient safety during acute care. By elucidating the principles of human behavior and decision-making in critical situations and identifying frequent sources of human error, it will help healthcare professionals provide safer, more effective treatment when dealing with emergencies characterized by uncertainty, high stakes, time pressure, and stress. The third edition has emerged from an ongoing synergistic relationship between clinicians and behavioral scientists on both sides of the Atlantic to update and enhance each chapter -- blending the strengths of the two professions into a readily accessible text. Among other improvements, readers will find sharper articulation of concepts and significantly more information on the organizational impact on individual and team performance. Crisis Management in Acute Care Settings is the required reference for all who are learning about, teaching, or providing acute and emergency healthcare. It will be of high value for undergraduate and graduate medical and nursing program and offer a much-needed resource for those who use high-fidelity healthcare simulation to teach teamwork.

Abstract
Stroke is a leading cause of death and disability throughout the world. Intravenous lytics and endovascular stroke treatment were demonstrated to be highly beneficial in clinical trials and in achieving net cost savings. However, time is of the essence in the management of stroke as for every minute that stroke is left untreated, an estimated 1.9 million neurons are destroyed. An optimized stroke pathway can dramatically impact patient outcome. Every medical service will differ. This book offers a concise and practical guide on how to set up and efficiently run an acute stroke service tailored to the needs of the individual center as a provider of primary, secondary, or tertiary care. It documents the essential knowledge on current imaging and treatment options and offers guidance in organizing a site-specific stroke pathway that will ensure the quickest possible provision of treatment, thereby optimizing benefit to the patient and maximizing cost-effectiveness. The authors played an integral part in the development and operation of one of Europe's most successful early stroke centers and also have experience of the factors that can undermine the creation of world-class stroke units. On this basis, they address the key aspects of the stroke pathway and explain how organizing this can reduce the number of severe outcomes by ensuring that appropriate steps are taken during the early moments after stroke when intervention can make a profound difference. The challenging, multidisciplinary nature of care is emphasized, and this handbook will be an invaluable source for every member of the stroke team, including doctors, nurses, stroke coordinators, emergency and rehabilitation providers as well as hospital administrators.

Abstract
The origin of modern intensive care units (ICUs) has frequently been attributed to the widespread provision of mechanical ventilation within dedicated hospital areas during the 1952 Copenhagen polio epidemic. However, modern ICUs have developed to treat or monitor patients who have any severe, life-threatening disease or injury. These patients receive specialized care and vital organ assistance such as mechanical ventilation, cardiovascular support, or hemodialysis. ICU patients now typically occupy approximately 10% of inpatient acute care beds, yet the structure and organization of these ICUs can be quite different across hospitals. In The Organization of Critical Care: An Evidence-Based Approach to Improving Quality, leaders provide a concise, evidence-based review of ICU organizational factors that have been associated with improved patient (or other) outcomes. The topics covered are grouped according to four broad domains: (1) the organization, structure, and staffing of an ICU; (2) organizational approaches to improving quality of care in an ICU; (3) integrating ICU care with other healthcare provided within the hospital and across the broader healthcare system; and (4) international perspectives on critical care delivery. Each chapter summarizes a different aspect of ICU organization and targets individual clinicians and healthcare decision makers. A long overdue contribution to the field, The Organization of Critical Care: An Evidence-Based Approach to Improving Quality is an indispensable guide for all clinicians and health administrators concerned with achieving state-of-the-art outcomes for intensive care.

Organizing Intensive Care -- Organizational Change in Critical Care: The Next Magic Bullet? -- Origins of the Critically Ill: The Impetus for Critical Care Medicine -- Intensivist and Alternative Models of ICU Staffing -- Health Professionals in Critical Care -- Computers in Intensive Care -- Integrating Subspecialty Expertise in the Intensive Care Unit -- Part II. Improving Intensive Care -- Quality Improvement in the Intensive Care Unit -- Facilitating Interactions Between Healthcare Providers in the ICU -- Teamwork and Leadership in the Critical Care Unit -- Caring for ICU Providers -- Part III.
